Thiamin status of a sample of homeless clinic attenders in Sydney.

Thiamin is one of the marginally adequate nutrients in the Australian diet. The incidence and prevalence of Wernicke-Korsakoff syndrome in this country may be the highest in the world. Homeless men could be at risk for low intakes of thiamin in association with irregular high alcohol intakes. A sample of 107 homeless men from two hostels and one clinic for homeless persons in inner Sydney was investigated for nutritional status; their thiamin status is reported here. By means of 24-hour recall methods, their mean dietary thiamin intake--0.76 mg per day--was less than the National Health and Me

A mechanism for the thiamin-sparing action of dietary xylitol in the rat.

The changes induced by dietary xylitol in the gastrointestinal tract of the rat were investigated in relation to the phenomenon of vitamin-sparing. Within 18 days of consuming a synthetic diet, deficient in thiamin, riboflavin and pyridoxine, rats ceased to grow and began to lose weight rapidly. If xylitol was then included in the diet (10% w/w), the effect of the vitamin-deficient diet on growth was reversed. Moreover, within 3 days of the rats ingesting xylitol, the metabolism of this sugar polyol by the caecal microflora was increased 17-fold and the caecal concentrations of thiamin and thi

[Vitamin B1 (thiamine)].

Vitamin B1 (or thiamine) plays a key role in energy production from glucose. Since the main fuel of the nervous system is glucose, thiamine deficiency causes severe neurological symptoms. The biological exploration of vitamin B1 status is based on the measurement of thiamine pyrophosphate concentration or of the activity of a thiamine-dependent enzyme, transketolase, in erythrocytes. Severe deficiency states can be observed in chronic alcoholics, after protracted vomiting during pregnancy and after bariatric surgery. Mild deficiencies are common in the general population, but their clinical co

The influence of thiamine deficiency on porphyrin synthesis and porphyrin proteins.

1. Studies carried out in cell-free rat-liver extracts showed an inhibitory effect of thiamine deficiency on porphyrin synthesis. 2. Extracts of normal and thiamine-deficient rat livers were incubated with the cofactors and substrates of the thiamine pyrophosphate-dependent enzyme, 2-oxoglutarate glyoxylate carboligase. There was a marked inhibition of porphyrin synthesis when the substances were added either individually or together. 3. Thiamine deficiency resulted in a decrease of haemoglobin concentration and catalase activity whereas a rise in tryptophanpyrrolase activity and cytochrome b5

Red blood cell transketolase activity and the effect of thiamine supplementation in patients with chronic liver disease.

Biochemical evidence of thiamine deficiency was found in 58% of patients with chronic liver disease, the incidence being higher in alcoholic than in non-alcoholic patients. Daily supplementation with high doses of thiamine hydrochloride (200 mg/day) for one week restored levels of thiamine pyrophosphate (TPP), the active co-enzyme form of thiamine, to normal in all cases. Such supplementation also stimulated synthesis of the TPP dependent enzyme transketolase. Because of the essential role of TPP as a co-factor in intermediary metabolism, it is concluded that high doses of thiamine should be i

Determination of thiamine and its phosphate esters by gradient-elution high-performance liquid chromatography.

A high-performance liquid chromatographic method for the determination of thiamine and its phosphate esters using gradient elution is presented. Chromatography was performed on an octadecyl C18 column with post-column derivatization using potassium ferricyanide: thiamine was oxidized to thiochrome, which was detected fluorimetrically. The gradient system avoids the use of a modifier and an ion-pairing reagent. The proposed method enables the separation of thiamine triphosphate, thiamine pyrophosphate, thiamine monophosphate and free thiamine, at concentrations of ca. 2 nM, within 10 min The wi

Transketolase abnormality in cultured fibroblasts from familial chronic alcoholic men and their male offspring.

We have investigated a thiamine-dependent enzyme, transketolase, in cultured fibroblasts from 41 human subjects, including patients with alcoholism-associated Wernicke-Korsakoff syndrome (n = 3), familial chronic alcoholic males (n = 7), their sons (n = 7), nonalcoholic men (n = 7), their male offspring (n = 7), and three generations of an Amish family (n = 10) without any history of alcoholism. This study was undertaken to delineate whether transketolase abnormality (i.e., high Michaelis Menton constant (Km) for thiamine pyrophosphate), previously reported in patients with Wernicke-Korsakoff

Thiamine deficiency in general hospital patients.

The production of sedoheptulose-7-phosphate in the red blood cells of 15 patients was found to be below the suggested normal value. This intermediate product of glucose metabolism is the result of transketolase activity for which thiamine pyrophosphate is one of the co-factors. Without thiamine, the transketolase reaction is inhibited. Some nonlocalizing symptoms, perhaps previously thought due to hypoglycemia, may be occasioned by faulty glucose metabolism at the cellular level from a relative lack of thiamine.

Thiamine-responsive anemia in DIDMOAD syndrome.

Two children with the DIDMOAD syndrome (diabetes insipidus, diabetes mellitus, optic atrophy, deafness) developed a megaloblastic and sideroblastic anemia, neutropenia, and borderline thrombocytopenia. Plasma thiamine concentration was low in one patient and normal in the other; in both children, thiamine pyrophosphate in erythrocytes and thiamine pyrophosphokinase activity were lower than the lowest values observed in control subjects. A month after institution of treatment with thiamine, the hematologic findings had returned to normal and the insulin requirements had decreased. Withdrawal of

Hypothesis and case reports: possible thiamin deficiency.

Three family members are reported with functional symptoms considered to be caused by intracellular deficiency of thiamin. Persistence of desaturation of erythrocyte transketolase in the face of megadose thiamin hydrochloride (THCl), accompanied by a balanced multivitamin and mineral formula, suggested a familial thiamin dependency state. Each of three individuals responded clinically to the administration of thiamin tetrahydrofurfuryl disulfide (TTFD), and erythrocyte transketolase (TKA) became fully saturated with thiamin pyrophosphate (TPP). Dysautonomic symptoms observed are compared with

Thiamin deficiency in the lamb: changes in thiamin phosphate esters in the brain.

Concentration of thiamin (unphosphorylated), thiamin monophosphate (TMP), thiamin diphosphate (TDP), and thiamin triphosphate (TTP) were measured in three regions of the brain of seven pairs of lambs. The lambs were maintained on a thiamin-free synthetic diet for 2, 3, or 4 weeks. Controls were pair-fed and supplemented with thiamin. The three brain regions were: (1) dorso-lateral aspect of the cortex [common site for lesions of polioencephalomalacia (PEM)]; (2) pyriform lobe of the cortex (no PEM lesions are found here); (3) white matter of the internal capsule (no PEM lesions found here). Reduced thiamine binding is a novel mechanism for TPK deficiency disorder.

Thiamine pyrophosphokinase (TPK) converts thiamine (vitamin B1) into thiamine pyrophosphate (TPP), an essential cofactor for many important enzymes. TPK1 mutations lead to a rare disorder: episodic encephalopathy type thiamine metabolism dysfunction. Yet, the molecular mechanism of the disease is not entirely clear. Here we report an individual case of episodic encephalopathy, with familial history carrying a novel homozygous TPK1 mutation (p.L28S). The L28S mutation leads to reduced enzymatic activity, both in vitro and in vivo, without impairing thiamine binding and protein stability. Thiami

Absorption, utilization and clinical effectiveness of allithiamines compared to water-soluble thiamines.

Oral administration of lipid-soluble allithiamines [thiamine propyl disulfide (TPD) and thiamine tetrahydrofurfuryl disulfide (TTHF)] rapidly increased thiamine activity in whole blood, red blood cells, cerebrospinal fluid, and urine in normal and thiamine-deficient subjects. These thiamine congeners also restored red blood cell transketolase to normal in alcoholics with thiamine deficiency. Such repletion equaled that produced by parenteral, water-soluble thiamine hydrochloride (THCl) or thiamine pyrophosphate (TPP). Oral administration of water-soluble thiamines (THCl, TPP) neither elevated

Thiamine Deficiency in Adolescents with Eating Disorders: A Prospective Cohort Study.

BACKGROUND: Pediatric eating disorders (PED) patients are prone to nutritional deficiencies. Thiamine deficiency is well described in other malnutrition states but is not routinely screened for in PED. In the current study we evaluated the prevalence of thiamine deficiency among PED patients on their first admission to an outpatient day hospital for eating disorders (DH). METHODS: In this prospective cohort study, we measured whole blood thiamine pyrophosphate concentrations (TPP) in addition to a routine laboratory workup in 69 girls on their first admission to DH. Two subgroup analyses were

The effect of nitrofurazone on the thiamin status of chickens.

Nitrofurazone, given orally at doses of 10 and 20 mg/kg for seven days, decreased the activity of erythrocyte transketolase (TK) and increased the activation of TK by thiamin pyrophosphate (TPP effect %). Nitrofurazone also decreased the feed intake and growth of the chickens, and increased the concentrations of lactate and pyruvate of their blood. It was concluded that nitrofurazone has induced thiamin deficiency in the treated birds. Pair-feeding experiments showed that the decreased growth was due to anorexia, and that the effects produced by nitrofurazone treatment on the thiamin status we

The vitamin B1 status among young Egyptian males in relation to infection with parasites.

Erythrocytic transketolase activity and thiamin pyrophosphate effect (TPP%) were used to define the nutritional thiamin status of an age stratified sample of 66 young males with parasitic infection. The results revealed that 6% of the control group were biochemically deficient in vitamin B1 compared with a percentage incidence of 42% among the parasitized subjects. Intestinal infection with Entamoeba histolytica, Hymenolepsis nana or with more than one parasite had the most depleting effect on the tissue thiamin status.

Thiamine status of elderly patients with cardiac failure.

This study compared the thiamine status of 37 elderly patients admitted with cardiac failure (CF) with that of 35 unselected elderly patients with other diagnoses (Non-CF), and with that of 41 apparently healthy elderly people. The thiamine pyrophosphate effect (TPPE) on erythrocyte transketolase activity was used to estimate thiamine status. The prevalence of thiamine deficiency (TPPE greater than 19%) on admission in the CF and Non-CF groups was 13% and 29%, respectively. The TPPE of the CF group was significantly lower than that of the Non-CF group, and was not significantly different from

Intrauterine growth retardation induced by thiamine deficiency and pyrithiamine during pregnancy in the rat.

The role of nutritional factors in the development of prenatal and postnatal growth retardation is not well understood. We tested if thiamine deficiency may cause intrauterine growth retardation in rats. From the second day of gestation Sprague-Dawley rats were freely fed either a nutritionally complete or a thiamine-deficient diet. A similar group of rats was pair-fed with a complete or a thiamine-deficient diet and daily pyrithiamine injections (50 micrograms/100 gm of body weight) were given to precipitate thiamine deficiency during the short gestation of the rat. Maternal thiamine levels i
