Three-dimensional Mapping Analysis of Talus Fractures and Demonstration of Different Surgical Approaches for Talus Fractures.

OBJECTIVE: The talus is an important component in the ankle, and its treatment after injury is crucial. However, complications and adverse events due to incomplete traditional classifications may still occur, and these classifications fail to analyze the patterns and distribution of fractures from a three-dimensional perspective. Therefore, in this study, we aimed to analyze the location and distribution of fracture lines in different types of talus fractures using three-dimensional (3D) and heat mapping techniques. Additionally, we aimed to determine the surface area of the talus that can be

Designing and additive manufacturing of talus implant for post-traumatic talus avascular necrosis: a case study.

New technologies in additive manufacturing and patient-specific CT-based custom implant designs make it possible for previously unimaginable salvage and limb-sparing operations a practical reality. This study presents the design and fabrication of a lattice-structured implant for talus replacement surgery. Our primary case involved a young adult patient who had sustained severe damage to the talus, resulting in avascular necrosis and subsequent bone collapse. This condition caused persistent and debilitating pain, leading the medical team to consider amputation of the left foot at the ankle le

Atypical Talus Fracture: A Case Report.

INTRODUCTION: Talus fractures are relatively rare injuries. These injuries are very serious because they can compromise the motion of foot and ankle and result in severe disability. Fractures of talar neck and body constitutes about 40% of all talus fractures. These occur secondary to high energy trauma most commonly motor vehicle collisions or fall from height. Fractures involving the lateral process results due to ankle inversion and dorsiflexion. Majority of talar surface is covered by articular cartilage and its role in force transmission between leg and foot makes successful treatment of

Congenital Vertical Talus: Etiology and Management.

Congenital vertical talus is a rare foot deformity. If left untreated, it causes significant disability, including pain and functional limitations. Although the etiology of vertical talus is likely heterogeneous, recent evidence strongly supports a genetic cause linking it to genes expressed during early limb development. Traditional management for vertical talus involves extensive surgeries that are associated with significant short- and long-term complications. A minimally invasive approach that relies on serial manipulation and casting to achieve most of the correction has been shown to pro

Primary aneurysmal bone cyst of talus.

Aneurysmal bone cyst (ABC) of the talus is an extremely rare lesion; less than 20 cases have been reported in PubMed till 2012. We report a primary ABC of the talus in a 20-year-old male that was managed by extended intralesional curettage with phenol as an adjuvant and autologous cancellous iliac crest bone grafting. The patient had excellent functional outcome and there was no recurrence at 2 years of follow-up.

Mechanical properties of human fetal talus.

Mechanical characterization of human cartilage anlagen is required to effectively model congenital musculoskeletal deformities. Such modeling can effectively explore the effect of treatment procedures and potentially suggest enhanced treatment methods. Using serial MRI, we have noted shape changes of the cartilaginous hindfoot anlagen in patients with clubfoot, suggesting they are soft and deformable. We therefore determined the stress relaxation behavior of cartilage plugs obtained from third-trimester stillborn fetuses in unconfined and confined compression geometries. The material parameter

Ballistic talus fractures: a retrospective analysis.

INTRODUCTION: Ballistic talus fractures are difficult to treat and there is a paucity of literature regarding the subject. The goal of the current study is to outline our experience and epidemiological findings from a single center with a large case series of ballistic talus fractures. MATERIALS AND METHODS: Institutional Review Board approval was obtained for a retrospective review of skeletally mature patients with ballistic talus fractures from August 2019 to June 2023. Fracture morphology of the talus, the presence of displacement, and the talar declination angle (TDA) were all characteri

Congenital Vertical Talus: An Updated Review.

Congenital vertical talus (CVT) is the presence of rigid flatfoot deformity characterized by hindfoot valgus and equinus. This foot deformity is associated with midfoot dorsiflexion and forefoot abduction due to a fixed dorsal dislocation of the navicular relative to the head of the talus. It is often underdiagnosed in children due to its similarity to other disorders of the foot. Misdiagnosis of CVT and subsequent failure to address it leads to significant disability and pain. While past surgical management consisted of soft tissue releases that produced varying efficacy, current management o

Acute and chronic osteochondral talus lesions.

Osteochondral lesions of the talus are often not or delayed diagnosticated. They mostly are related to traumatic lesions of the upper ankle joint, where by traumatic distorsion are most common among. The degenerative change classified as osteochondrosis dissecans tali shows in the most cases also an originally traumatic generic. The classification by osteochondrosis dissecans tali in four stages by Berndt and Harty is nowadays accepted. These can be distinguished by means of several diagnostic methods. They are regarded as state-of-the-art of therapy and prognosis. Basically for diagnostic pur

[Late results of central talus injuries].

From 1982 to 1992 we treated 66 talar injuries at our institution, 44 of which were central fractures (25 talar neck fractures, 17 corpus fractures, 1 comminuted fracture and 1 complete dislocation of the intact talus). In 77% of patients additional injuries were present; 34% were polytraumatized. In 41 patients (93%) clinical and radiological follow-up was possible after an average of 6.3 years. All displaced fractures were treated by open reduction (84%) and, usually, screw fixation, 41% of these within 6 h after the accident. Only 1 required an osteotomy of the medial malleolus, and in 4 a

[Injury analysis of pediatric talus fractures].

Fractures of the talus are very rare in children. The present paper analyzes the treatment and long-term follow-up of 11 talus fractures in children over a period of 20 years (1974-1994). There were 5 fractures of the talus neck, 3 flake fractures, 1 depressed fracture of the talus head, and 2 lesions at the trochlea of the talus. Two fractures not previously described in the literature are presented. Treatment was conservative in all nondisplaced fractures. The displaced lesions were treated by compression screws and K-wire fixation of articular surface fractures. In 9 of the 11 fractures hea

Enigma of pediatric vertical talus deformity.

Vertical talus is an unusual pediatric deformity that often is unrecognized at birth. Its treatment is necessarily surgical and not typically addressed by the podiatric medical profession. In this case report, the enigmatic deformity is reviewed, along with a detailed photographic assay of useful techniques for its surgical reduction.

Surgical Treatment for Osteochondral Lesions of the Talus.

Osteochondral lesions of the talus (OLT) are often associated with ankle pain and dysfunction. They can occur after ankle trauma, such as sprains or fractures, but they usually present as a continued ankle pain after the initial injury has resolved. Chronic ankle ligament instability and subsequent microtrauma may lead to insidious development of an OLT. Medial-sided lesions are more common (67%) than lateral-sided lesions. For acute lesions that are nondisplaced, nonoperative management is initially performed, with a 4-6 week period of immobilization and protected weight bearing. Symptomatic

Avascular Necrosis of the Talus After Subchondroplasty.

BACKGROUND: Subchondroplasty (SCP) is a relatively new procedure, developed in 2007 for the treatment of bone marrow lesions (BMLs), that has shown promising results in the knee through several different case series. The foot and ankle literature, however, is sparse, with only a few documented case reports or case series. At our institution, we have identified several patients with complications after this specific procedure. As a result, we report our case series of patients who developed talar avascular necrosis (AVN) after undergoing SCP. METHODS: A retrospective review was performed of pa

Evaluation and Management of Osteochondral Lesions of the Talus.

Osteochondral lesions of the talus are common injuries that affect a wide variety of active patients. The majority of these lesions are associated with ankle sprains and fractures though several nontraumatic etiologies have also been recognized. Patients normally present with a history of prior ankle injury and/or instability. In addition to standard ankle radiographs, magnetic resonance imaging and computed tomography are used to characterize the extent of the lesion and involvement of the subchondral bone. Symptomatic nondisplaced lesions can often be treated conservatively within the pediat

Spontaneous osteonecrosis of navicular and talus (SONNT).

We describe a rare case of simultaneous and spontaneous osteonecrosis affecting bilateral navicular and talar bones in a young male. On both sides, there was associated symptomatic talonavicular and naviculocuneiform arthritis. The more symptomatic left foot was managed successfully by talonavicular-cuneiform (TNC) arthrodesis and core decompression of talar body. A probable cause of this rare disorder was most likely due to a developmental anomaly in the preaxial ray of the foot. As no literature is currently available for this rare condition, our case report is intended to provide a referral
