Wrist Injuries in Tennis Players: A Narrative Review.

The wrist/hand complex forms the crucial final link in the kinetic chain between the body and the racquet and therefore has a number of important roles in the production of all tennis strokes. However, the internal and external loads that are created at the wrist during these strokes have the potential to contribute to pain and injury. Therefore, the purposes of this narrative review are to (1) determine the extent of the problem of wrist pain/injury in tennis players, (2) identify bony and soft tissue structures of the wrist that are susceptible to damage as a result of tennis play and (3) ex

Tennis leg: mechanism of injury and radiographic presentation.

As summer is upon us, we thought a discussion of tennis leg would be beneficial to our fellow clinicians. Tennis leg is a relatively common clinical condition, classically manifesting as acute, sports-related pain in the mid-calf. First described in 1883, the pathogenesis has been debated--tennis leg was first attributed to rupture of the plantaris tendon, though more recent investigations have implicated rupture of the medial head of the gastrocnemius at its myotendinous junction. For simplicity, many authors use the term tennis leg to describe all such acute muscle injuries in the superficia

Mechanical solution for a mechanical problem: Tennis elbow.

Lateral epicondylitis is a relatively common clinical problem, easily recognized on palpation of the lateral protuberance on the elbow. Despite the "itis" suffix, it is not an inflammatory process. Therapeutic approaches with topical non-steroidal anti-inflammatory drugs, corticosteroids and anesthetics have limited benefit, as would be expected if inflammation is not involved. Other approaches have included provision of healing cytokines from blood products or stem cells, based on the recognition that this repetitive effort-derived disorder represents injury. Noting calcification/ossification

Reliability of sonographic assessment of tendinopathy in tennis elbow.

OBJECTIVE: To assess the reliability and compute the minimum detectable change using sonographic scales to quantify the extent of pathology and hyperaemia in the common extensor tendon in people with tennis elbow. MATERIALS AND METHODS: The lateral elbows of 19 people with tennis elbow were assessed sonographically twice, 1-2 weeks apart. Greyscale and power Doppler images were recorded for subsequent rating of abnormalities. Tendon thickening, hypoechogenicity, fibrillar disruption and calcification were each rated on four-point scales, and scores were summed to provide an overall rating of

Operative management of tennis elbow: a quantitative review.

INTRODUCTION: The results of operative management of tennis elbow are varied, and the indications for surgery are not well codified. Many operative techniques are reported, but a clear consensus on whether a given surgical procedure is more effective over another is yet to be reached. METHODS: We conducted a MEDLINE, CINAHL and EMBASE search on all available scientific articles that reported the outcomes of surgery for lateral epicondylopathy. Keywords used were 'tennis elbow', 'lateral epicondylitis', 'lateral epicondylalgia', 'tendinopathy', 'tendonitis' and 'tendon'. Subheadings used were

Outcome of modified open surgery in tennis elbow.

BACKGROUND: Tennis elbow is a common disorder of upper extremity. It can be treated conservatively in majority of the patients but some resistant cases eventually need surgery. MATERIAL & METHODS: This descriptive study was conducted at Department of Orthopaedics, Ayub Teaching Hospital, Abbottabad, from January 2002 to June 2006. Sixteen patients had surgical release of the extensor tendon origin along with excision of the lateral epicondyle ridge. These patients did not respond to conservative treatment i.e. rest, non- steroidal anti inflammatory drugs (NSAIDS), local steroid injections and

Anatomic factors related to the cause of tennis elbow.

BACKGROUND: The pathogenesis of lateral epicondylitis remains unclear. Our purpose was to study the anatomy of the lateral aspect of the elbow under static and dynamic conditions in order to identify bone-to-tendon and tendon-to-tendon contact or rubbing that might cause abrasion of the tissues. METHODS: Eighty-five cadaveric elbows were examined to determine details related to the bone structure and musculotendinous origins. We identified the relative positions of the musculotendinous units and the underlying bone when the elbow was in different degrees of flexion. We also recorded the conta

Bilateral segmental dystonia in a professional tennis player.

Dystonias occur frequently as repetitive movements, persistent elevations of muscle tone, or tonic contortions, whereby the cause is assumed to be an impairment of basal ganglia function. Focal dystonias are especially known in musicians, although little is reported on focal dystonias in athletic stress. The present case report describes the case of a 34-yr-old professional tennis player with bilateral segmental dystonia. The symptoms were expressed in involuntary movements when he intended to hit the ball and in a progredient tremor, initially in one hand, later in both, making him unable to

Tennis elbow: treatment of resistant cases by denervation.

Anatomical and clinical research has shown that the entire lateral epicondylar region is innervated only by radial nerve branches. Based on these investigations we have developed a surgical procedure for complete denervation which is indicated only in resistant cases of tennis elbow. Only one nerve pathway calls for careful exposure, otherwise denervation is accomplished blindly by disinsertion of certain muscles. The result of this procedure also depends on simultaneous indirect decompression of the posterior interosseous nerve. Excellent or good results were obtained in 90% on average. Resul

Patellofemoral dysfunction in tennis players. A dynamic problem.

Our mechanically-based dynamic approach to PFD focuses on immediate alteration of symptoms during functional activities. This approach and this article focuses on mechanically-based PFD. The athlete benefits from this approach because if symptom alteration is successful, activity modification may not be necessary. The success of our ability to alter symptoms is an excellent indicator of the athlete's rehabilitation prognosis. Although remaining active is an immediate benefit to the athlete, this program focuses on correction of mechanical faults and musculoskeletal imbalances in an attempt to

Comparison of diflunisal and naproxen in the treatment of tennis elbow.

The authors favor conservative treatment of tennis elbow, starting with cessation of the offending activity and prescription of a nonsteroidal anti-inflammatory drug (NSAID) and followed by isometric and isotonic exercises when pain and inflammation have subsided. Two NSAIDs, diflunisal and naproxen, were evaluated in 38 patients with mild-to-moderate pain associated with tennis elbow. The physicians' assessments found no statistically significant differences between the two drugs; both provided effective reduction of pain and swelling. Patients' assessments of pain relief achieved with their

Overuse syndromes in baseball, tennis, gymnastics, and swimming.

The shoulder is the "center of action" for most sports including those discussed in this article. Overuse problems of the shoulder are the most common group of injuries affecting the athlete involved in baseball, tennis, swimming, and gymnastics. Most of the injuries can be directly related to the mechanics of throwing, swimming, or gymnastics. After diagnosis, treatment is directed at "selective rest" of the injured shoulder and modalities and medications intended to decrease the inflammatory response of the body to stress.

Ultrastructure of the common extensor tendon in tennis elbow.

In patients with tennis elbow, the common extensor tendon showed a pronounced reactive change consisting of mesenchymal cell proliferation along with aggregates of newly formed vascular channels. When studied ultrastructurally, many of the mesenchymal cells adjacent to the vascular channels were surrounded by a basal lamina, which is not normally seen around tenocytes. The cytoplasm of these cells showed features of both endothelial cells and tenocytes. It thus appeared, that the endothelial cells of the newly formed vascular channels were the source of proliferating mesenchymal cells differen

Rally and recover: Physiological demands between tennis drills.

This study examined physiological recovery between repeated tennis drills in elite adolescent tennis players. Ten tennis players (5 males, 5 females; age 17 ± 2 years) underwent treadmill testing to establish maximal physiological characteristics. Several days later participants completed three standardized on-court tennis drills with fixed rest intervals, during which physiological parameters were monitored. A one-way repeated-measures ANOVA was used to compare physiological responses across the three drills. [Formula: see text] utilization stayed above 75% in all drills, peaking during the f

[Compartment syndrome complicating tennis leg: about a case].

Tennis-leg is a partial or total disinsertion of the medial head of the gastrocnemius muscle from its musculoskeletal junction. Patients receiving adequate treatment generally have favorable outcome. Tennis leg is rarely associated with major complications such as acute compartment syndrome. We report the case of a judo athlete with disinsertion of the right medial head of the gastrocnemius muscle complicated by acute compartment syndrome. Emergency aponeurotomy was required.
