Temporomandibular joint (TMJ) dysfunction describes a pain-dysfunction phenomenon that usually afflicts persons in their 4th or 5th decade. The syndrome can be produced by a variety of etiologic factors including occlusal disharmony, articular disorders, and muscle imbalance. It may cause severe otalgia and refer pain to the temple, occiput, nape of neck, and shoulders. Often, associated joint clicking or popping, aural fullness, vertigo, tinnitus, subjective hypoacusis, and nausea occur. Ganglion cysts are common pseudocystic masses, whereas those arising from the temporomandibular joint (TMJ) are rare entities. We report a case of ganglion cyst of the right TMJ with symptomatic bilateral TMJ internal derangement in a 24-year-old man. Disk repositioning using bone anchors and excision of the ganglion cyst were performed. A unique characteristic of inflammatory infiltrates was revealed in the specimen, and the relationship between these 2 distinct entities and probable pathogenesis of infectious involvement are discussed.

Immunological investigation was performed in patients with trigeminal neuralgies and painful myofascial temporomandibular joint dysfunction before and after treatment. The data suggest a connection between the state of humoral immunity and the peculiarities of pain syndrome in trigeminal neuralgia and clinically related prosopalgies that was most probably due to different mechanisms of realization of pain syndrome.

Disorders of the TMJ are best evaluated with MR. Anatomy and morphology of the meniscus are key features of the imaging evaluation conducted using coronal and sagittal views with open and closed mouth. There is no direct correlation between symptoms and MR imaging findings. Anterior or lateral meniscal displacement, permanent or reversible, are described. Arthroscopy or arthrography may help in diagnosis of meniscal adhesion or perforation. HRCT is an adjunct to study degenerative bone changes.

The depth, length, and depth:length ratio of the mandibular fossa, and the angles of the posterior slope of the articular eminence were measured for 91 temporomandibular joints (TMJ) with anterior disk displacement (ADD) in 79 female patients with TMJ dysfunction. In this paper the painful syndromes of temporal arteritis, polymyalgla rheumatica, glaucoma, trigeminal neuralgia, post-herpetic neuralgia, and temporomandibular joint dysfunction have been described. These conditions occur commonly in the elderly. The authors report one case of Von Recklinghausen's disease localized to the temporomandibular joint capsule and unilaterally associated with other cephalic manifestations. Although no other study, notably a CAT-scan, showed this tumor localization, surgical exploration was imperative and led to exeresis of the lesion which proved to be the treatment of choice.

High-resolution computed tomography (CT) is evolving as the most valuable radiographic study when detailed information about the status of the temporomandibular joint is needed. High-resolution CT is faster, less invasive, and in most cases much more accurate than other imaging modalities used in the past. Erosive, hypertrophic, and ankylosing arthropathies are illustrated, as are condylar dislocations secondary to acute trauma, chronic trauma, and neoplasm. A detailed discussion of the common problem of the anterior displaced meniscus is undertaken.
