Trichiasis and disability in a trachoma-endemic area of Tanzania.

OBJECTIVE: To measure limitations in the daily activities of village life associated with having trichiasis for individuals with and without visual acuity loss. METHODS: Men and women 40 years and older in 6 randomly chosen rural villages in the Kongwa district of Tanzania had visual acuity measured and were examined by an ophthalmologist. Subjects indicated the degree of difficulty with daily activities of village life and whether the difficulty was related, in any way, to vision. Limitations were scored using an indicator of "any difficulty" and using a 4-point scale ranging from "no diffic

Autotransfusion and ectopic pregnancy: an experience from Tanzania.

Intraoperative autotransfusion was performed in 48 of 90 women with ectopic pregnancy, seen between 1990 and 1995. Autotransfusion was performed more frequently in the presence of hypovolaemic shock and more severe haemoperitoneum. The procedure appeared safe and useful. Contraindications and complications are discussed.

The efficacy of amitraz against cattle ticks in Tanzania.

The efficacy of amitraz on cattle ticks was assessed by susceptibility tests, spraying and dipping trials. Tests on the susceptibility of three tick species, Rhipicephalus appendiculatus, Amblyomma variegatum and Boophilus decoloratus to amitraz (technical grade-purity 98.6% w/w) showed all 15 tick strains tested to be highly susceptible, with LC50 ranging between 0.001% and 0.03%, and LC99, between 0.01% and 0.07%. Spraying trials were carried out on calves infested with R. appendiculatus, A. variegatum and B. decoloratus. Amitraz wettable powder [as Taktic total replacement (TR)] was tested

Traditional eye medicines and corneal ulceration in Tanzania.

A one-year prospective study into the aetiology of corneal ulceration in 103 patients attending Mvumi hospital, Tanzania, showed 25% (95% CL 16.5-33.5%) of ulcers to be associated with the use of traditional eye medicines (TEM) within the previous 7 days. There was no statistically significant difference between TEM users and non-users in terms of sex of the patients, season of presentation or age at presentation. Of 26 corneal ulcers in TEM users, 58% (n = 15) had no other identified cause of ulceration apart from TEM use. Of the remaining 11, eight showed the appearances of HSV keratitis, an

The conjoined twins of Shinyanga, Tanzania: case report.

The first recorded set of conjoined twins in Tanzania was successfully separated on the 10th of February 1994 at the Muhimbili Medical Centre (MMC) in Dar es Salaam. The male omphalopagus twins were born to a 22-year-old para 1 gravida 2 mother by normal vaginal delivery at a Rural Health Centre in Shinyanga Region two months earlier, and had been transferred to the National Referral Hospital at the age of two weeks. At the age of about four months, the first twin died following multiple complications which necessitated several re-operations, while the second twin is still thriving and has bee

Measures to reduce the infant mortality rate in Tanzania.

Tanzanian health problems reflect those in other developing countries where the standard of living is low and housing and sanitation are inadequate. The major cause of infant mortality can be attributed to preventable diseases such as gastroenteritis, acute respiratory infections and malnutrition. In spite of the fact that various efforts have been made to extend primary health care coverage, particularly in rural areas, the scarcity of economic resources impedes the implementation of many health programmes. However, only by maintaining primary health care as a major part of the country's deve

Prevalence and causes of vision loss in central Tanzania.

A population-based survey of the prevalence of major blinding disorders was conducted in three villages in central Tanzania. Overall, 1827 people over the age of seven years old were examined. In those age seven and older, the prevalence of bilateral blindness (visual acuity in the better eye of less than 3/60) was 1.26% and monocular blindness (visual acuity of less than 3/60 in one eye) was 4.32% and the prevalence of visual impairment (visual acuity less than 6/18 but greater than or equal to 3/60 in both eyes was 1.04% and in one eye was 1.75%. Corneal opacities were responsible for 44% of

Multiple pregnancy as a risk factor in rural Tanzania.

The prevalence of twin pregnancies in two rural hospitals in the South Western Highlands of Tanzania ranged from 15 to 20 per 1,000 births. The literature on the generally assumed high twinning rates in the whole of Africa is reviewed. High prevalence figures can be the result of hospital selection, as most African studies are hospital-based. Only from Nigeria, one population-based study revealed a rate of 45 per 1,000. In both hospitals perinatal mortality in twin births was strikingly low, especially when birthweight was at least 1,500 g: between 30 and 40 per 1,000 births. It is argued that

Corneal ulceration, measles, and childhood blindness in Tanzania.

One hundred and thirty Tanzanian children with corneal ulceration were clinically examined to determine the cause of the ulceration. 37% of the ulcers were associated with recent measles infection and 38% of the children had bilateral ulceration. Herpes simplex virus infection was the commonest cause of ulceration in the series, but vitamin A deficiency was the major cause of bilateral ulceration, subsequent blindness, and mortality in this series. Other significant causes of childhood corneal ulceration were the use of traditional eye medicines, confluent measles keratitis, and ophthalmia neo

The impact of colonialism on health and health services in Tanzania.

This article is about the problems of malnutrition and disease in a rural area of an underdeveloped country, Tanzania. The particular way in which health problems were conceptualized during the colonial era, the structure of the medical services established, and the effects of health care on the health status and size of the rural population of Songea District in Tanzania are shown in the article to have been determined by the economic, social, and political requirements of German and British colonial rulers rather than by the health needs of the African population. Colonial economic policy em

Widespread Treponema pallidum Infection in Nonhuman Primates, Tanzania.

We investigated Treponema pallidum infection in 8 nonhuman primate species (289 animals) in Tanzania during 2015-2017. We used a serologic treponemal test to detect antibodies against the bacterium. Infection was further confirmed from tissue samples of skin-ulcerated animals by 3 independent PCRs (polA, tp47, and TP_0619). Our findings indicate that T. pallidum infection is geographically widespread in Tanzania and occurs in several species (olive baboons, yellow baboons, vervet monkeys, and blue monkeys). We found the bacterium at 11 of 14 investigated geographic locations. Anogenital ulcera

High seroprevalence for typhus group rickettsiae, southwestern Tanzania.

Rickettsioses caused by typhus group rickettsiae have been reported in various African regions. We conducted a cross-sectional survey of 1,227 participants from 9 different sites in the Mbeya region, Tanzania; overall seroprevalence of typhus group rickettsiae was 9.3%. Risk factors identified in multivariable analysis included low vegetation density and highway proximity.

Histoplasmosis among hospitalized febrile patients in northern Tanzania.

Histoplasmosis may be common in East Africa but the diagnosis is rarely confirmed. We report 9 (0.9%) cases of probable histoplasmosis retrospectively identified among 970 febrile inpatients studied in northern Tanzania. Median (range) age was 31 (6, 44) years, 6 (67%) were female, 6 (67%) HIV-infected; 7 (78%) were clinically diagnosed with tuberculosis or bacterial pneumonia. Histoplasmosis is an important cause of febrile illness in Tanzania but is rarely considered in the differential diagnosis. Increased clinician awareness and availability of reliable diagnostic tests may improve patient

Leptospirosis among hospitalized febrile patients in northern Tanzania.

We enrolled consecutive febrile admissions to two hospitals in Moshi, Tanzania. Confirmed leptospirosis was defined as a ≥ 4-fold increase in microscopic agglutination test (MAT) titer; probable leptospirosis as reciprocal MAT titer ≥ 800; and exposure to pathogenic leptospires as titer ≥ 100. Among 870 patients enrolled in the study, 453 (52.1%) had paired sera available, and 40 (8.8%) of these met the definition for confirmed leptospirosis. Of 832 patients with ≥ 1 serum sample available, 30 (3.6%) had probable leptospirosis and an additional 277 (33.3%) had evidence of exposure to pathogeni

Controlling postpartum hemorrhage after home births in Tanzania.

OBJECTIVES: Determine safety of household management of postpartum hemorrhage (PPH) with 1000 microg of rectal misoprostol, and assess possible reduction in referrals and the need for additional interventions. METHODS: Traditional birth attendants (TBAs) in Kigoma, Tanzania were trained to recognize PPH (500 ml of blood loss). Blood loss measurement was standardized by using a local garment, the "kanga". TBAs in the intervention area gave 1000 microg of misoprostol rectally when PPH occurred. Those in the non-intervention area referred the women to the nearest facility. RESULTS: 454 women in
