Traumatic and Overuse Wrist Injuries: Osseous and Tendon Pathologies.

Traumatic and overuse injuries of bones and tendons of the wrist represent a significant proportion of the musculoskeletal disorders encountered in both athletic and occupational settings. Conventional radiography remains the first-line imaging modality for osseous trauma, but it can miss up to a third of scaphoid and hamate fractures. Computed tomography is superior for characterizing occult or nondisplaced fractures. Magnetic resonance imaging provides important information on occult fractures, overuse injuries of osseous structures, and tendinous abnormalities.A comprehensive understanding

Current Concepts in the Management of Achilles Tendon Injuries.

Achilles tendon injuries are commonly encountered in orthopaedic practice. The Achilles tendon is prone to rupture, and this negatively imparts ambulation. The diagnosis is clinical. Treatment options have evolved from conservative to surgeries, however there is no consensus on the most superior option of treatment. This narrative review aims to highlight the evolution in clinical diagnosis and treatment of Achilles tendon ruptures and discuss the current evidence in the treatment with emphasis on outcome measures of each treatment modality. Relevant studies on Achilles tendon ruptures with em

Bilateral quadriceps tendon rupture: two injuries 6 years apart.

We present a case of a fit man in his 50s, with simultaneous bilateral quadriceps tendon repair of injuries sustained 6 years apart. Spontaneous closed ruptures of the quadriceps tendon are uncommon. Clinical data of a single case of bilateral quadriceps tendon injury with simultaneous repair was gathered via the patient, notes and surgeon. Diagnosis was primarily based on history and clinical examination. Suggestive features on the plain radiographic imaging were also present. Confirmation was attempted using ultrasonography but yielded conflicting reports. The patient was screened for any as

[Primary repair of extensor tendon injuries of the hand].

Knowledge of the functional anatomy of the extensor tendons with the sliding mechanism is essential for better understanding the surgical therapy and the postoperative care in the different trauma zones of the extensor tendon. In addition the localization of the injury is very important for the clinical findings, therapy, after-care and long term prognosis. Anatomical facts, injury mechanisms and associated injuries have to be considered in the therapy concept. Several therapy possibilities are known: immobilizing with splint for the closed rupture in zone I, reinsertion in zone III (with a te

Avoiding sural nerve injuries during percutaneous Achilles tendon repair.

BACKGROUND: Sural nerve injury is a reported risk during percutaneous repair of the Achilles tendon. HYPOTHESIS: Exposure of the sural nerve during percutaneous repair can minimize the risk of nerve injury. STUDY DESIGN: Case control study; Level of evidence, 3. METHODS: The authors retrospectively examined the results of 84 patients who were treated for acute Achilles tendon rupture at 2 different hospitals. Both hospitals used the same percutaneous repair technique, except that the sural nerve was exposed in the 38 patients (mean age, 38 years; range, 23-68 years) of one hospital; the ner

Peroneal tendon injuries. Report of thirty-eight operated cases.

BACKGROUND AND AIMS: Peroneal tendon injuries are probably frequently overlooked causes of lateral ankle pain after distorsion trauma. We report a series of 38 patients with peroneal tendon injuries and outcome after operation. MATERIAL AND METHODS: The mean age of the patients was 30 years (range, 13 to 61). All patients were operated by the same orthopaedic surgeon (SO) and the final outcome was evaluated. RESULTS: Eighty-two per cent of the patients were competitive athletes. There were 11 partial and 3 total ruptures of the peroneus brevis tendon (PBT), 2 partial and 2 total ruptures of

[Injuries of the tibialis posterior tendon: diagnosis and therapy].

Complete traumatic rupture of the tibialis posterior tendon is absolutely rare. From the scarce case reports in the medical literature a extreme pronation-abduction or pronation-external rotation mechanism according to the Lauge-Hansen classification can be presumed, leading to a malleolar fracture because of forced pronation, external rotation and dorsiflexion of the foot. With primary suture the prognosis is favorable. Traumatic dislocations, mostly with luxatio pedis sub talo, are treated by atraumatic reduction and refixation of the retinaculum. Again, the prognosis is favorable. Incomplet

Tendon injuries about the foot and ankle in children.

Sixty-four children with tendon injuries about the foot and ankle have been reviewed in order to present a plan of management when such injuries occur. One-third involved the tendo Achillis; the others were equally distributed between the dorsum and sole of the foot, and in only 12% was more than one tendon involved. Sensory nerves were frequently severed, but nearly all recovered completely after early repair. The results of immediate repair of tendons were good. When repair was delayed the results were less satisfactory, as severe deformities developed in the growing feet. Early exploration

Management of extensor tendon injuries concerning distal radius fractures.

INTRODUCTION: Rupture of the extensor tendons secondary to fractures involving the distal radius is a well-recognized rare complication. In patients with implants particularly, there exists a tendency for attributing the implant as a cause for the tendon rupture. We retrospectively studied the patients with extensor tendon injuries related to distal radius fractures, analyzed the factors leading to the rupture, suggest few preventive measures and describe the management strategy of these ruptures. MATERIALS AND METHODS: 21 patients who were treated for extensor tendon rupture following distal

Zone 1 Flexor Tendon Repairs: Laceration and Avulsion Injuries.

In contrast to other zones of the hand, zone 1 flexor tendon injuries include laceration as well as avulsion mechanisms. Although zone 1 tendon lacerations are treated similarly to other zones, with various suture configurations and techniques, zone 1 avulsion injuries often require repair of tendon to bone. Proximity of the repair site to the distal interphalangeal joint often results in persistent flexion contracture and stiffness. Despite these differences in injury mechanism and location, zone 1 flexor tendon repairs are well tolerated and often lead to fair-high patient satisfaction.

Pulley, Flexor, and Extensor Tendon Injuries of the Hand.

Tendon injuries represent the second most common injury of the hand (after fractures) and are a common scanning indication in radiology. Pulley injuries are very frequent in rock climbers with the A2 pulley the most commonly affected. Tendon and pulley injuries can be reliably evaluated using ultrasound (US) and magnetic resonance imaging (MRI). US can be postulated as a first-line imaging modality, allowing dynamic examination. MRI is essential for cases with ongoing diagnostic doubt post-US and also for preoperative pulley reconstruction assessment.

Suture Anchor Repair of Quadriceps and Patellar Tendon Injuries.

Operative treatment of quadriceps and patellar tendon ruptures with transosseous bone tunnels remains the gold standard, but potential benefits exist with the use of suture anchor fixation for these injuries. Such benefits include stronger biomechanical fixation, reduced soft-tissue disruption, smaller incision, reduced postoperative pain, shorter operative time, lower knot burden, lack of prepatellar bursa scarring, and avoidance of some transosseous repair risks. In this investigation, we present the reproducible technique and outcomes of using suture anchors for repair of quadriceps and pat

Acute tendon injuries in the hand and their management.

Acute tendon injuries to the hand are common. In the UK, there are approximately 12000 inpatient admissions annually for injuries to the tendons and muscles of the hand and wrist (Dew, 2009). Hand injuries involving the dominant hand can cause long-term disability if not appropriately managed. Diagnosis of tendon injuries in the hand relies on careful clinical assessment, particularly if flexor and extensor tendon injuries are not to be missed. This article describes the tendon structure, explains the causes of tendon injury and discusses their assessment and management for a foundation level

[Primary management of flexor tendon injuries of the hand].

The repair of interrupted flexor tendons of the hand and their return to satisfactory function has been one of the most difficult and challenging tasks and problems even for the hand surgeon. Accordingly, nearly all publications have dealt with suture techniques, suture material and even the number of knots with regard to vascularization, course of sheaths and biology of healing. The aim of all surgical intentions is intrinsic healing, with as few adhesions as possible. On the one hand, this requires non-traumatic treatment of the tendon, respecting the dorsally located blood support and, on t

[Ultrasonography and magnetic resonance tomography (MRI) of tendon injuries].

Both ultrasound and MR imaging have become increasingly important for the assessment of tendon abnormalities. Ultrasound is widely available and is mainly suitable for emergency situations in traumatology and as a screening method. For soft tissue imaging only up-to-date sonographic equipment should be used. Moreover, the examiner's experience is crucial for adequate results. MR imaging is not as easily available. However, it is more reproducible and may be indicated in a preoperative situation. Most publications about sonography and MR imaging deal with the rotator cuff and the Achilles tendo
