Otosclerosis and Dysplasias of the Temporal Bone.

Many bone dysplasias, some common and others rare, may involve the temporal bone causing conductive, sensorineural, or mixed hearing loss, vestibular dysfunction, or skull base foraminal narrowing, potentially affecting quality of life. Some conditions may affect only the temporal bone, whereas others may be more generalized, involving different regions of the body. High-resolution computed tomography may detect subtle osseous changes that can help define the type of dysplasia, and MR imaging can help define the degree of activity of lesions and potential associated complications.

[Modern imaging of the temporal bone].

This article describes the current significance of computed tomography (CT), magnetic resonance imaging (MRI), cone beam CT, digital subtraction angiography (DSA), and special X‑rays in the diagnostics of temporal bone diseases. The latter is obsolete for diagnostic intentions. Possibilities and limitations in terms of detection and/or depiction of the extent of inflammatory, traumatic, tumorous, and postoperative pathologies are discussed. A concrete question and conveyance of clinical findings influence the choice of the method to be applied in the individual case. Malformations of the middl

Schneiderian papilloma of the temporal bone.

Temporal bone Schneiderian papilloma may present as a primary tumour originating from the middle ear and mastoid process, or an extension from sinonasal disease. Both forms are rare, this being only the 18th case of primary temporal bone Schneiderian papilloma described to date. Although the current patient has remained disease free after excision of the papilloma, the reported recurrence rate is high, comparable to sinonasal Schneiderian papilloma with extrasinus extension. Malignant progression of primary Schneiderian papillomas is significantly reduced as compared to Schneiderian papillomas

[Fibrous dysplasia of the temporal bone].

Fibrous dysplasia of the temporal bone is a rare disease which may lead to progressive stenosis of the external auditory canal with conductive hearing loss. Sensorineural hearing loss has usually been attributed to involvement of the otic capsule or inner auditory canal. The case of fibrous dysplasia of the temporal bone in 11 years old girl was described. The conductive hearing loss with sensorineural component improved after surgical treatment.

Rare osteodysplasia of the temporal bone.

Temporal bone osteodysplasia can produce many different symptoms, such as involvement restricted to the temporal bone or impairment of other bones. We consider, in this study two entities that are rare osteodysplasia cases, which are osteopetrosis and Camurati-Engelmann disease, the latter being extremely rare. We present two cases of benign form of osteopetrosis (Albers-Schulenburg's disease), a patient of 11 years old and another one of 48 years old, both male, and a patient of 28 years old, female, with Camurati-Engelmann's disease. The facial palsy was a manifestation in two of the patient

Gunshot injuries to the temporal bone.

The current incidence of missile injury to the temporal bone (MITB) is very low in the United Kingdom. However, the increasing frequency of firearm violence in Britain suggests a greater risk of occurrence. This, along with the devastating potential sequelae of MITB (facial palsy, dead ear, intracranial damage, major vascular injury and cosmetic disfigurement), requires otolaryngologists to be conversant with all aspects of their management. The risk of major complications is much higher with MITB than with temporal bone injury following blunt trauma, and surgical management is, therefore, muc

Segmental anatomy of the temporal bone.

More of us suffer from hearing loss than from visual impairment, coronary artery disease, and cancer combined. Auditory dysfunction is detected in less than one third of newborns afflicted and is underdiagnosed in the elderly. Despite biomedical and technological advances, our understanding of vestibular dysfunction has grown little. Forming part of the lateral and inferior surfaces of the skull, the temporal bone contains the organs of hearing and balance. Diagnosis of diseases in these important regions requires adequate clinical information and a thorough knowledge of normal temporal bone a

[Glomus tumor of the temporal bone].

A case story describing the typical symptoms and course of a glomus tumour of the temporal bone is presented. The most frequent symptoms are pulsatile tinnitus, unilateral hearing loss, aural fullness and paresis of the vagal nerve or other lower cranial nerves. The tumour is frequently visible by otoscopy. Despite being histologically benign, the tumour is infiltrative and may affect the surrounding cranial nerves or spread into the cranial cavity. The early signs and findings are vague. Since the sequelae are fewer when the tumour is treated while it is small, an increased awareness will be

Verrucous carcinoma of the temporal bone.

A case report of a patient suffering from severe otalgia for 12 months and intermittent otorrhoea over a 53 year period is presented. Diagnosed as verrucous carcinoma of the temporal bone, this is only the ninth case found in the literature. The difficulty of histological diagnosis and subsequent management are important features.

The temporal bone in multiple myeloma.

Multiple myeloma is a malignant disease of the skeleton. Anatomically, any bone may ultimately come to be involved in a given case. In a literature review, no published series of temporal bone findings have been reported. In this study, oto-histopathologic changes associated with multiple myeloma are analyzed and reported in 15 temporal bones from 8 patients. The bone marrow of 13 temporal bones was involved by the tumor. Osteolysis was evident in 11 temporal bones. Serous otitis media (SOM) or purulent otitis media (POM) was seen in 13 ears with 12 showing mastoid effusions. Eight ears exhibi

Dural defects of the temporal bone.

Cerebrospinal fluid leakage (otorrhea or otorhinorrhea) from the temporal bone is the end result of rupture of the arachnoid membrane or herniation of the brain through a defect in the protective dura mater and calvarium. The rupture may be small, admitting only a herniation of arachnoid (meningocele), or be large enough to accommodate brain tissue (encephalocele). Flow of cerebrospinal fluid through either type of fistula may be a trickle or profuse, chronic or intermittent, and usually ceases temporarily for a few weeks following an attack of meningitis. The etiology, anatomy, signs and symp

[Malignant melanoma of the temporal bone].

The present article describes a case of malignant melanoma with the rare localisation in the right temporal bone. The 79-year old female patient presented a polypous tumour with obturation of the outer ear canal and sanguineous otorrhoea (Fig. 1). The inner ear function was impaired showing a combined defective hearing and complete peripheral palsy of the facial nerve as well as an abducens nerve palsy (Fig. 1) with diplopia. CT and MR imaging showed a tumour destroying the temporal bone up to the apex of the pyramid and the clivus reaching the middle cranial fossa (Fig. 2-3). After palliative

Radiation injury to the temporal bone.

Osteoradionecrosis of the temporal bone is an unusual sequela of radiation therapy to the head and neck. Symptoms occur many years after the radiation is administered, and progression of the disease is insidious. Hearing loss (sensorineural, conductive, or mixed), otalgia, otorrhea, and even gross tissue extrusion herald this condition. Later, intracranial complications such as meningitis, temporal lobe or cerebellar abscess, and cranial neuropathies may occur. Reported here are five cases of this rare malady representing varying degrees of the disease process. They include a case of radiation

Congenital anomalies of the temporal bone.

Inner ear anomalies are a rare but important cause of sensorineural hearing loss. Combined external and middle ear anomalies are more common, causing conductive hearing loss and cosmetic deformity. The course of the facial nerve canal is altered in many temporal bone anomalies. Its location is of great importance when surgical therapy is anticipated. Vascular anomalies and bone dysplasias occasionally affect the temporal bone. High resolution CT, using thin sections and edge enhancement techniques, is capable of demonstrating the finer bony details of these temporal bone anomalies.

Fibrous dysplasia of the temporal bone.

A 38-year-old woman with fibrous dysplasia of the entire right half of the mandible and the right temporal bone is presented. She had a unilateral progressive conductive hearing loss and stenosis of the auditory canal complicated by a chronic external otitis, a hidden cholesteatoma, destruction of the ossicles and an impending facial palsy. Surgery was performed to create a new auditory canal, to eradicate the cholesteatoma and to decompress the facial nerve from the surrounding fibrous dysplasia.

Traumatic injuries of the temporal bone.

The most common serious consequences of trauma to the temporal bone are hearing loss and facial nerve paralysis. In order to prevent these sequelae, radiologic evaluation is often necessary. CT offers specific information about the extent of injury, including the presence of bony fragments that need removal in order to free the facial nerve.

Spontaneous encephalocele of the temporal bone.

Postsurgical and posttraumatic encephaloceles of the tegmen tympani and tegmen mastoideum are well-recognized occurrences. Less frequently recognized, however, is the spontaneous occurrence of an encephalocele associated with cerebrospinal fluid otorrhea or rhinorrhea. This cerebral herniation into the middle ear and the mastoid cavity in adults is associated with a loss of both bony and dural support, and surgical management must deal with both the cerebrospinal fluid leak and herniation of the brain.
