Tension-type headache introduction and diagnostic criteria.

Tension-type headache is the most common primary headache syndrome. Lifetime prevalence ranges from 35% to 80%. There is a distinction between an episodic and a chronic form. This distinction is important, because the chronic subtype strongly implies a neurobiological basis and causes both impairment of quality of life and a high disability score, and is comorbid with anxiety and depressive disorders. The episodic tension-type headache is divided into an infrequent and a frequent subtype. It is known that the frequent type will often be transformed into the chronic type. Tension-type headache

The clinical neurophysiology of tension-type headache.

Despite being widely investigated, the pathogenesis of tension-type headache (TTH) continues to be debated. Among the different approaches used to explore the mechanisms underlying TTH, clinical neurophysiology plays an important role. Studies to date have focused mainly on two areas: (1) evaluation of peripheral factors (i.e., by electromyography (EMG)); and (2) exploration of the role of the pain control system. In the second of these areas, a large number of studies have explored trigeminal pathways, in particular using reflexes (e.g., the trigeminofacial reflex, trigeminotrigeminal reflexe

Chronic tension-type headache: what is new?

PURPOSE OF REVIEW: This review discusses current data on nosological boundaries related to diagnosis, pathophysiology and therapeutic strategies in chronic tension-type headache (CTTH). RECENT FINDINGS: Diagnostic criteria of CTTH should be adapted to improve its sensitivity against migraine. It seems that mechanical pain sensitivity is a consequence and not a causative factor of CTTH. Recent evidence is modifying previous knowledge about relationships between muscle tissues and CTTH, suggesting a potential role of muscle trigger points in the genesis of pain. An updated pain model suggests t

Migraine, tension-type headache, and transformed migraine.

Migraine and tension-type headache (TTH) are highly prevalent primary headaches that remain underdiagnosed and undertreated in clinical practice. The similarities and differences between migraine and TTH may impose diagnostic challenges as well as management difficulties. In addition, the possibility of migraine chronification or transformation in daily or near-daily headache raises the potential level of interaction between pathophysiologic mechanisms of TTH and migraine. The continuum concept is a possible key to the understanding of this association. Future studies are necessary to clarify

Future therapeutic perspectives for tension-type headache.

The therapy for tension-type headache remains insufficient, and recent advances have been scarce. Although tricyclic antidepressants are at the forefront of treatment advances, upcoming agents tentatively modifying central sensitization are promising. Botulinum toxin failed to meet expectations. This article reviews current treatments, emphasizing newer approaches. Much remains to be achieved.

Familial occurrence of chronic tension-type headache.

Chronic tension-type headache (CTTH) assessed by proband report was evaluated in a family study of CTTH. A clinical interview of first-degree relatives by a physician was used as index of validity. Familial occurrence of CTTH in first-degree relatives was also investigated. Patterns of familial aggregation of CTTH were assessed by calculating the population relative risk. A neurological resident carried out all the interviews of probands and their first-degree relatives. The operational diagnostic criteria of the International Headache Society were used. The 122 probands had 377 first-degree r

Serotonin metabolism in chronic tension-type headache.

Serotonergic neurons play a major role in the regulation of pain and may therefore also be involved in the pathophysiology of tension-type headache. Platelets are important in the regulation of the free serotonin level in plasma and may be a model of serotonergic neurons. The aim of the present study was to investigate the peripheral serotonin (5HT) metabolism in patients with chronic tension-type headache. The 5HT levels in platelets and in plasma, the beta-thromboglobulin (beta-TG) levels in plasma, and the urinary excretion of 5-hydroxyindoleacetic acid (5HIAA) were measured in 40 patients

Epidemiology of migraine and tension-type headache.

The introduction of operational diagnostic criteria for the primary headaches in 1988 has provided new opportunities for comprehensive epidemiologic headache research. In recent years, several studies on the epidemiology of the primary headaches have emerged. This review deals with methodologic considerations and provides a "snapshot" of the main findings in these studies. The importance of epidemiologic studies in clarifying the natural history of the primary headaches, including the distinction between separate clinical entities, is elucidated.

Pathophysiology of migraine and tension-type headache.

Cortical spreading depression is probably the event underlying the aura of migraine, and biochemical and other studies suggest that a state of increased excitability of central neurons may lead to development and propagation of cortical spreading depression. Anatomical studies delineate some of the sites involved in the central processing of craniovascular nociception. 5-Hydroxytryptamine receptors, particularly 5-HT1, exist at both neural and vascular sites and are implicated in the pathogenesis of migraine. Studies in tension-type headache suggest that its pathogenesis is multifactorial, wit

Anger and hostility in tension-type headache.

A battery of standardized psychometric tests was administered to a group of 47 episodic tension-type headache sufferers and 47 headache-free controls. Compared to controls, headache subjects showed higher levels of anxiety, depression, and anger/hostility. The groups did not differ significantly on a measure of anger expressed toward persons or objects, but headache subjects showed significantly greater levels of suppressed anger. The results provide objective data that are in general agreement with predictions derived from psychosomatic theories about the interrelationships among anxiety,

Acupuncture for tension-type headaches and migraine.

Headache is one of the most frequent reasons for medical consultation in both general practice and neurology clinics.1 Tension-type headaches occur in up to around 80% and migraine occurs in about 15% of the UK adult population.1(,)2 Prescribed and over the counter-medications are taken to alleviate headaches and migraine, but may be used incorrectly.3 In particular, use of some drugs both frequently and regularly can have a paradoxical effect, causing headaches rather than relieving them, and leading to medication overuse headache.4 Acupuncture, which is usually provided privately by an indep

Central mechanisms in tension-type headaches.

Although tension-type headache is the most frequent primary headache, little is known about its pathophysiology. It is a matter of debate if the pain in tension-type headache originates from myofascial tissues or from central mechanisms in the brain. This article presents a summary of available data on the pathophysiology of tension-type headache and proposes a pathogenic model. From experimental research and clinical studies, it appears that myofascial nociception is important in episodic tension-type headache; however, central mechanisms (ie, central sensitization) are preponderant in the pa

An experimental test of the muscle tension hypothesis of tension-type headache.

The present study employed an experimental design to provide a direct test of the classic etiological account of tension-type headaches, that these stem from elevated levels of muscle tension. Twenty-eight female subjects with relatively frequent headaches were divided into 4 groups, according to a 2 x 2 design. The independent variables were (1) Target response (to maintain elevated levels of either frontalis EMG or digital temperature for 40 min), and (2) Expectation (either specifically of a headache or of some unspecified discomfort). Dependent variables included headache as well as a numb

Nitric oxide-induced headache in patients with chronic tension-type headache.

An experimental model of headache offers unique possibilities to study the mechanisms responsible for head pain. Using the glyceryl trinitrate [GTN; nitric oxide (NO) donor] model of experimental headache, we studied the intensity, quality and time profile of headache after infusion of GTN in 16 patients with chronic tension-type headache and in 16 healthy controls. Subjects were randomized to receive intravenous infusion of GTN (0.5 microg/kg per minute for 20 min) or placebo on two headache-free days separated by at least 1 week. Headache intensity was measured on a 10-point verbal rating sc

Postlumbar puncture headache and its relation to chronic tension-type headache.

One hundred consecutive patients, the majority suffering from bilateral chronic tension-type headache, investigated with lumbar puncture, were studied as to age, sex, body mass index, diagnosis, lumbar cerebrospinal fluid pressure, and signs of inflammation in the serum in relation to postlumbar puncture headache. Patients younger than 40 years of age were significantly more prone to develop postlumbar puncture headache than patients older than 40 years of age (P = 0.01). Sex, body mass index, cerebrospinal fluid pressure, and signs of inflammation in the serum were not related to the frequenc

Tension-type headache in different age groups at two headache centers.

At two headache centers, one for children and adolescents and the other for adults, we investigated patients aged 5-80 years with episodic or chronic tension-type headache to evaluate the relation between age and headache characteristics as well as analgesics intake. We found an increasing headache frequency and duration, an increasing variability of the headache location, and an increasing frequency of nausea with increasing age. All other headache features did not depend on age. Additionally, our study revealed a marked increase of analgesics use in adults compared to children and adolescent

Cervicogenic headache, migraine, and tension-type headache. Pressure-pain threshold measurements.

Pressure-pain threshold (PPT) measurements were performed with a pressure algometer, at 22 specified points in the head in patients with cervicogenic headache (n = 32), migraine (with and without aura) (n = 26) and tension-type headache (n = 17). Comparisons were made with a group of healthy controls (n = 20). The average PPT differed significantly between the groups (ANOVA, F = 9.5, P < 0.0005), largely caused by the low threshold in cervicogenic headache patients. There were no significant differences between controls and the 2 other headache groups. In the cervicogenic headache group, the l

Prevalence of headache in Swedish schoolchildren, with a focus on tension-type headache.

To estimate the prevalence of tension-type headache, migraine and other headaches, 1850 schoolchildren, age 7-15 years, from the city of Uppsala, were invited to complete a questionnaire. The response rate was 74.1%. To validate the information from the questionnaires, 131 children and their parents were interviewed. Using the criteria of the International Headache Society, the 1-year prevalence of tension-type headache and migraine was 9.8 and 11.0%, respectively. However, these prevalence rates increased considerably, to 23.0 and 17.0%, respectively, when excluding the criteria defining the
