Diversified social cognition in temporal lobe epilepsy.

OBJECTIVES: In an integrated model of social cognition (SC), the theory of mind (ToM), the recognition of behavior in social situations (RBSS), empathy, and sensitivity to moral and conventional rules (SMCR) cooperate in generating mental representations of the interpersonal relationships. The aim of this study was to extend our knowledge of the SC of temporal lobe epilepsy (TLE) patients by characterizing its various aspects and predictors. MATERIALS AND METHODS: Fifty adult patients with TLE and 50 healthy controls were assessed using ToM, RBSS and SMCR neuropsychological tests, the Empathy

Surgical treatment of nonlesional temporal lobe epilepsy.

PURPOSE: One-third of the patients with drug-resistant temporal lobe epilepsy (TLE) have a normal MRI, but there are only a few studies regarding the surgical outcomes and the efficacy of anterior temporal lobectomy (ATL) in patients with nonlesional TLE. The objective of this study is to evaluate the surgical outcomes and efficacy of ATL in patients with nonlesional TLE. METHODS: We included 77 consecutive patients without MRI-identifiable lesions who had undergone surgical resection for drug-resistant TLE. We performed univariate and multivariate logistic regression analyses to identify the

New Horizons in Temporal Lobe Seizure Control.

In patients with mesial temporal lobe epilepsy, high-frequency, low-amplitude electrical stimulation (ES) was applied during 3 weeks through contacts of intracranial electrodes that defined the epileptogenic zone. This subacute ES induced cessation of spontaneous seizures, decreased the number of EEG interictal spikes, caused a 10-fold increase in threshold to induce postdischarges, and showed a profound decrease in regional blood flow of the stimulated area in SPECT studies. Autoradiography analysis of surgical specimens from these patients demonstrated increased expression of benzodiazepine

Self injurious behavior in temporal lobe epilepsy.

Self-injurious behavior (SIB) or self-mutilating behavior (SMB) is rare but can occur in temporal lobe epilepsy. Such a behavior during seizures is not usually recalled by patient. Here is a case with self mutilating behavior in left temporal lobe epilepsy, presented because of its rare manifestation and diagnostic dilemma. A 19 year old unmarried Muslim student presented to emergency with SMB, guilty rumination and a persecutory delusion. The patient was intermittently confused about place and time. In subsequent assessments, he was found harboring death wishes and suicidal ideation. He trans

Falcotentorial meningioma accompanied by temporal lobe hematoma.

We report a case of a falcotentorial meningioma accompanied by hematoma in the temporal lobe. A healthy 51 year-old-female with no history of hypertension presented with sudden onset of consciousness disturbance and right hemiparesis. Computed tomography revealed a hematoma 5.5 cm in diameter surrounded by thick edematous brain in the left temporal lobe and a tumor 3.5 cm in diameter in the pineal region. Bilateral carotid angiography detected occlusion of the Galenic vein and straight sinus. No causative abnormality of hemorrhage was apparent. However, the left basal vein of Rosenthal had dis

[Glutamatergic excitatory receptors and temporal lobe epilepsy].

Despite a considerable wealth of data, the mechanisms responsible for the generation of epileptic bursts in temporal lobe epilepsy (TLE) remain a mystery. Recently, research and therapy have focused on impairment of GABAergic inhibition in epilepsy. Several lines of evidence support this approach: 1-GABA is the main inhibitory neurotransmitter in the neuronal structures involved in TLE. 2-Enhancers of GABAergic inhibition (such as benzodiazepines or barbiturates) are commonly used as antiepileptic drugs. 3-Interictal discharges can be obtained following the pharmacological blockade of GABAA re

Magnetic resonance spectroscopy in temporal lobe epilepsy.

We used proton magnetic resonance spectroscopy (1H MRS) to investigate the temporal lobes of 25 patients with temporal lobe epilepsy. Spectra were obtained from 2 x 2 x 2 cm cubes in the medial region of the temporal lobe, and were analyzed on the basis of signals from N-acetylaspartate (NAA), creatine + phosphocreatine (Cr), and choline-containing compounds (Cho). In comparison with control subjects, the temporal lobes ipsilateral to the seizure focus showed a mean reduction of 22% in the NAA signal, with a 15% increase in the Cr signal and a 25% increase in the Cho signal. There were smaller

Sequelae of temporal lobe herniation: MR imaging.

Acute temporal lobe herniation is a serious complication that may occur with an expanding temporal or parietal lobe mass. Temporal lobe herniation may produce further damage to intracranial structures due to compression or vascular compromise. Differentiation of clinical and imaging abnormalities due to the primary lesion from those related to temporal lobe herniation is essential for proper diagnosis and management. The CT and magnetic resonance findings in a 2 1/2-year-old child who survived severe temporal lobe herniation are reported.

Ipsilateral symptomatology in left temporal lobe lesions.

Two cases of ipsilateral symptomatology related to left temporal lobe lesions are presented. The possibility of ipsilateral symptoms after temporal lobe lesions has to be considered, mainly when they occur in the dominant hemisphere. Direct or indirect damage to the insula of Reil's sensory-motor secondary area seems to be the motive of these ipsilateral symptoms.

Temporal lobe meningioma masked by polymyalgia rheumatica.

An elderly woman who had a typical polymyalgia rheumatica (PMR) syndrome, associated with severe headache, was considered to have giant cell arteritis despite negative temporal artery biopsies and was treated with corticosteroids. At autopsy, no arteritis, but a large temporal lobe meningioma, was found as the cause of her headache. Both PMR and meningiomas are frequent in elderly patients, and overlapping symptoms may occur. Patients with PMR and headache should be fully investigated to exclude an occult meningioma or other disease, especially when no positive histologic evidence of temporal

Psychoses in drug-resistant temporal lobe epilepsy.

In the survey of 74 Danish patients with temporal lobe epilepsy who underwent temporal lobectomy, a total of 20 patients were psychotic. Nine of these became psychotic during the follow-up period, six of them after cessation of their epileptic seizures. There were 13 schizophrenia-like psychoses, six paranoid delusional and depressive psychoses, and one childhood psychosis. Operation was on the right side in 39 and on the left side in 35 patients. When the various psychotic groups were compared with each other or with the nonpsychotic patients, the side of operation was not found to be statist

Amyotrophic lateral sclerosis with temporal lobe atrophy.

Clinical and neuropathological findings are reported on a 48-year-old man who developed progressive bulbar palsy, muscle atrophy of four extremities, and hyperreflexia. Duration of the illness was about 19 months. Moderate personality changes were observed during his hospitalization. Neuropathological examination revealed the presence of an ALS: severe loss of the large motoneurons in the spinal anterior horns, and degeneration of the corticospinal tract, more prominent on the left side. The hypoglossal, the facial, and the motor trigeminal nuclei were also involved, but the oculomotor, the tr

Temporal lobe epilepsy and depression: a dysfunctional lobe overshadows a sclerotic hippocampus.

Depression is one of the most frequent psychiatric comorbidities in drug-resistant patients with epilepsy, reaching up to 60% of them. Studies indicate that the pathophysiology of depression and epilepsy have a lot in common, especially the temporal lobe epilepsy (TLE). Since hippocampal sclerosis (HS) is a frequent histopathology finding in patients with TLE, the purpose of this study was to investigate if there is a frequency relation between depression and the different types of HS. 119 patients with TLE, with and without HS, were selected (HS1 = 40, HS2 = 40, NHS = 39) and their medical re

Molecular Genetics of Acquired Temporal Lobe Epilepsy.

An epilepsy diagnosis reduces a patient's quality of life tremendously, and it is a fate shared by over 50 million people worldwide. Temporal lobe epilepsy (TLE) is largely considered a nongenetic or acquired form of epilepsy that develops in consequence of neuronal trauma by injury, malformations, inflammation, or a prolonged (febrile) seizure. Although extensive research has been conducted to understand the process of epileptogenesis, a therapeutic approach to stop its manifestation or to reliably cure the disease has yet to be developed. In this review, we briefly summarize the current lite

Temporal lobe tumors modify local venous drainage.

OBJECTIVE: Superficial Middle Cerebral Vein (SMCV) is an anastomotic vein frequently exposed during surgery. Changes in the pattern of cerebral venous outflow can occur in many pathological settings. We explored the hypothesis that the growth of an intracranial tumor could determine alterations in the venous outflow. We analyzed SMCV anatomical variants in patients undergoing surgery for intracranial tumors; we furthermore focused on association with histology. METHODS: We retrospectively collected data of 120 patients undergoing surgery, 60 presenting intracranial tumor and 60 presenting cer
