Combination analgesic development for enhanced clinical efficacy (the CADENCE trial): a double-blind, controlled trial of an alpha-lipoic acid-pregabalin combination for fibromyalgia pain.

Drug therapy for fibromyalgia is limited by incomplete efficacy and dose-limiting adverse effects (AEs). Combining agents with complementary analgesic mechanisms-and differing AE profiles-could provide added benefits. We assessed an alpha-lipoic acid (ALA)-pregabalin combination with a randomized, double-blind, 3-period crossover design. Participants received maximally tolerated doses of ALA, pregabalin, and ALA-pregabalin combination for 6 weeks. The primary outcome was daily pain (0-10); secondary outcomes included Fibromyalgia Impact Questionnaire, SF-36 survey, Medical Outcomes Study Sleep

Are the beneficial effects of 'antioxidant' lipoic acid mediated through metabolism of reactive sulfur species?

The health benefits of lipoic acid (LA) are generally attributed to mitigating the harmful effects of reactive oxygen species (ROS). ROS are chemically similar to reactive sulfur species (RSS) and signal through identical mechanisms. Here we examined the effects of LA on RSS in HEK293 cells using H2S and polysulfide (PS) specific fluorophores, AzMC and SSP4. We show that LA concentration-dependently increased both H2S and PS. Physioxia (5% O2) augmented the effects of LA on H2S production but decreased PS production. Thiosulfate, a known substrate for reduced LA, and an intermediate in the cat

Out of Warburg effect: An effective cancer treatment targeting the tumor specific metabolism and dysregulated pH.

As stated by Otto Warburg nearly a century ago, cancer is a metabolic disease, a fermentation caused by malfunctioning mitochondria, resulting in increased anabolism and decreased catabolism. Treatment should, therefore, aim at restoring the energy yield. To decrease anabolism, glucose uptake should be reduced (ketogenic diet). To increase catabolism, the oxidative phosphorylation should be restored. Treatment with a combination of α-lipoic acid and hydroxycitrate has been shown to be effective in multiple animal models. This treatment, in combination with conventional chemotherapy, has yielde

Hypotensive effect of alpha-lipoic acid after a single administration in rats.

OBJECTIVE: The effect of alpha-lipoic acid on blood pressure was investigated many times in chronic studies, but there are no studies on the effect of this compound after a single administration. Alpha-lipoic acid is a drug used in diabetic neuropathy, often in obese patients, to treat hypertension. Therefore, knowledge of the potential antihypertensive effect of alpha-lipoic acid even after a single dose and possibly too much pressure reduction is interesting and useful. METHODS: The mechanism of the hypotensive effect of alpha-lipoic acid was examined in normotensive rats in vivo after a si

Tunable, High-Relaxivity Gd(III)-Conjugated Lipoic Acid Hydrogels for Magnetic Resonance Imaging.

A lipoic acid(LA)-base hydrogel polymerization, viscosity, and relaxivity were explored by varying several different bases, additives, Gd3+ chelates, and with or without a reducing agent (tris(2-carboxyethyl)phosphine, TCEP). The polymerization of LA was greatly improved by using TCEP to convert the more inert disulfides into thiol initiators. The base deprotonation of LA was found to be essential to solubilize LA for polymer propagation. The various bases and TCEP additions had a substantial effect on viscosity, with values ranging from 1 × 108 to 1 × 103 mPa·s. This can enable targeting of m

Hypoglycaemia in a non-diabetic patient due to alpha-lipoic acid-induced insulin autoimmune syndrome with HLA-DRB1*04:03:01G.

Insulin autoimmune syndrome (IAS), also known as Hirata syndrome, is a rare cause of endogenous hyperinsulinaemic hypoglycaemia characterised by markedly elevated circulating insulin levels and the presence of insulin autoantibodies in individuals without prior exposure to exogenous insulin. IAS is increasingly recognised worldwide, particularly in association with sulfhydryl-containing medications such as alpha-lipoic acid (ALA). We report the case of a woman in her 70s with no history of diabetes who presented with recurrent episodes of palpitations, tremors, sweating and hunger. A random pl

An In Situ Self-Solidifying Poly(Lipoic-Acid) Liquid Adhesive Forms Pathogen-Resistant Barrier for Ulcerative Colitis Therapy.

Ulcerative colitis (UC), a refractory inflammatory bowel disease, features exacerbated inflammation, barrier disruption, and microbiota dysbiosis. While nanodrug delivery is a prevalent therapeutic strategy, it faces severe limitations, including inadequate targeting, poor mucus penetration, lysosomal entrapment, reducing bioavailability, and a lack of barrier repair capability. To overcome these limitations, an in situ self-solidifying mucus-mimetic poly(lipoic-acid) liquid adhesive (PLALA) is engineered via a biobenign aqueous assembly route. The immunosuppressive, growth-hindering, hepatotoxic, and oxidative stress and immune related-gene expressions-altering effects of gibberellic acid in Oreochromis niloticus: A mitigation trial using alpha-lipoic acid.

This study aimed to examine the effects of gibberellic acid (GBA) on growth, hemato-biochemical parameters related to liver functions, digestive enzymes, and immunological response in Oreochromis niloticus. Besides, the probable underlying mechanisms were explored by assessing antioxidant, apoptotic, and immune-related gene expression. Furthermore, the likelihood of restoration following alpha-lipoic acid (LIP) dietary supplementation was explored. The fish (average initial weight 30.75 ± 0.46) were equally classified into four groups: the control group, the LIP group (fed on a basal diet plus

Alpha-tocopherol acetate and alpha lipoic acid may mitigate the development of wooden breast myopathy in broilers at an early age.

1. The objective of this study was to identify the effects of the antioxidant alpha-tocopherol acetate (ATA) and alpha lipoic acid (ALA) which have anti-inflammatory effects on developmental onset, severity and the progression of wooden breast (WB) based on Pectoralis major (P. major) muscle morphology and expression of genes associated with WB during the first three weeks post-hatch.2. A total of 160 newly hatched Ross 708 broiler chicks were randomly assigned in a replicated trial to either a control group or three dietary treatments (ATA 160 mg/kg feed, ALA 500 mg/kg feed or in combination)

Alpha-lipoic acid: A possible pharmacological agent for treating dry eye disease and retinopathy in diabetes.

Alpha-lipoic acid (ALA) is a naturally occurring dithiol micronutrient which acts as a cofactor for mitochondrial enzyme activity. Due to its potential antioxidant activity, it is considered as "universal antioxidant". Previous studies reported the pharmacological benefits of ALA such as glycaemic control, improved insulin sensitivity and alleviation of diabetic complications such as neuropathy and cardiovascular diseases. Dry eye disease and retinopathy are prevalent in diabetic patients. Experimental studies demonstrated the beneficial effects of ALA in dry eye and diabetic retinopathy. ALA

Direct determination of glutathione in human blood by micellar electrokinetic chromatography: simultaneous determination of lipoamide and lipoic acid.

A reproducible, rapid procedure for the determination of glutathione in human blood by micellar electrokinetic chromatography has been developed. Whole blood samples were deproteinized with 5% w/v sulfosalicylic acid (final concentration). After centrifugation, the supernatant was directly injected for analysis, without further derivatization. Separations were performed by using an uncoated capillary of 30 cm effective length and 50 microns internal diameter (ID), 50 mM Tris-HCl, 30 mM sodium dodecyl sulfate (SDS), pH 7.00, as running buffer, and 10-20 kV. On-line detection was carried out at

A H2S-releasing oridonin derivative improves diabetic wound healing based on antibacterial activity and regulation of macrophage phagocytosis.

Chronic diabetic wounds are characterized by persistent bacterial burden and impaired healing. Oridonin, a natural diterpenoid, shows potential for wound repair but is limited by poor solubility and bioavailability. A promising strategy to enhance oridonin's efficacy involves its conjugation with H2S donors. In our study, a type 1 diabetic mouse wound model was treated topically with hydrogels containing oridonin, α-thioctic acid, or H2S-releasing oridonin derivative D5 for 14 days. D5 treatment induced epithelial thickening, collagen deposition, and upregulation of Ki67 and CD31, as evidenced

Evaluation of anti-oxidant treatments in an in vitro model of alkaptonuric ochronosis.

OBJECTIVES: Alkaptonuria (AKU) is a rare genetic disease associated with deficient homogentisate 1,2-dioxygenase activity in the liver. This leads to the accumulation of homogentisic acid (HGA) and its oxidized/polymerized products in connective tissues, which in turn become characterized by the presence of melanin-like pigments (ochronosis). Since at present, further studies are necessary to support the use of drugs for the treatment of AKU, we investigated the effects of various anti-oxidants in counteracting melanin-like pigmentation and oxidative stress related to HGA and its metabolites.

Short-term recovery of alpha-lipoic acid on the intestine of Megalobrama amblycephala after ammonia stress.

This study aimed to investigate the short-term recovery effect of α-lipoic acid (α-LA) on the intestine of juvenile Megalobrama amblycephala stressed by ammonia. Experimental fish with a body weight of 13.7 ± 1.6 g were exposed to ammonium chloride solutions at 0 (G1), 5.45 (G2) and 10.89 mg/L (G3), respectively, for 72 h. The fish were moved to aerated tap water to conduct the recovery experiment after ammonia stress. Each group was subdivided into three subgroups and fed three experimental diets containing 0, 0.6 or 1.2 g/kg α-LA for another 72 h. Intestinal samples after ammonia exposure an

Dapsone: strategies for increasing patient tolerance.

Dapsone possesses anti-infective and anti-imflammatory properties, but its CYP-mediated metabolism to hydroxylamines causes dose-dependent haematological toxicity which impairs patient tolerance. In the 1990s cimetidine-mediated partial inhibition of the oxidative metabolism of dapsone was demonstrated clinically to lower methaemoglobin formation and subsequent case studies with fragile and immunosuppressed patients with multiple complex pathologies, showed cimetidine could minimize dapsone-mediated methaemoglobin formation in the place of methylene blue, when G6PD status is unknown, is defici
