[A not-so-typical ankle sprain].

BACKGROUND: An estimated hundreds of thousands of patients are treated in the Netherlands every year for an ankle sprain. We describe two cases where the course of an ankle sprain differed from the usual presentation. CASE PRESENTATION: Case 1: A 22-year-old male presented to the Emergency Department following an inversion sprain to the ankle. The pain and swelling were localized around the subtalar joint rather than the lateral ankle ligament(s), with limited range of motion in the subtalar joint. Imaging revealed a fracture through a tarsal coalition. The patient was treated with cast immob

GDF6 Knockdown in a Family with Multiple Synostosis Syndrome and Speech Impairment.

Multiple synostoses syndrome type 4 (SYNS4; MIM 617898) is an autosomal dominant disorder characterized by carpal-tarsal coalition and otosclerosis-associated hearing loss. SYSN4 has been associated with GDF6 gain-of-function mutations. Here we report a five-generation SYNS4 family with a reduction in GDF6 expression resulting from a chromosomal breakpoint 3' of GDF6. A 30-year medical history of the family indicated bilateral carpal-tarsal coalition in ~50% of affected family members and acquired otosclerosis-associated hearing loss in females only, whereas vertebral fusion was present in all

Cuboid dislocations: Short and long term results and a review of the literature.

Isolated cuboid dislocation is rare, with only 7 cases described in the literature. We describe three cases, two in professional rugby league players. The mechanism of injury was the same in the rugby league players with an external rotation injury to the midfoot during a tackle. The dislocation is easily missed on X-rays and CT would appear to be the investigation of choice. None of the dislocations could be reduced closed and all underwent open reduction and wire stabilisation. The wires were removed at 6 weeks. The short and long term functional results were good. Both rugby league players

The first metatarsocuneiform joint, hypermobility, and hallux valgus: what does it all mean?

The etiology and treatment of the hallux valgus deformity has long been a subject of controversy. Early reports focused on increased first metatarsocuneiform joint mobility as the primary cause of the deformity. While this theory was widely accepted, little evidence was offered in its support. Recent reports have provided objective evidence calling into question the notion of hypermobility. While flexibility in the first metatarsocuneiform joint is necessary for hallux valgus to develop, increased mobility is the result rather than the cause of the deformity.

SURGICAL MANAGEMENT OF A TARSAL LUXATION IN A RED KANGAROO (OSPHRANTER RUFUS).

Joint luxations commonly occur in animals secondary to traumatic injury. Because of the unique hind-limb anatomy of macropods, surgical stabilization of orthopedic injuries is considered challenging, and reports of successful management are limited. A 4-yr-old male neutered red kangaroo (Osphranter rufus) presented with a dorsolateral luxation of the left tibiotarsal joint. Surgical reduction and tarsal arthrodesis were performed. Although the full range of motion of the tarsal joint was limited, this kangaroo was still able to ambulate normally at slow speeds following surgery and recovery. T

Synovitis of the Tarsal Canal: An Uncommon Cause of Lateral Heel Pain After Triple Arthrodesis.

Lateral heel pain after triple arthrodesis can have numerous causes, including nonunion of the fusion site, hindfoot malalignment, degenerative arthritis in adjacent joints, and deep infection. We present a case of lateral heel pain after triple arthrodesis due to synovitis of the tarsal canal that was successfully treated with anterior subtalar arthroscopy.

Ultrasonography of the normal donkey tarsus (equus asinus).

Tarsal joint illness is a frequent source of hind limb lameness due to the complex anatomy of the region and the presence of numerous bony and soft tissue structures. Proper lameness diagnosis aims to discover the structure provoking lameness. Ultrasonography documents valuable information of soft tissues and characterizes soft tissue injuries that have heretofore been difficult to obtain either noninvasively or via radiography. The objectives of the current study were to develop and describe a standardized ultrasonographic protocol for investigation of the tarsal region in donkeys. The donkey

The Incidence of Nonunion of the Naviculocuneiform Joint Arthrodesis:A Systematic Review.

Naviculocuneiform (NC) joint arthrodesis is an effective procedure to treat pain and provide stability to the medial column. Various forms of fixation have been described for NC arthrodesis. Despite this, the available literature is scant and questions remain regarding nonunion rate and contributory factors. A systematic review of the literature was undertaken to determine the rate of nonunion for NC joint arthrodesis. Seven studies involving 139 NC joint arthrodeses met inclusion criteria. The nonunion rate was 6.5% at a weighted mean follow-up of 73.2 months. There is insufficient evidence t

Novel FGF9 variant contributes to multiple synostoses syndrome 3.

Multiple synostoses syndromes (SYNS) are autosomal dominant syndromes characterized by multiple joint fusions commonly involving the carpal-tarsal, interphalangeal, humeroradial, and cervical spine joints. They display genetic heterogeneity with pathogenic variants reported in four separate genes (NOG, GDF5, FGF9, and GDF6) defining four different SYNS forms. FGF9 variants have been reported in SYNS3, a SYNS with multiple synostoses, normal cognition, normal hearing, and craniosynostosis. Here, we report a novel FGF9 c.569G > C p.(Arg190Thr) variant identified by whole-exome sequencing in a pa

Prevalence of Juvenile Osteochondral Conditions in Yearling Thoroughbred Racehorses in Southern Brazil.

This study aimed to investigate the prevalence of juvenile osteochondral conditions (JOCCs) in yearling Thoroughbred (TB) racehorses in Southern Brazil, as well as to examine the same animals studied as weanlings by Bastos et al. (2017) and compare the radiographic findings for the two ages. Radiographs of 76 male and female TB yearlings from Paraná State, Southern Brazil, were investigated. The proximal interphalangeal, metacarpophalangeal/metatarsophalangeal (MC/MT), tarsal, and femorotibial (FT) joints were evaluated using 24 radiographic projections. The evaluation consisted of a severity

Surgical treatment of an osteochondral lesion associated with stress fracture of the tarsal navicular: a case report.

We surgically treated an osteochondral lesion associated with a stress fracture of the tarsal navicular. The surgical procedure involved the confirmation and complete resection of the lesion under direct vision, followed by the transplantation of block-shaped iliac bone grafts. The postoperative computed tomography scan showed that the lesions had disappeared, the grafted bone had fused, and the stress fracture had healed. However, the tarsal navicular joint surface was slightly irregular. The patient was able to resume her sports activities 15 weeks after surgery. We have described a novel me

Chopart fractures and dislocations.

Fractures and dislocations of the midfoot and Chopart complex are among the most difficult foot injuries to manage. The treating surgeon is faced with a wide array of treatment challenges. Plain radiographs often grossly underestimate the extent of injury. The anatomy in this region of the foot is quite intricate with numerous articulations. Fractures can occur in isolation or as part of a more complex injury pattern. Misdiagnosis and under treatment can lead to severe alterations of both normal anatomy and function. This article discusses the rationales and techniques for treating these diffi

Anomalous tibial nerve. A case report.

A rare anomaly of a nonterminating tibial nerve is reported. It was found to be associated with tarsal tunnel syndrome. The nerve failed to divide in the tarsal tunnel and in the talocalcaneal canal distally to the Knot of Henry. The branches of the medial and lateral plantar nerves as well as the medial calcaneal nerve, the nerve to the abductor digiti quinti, and motor branches to the abductor hallucis and to the joints, vessels, and skin took origin directly from the tibial nerve. The presence of this nerve anomaly and its relevance to surgery in the posterior ankle region is discussed. Sur
