Ultrasound-Guided versus Thoracoscopic Pleural Biopsy for Diagnosing Tuberculous Pleurisy Following Inconclusive Thoracentesis: A Randomized, Controlled Trial.

BACKGROUND Traditional diagnostic methods for tuberculosis (TB) cannot be reliably applied to tuberculous pleurisy. Therefore, this prospective, randomized, controlled trial was performed to compare the diagnostic sensitivity and safety of ultrasound-guided cutting-needle pleural biopsy versus thoracoscopic pleural biopsy in patients suspected of tuberculous pleurisy following inconclusive thoracentesis. MATERIAL AND METHODS A total of 196 adult patients with acid-fast bacillus (AFB)-negative exudative pleural effusions clinically suspected of tuberculous pleurisy were recruited. Enrollees wer

Pleural Pressure Pulse in Patients with Pleural Effusion: A New Phenomenon Registered during Thoracentesis with Pleural Manometry.

Pleural manometry enables the assessment of physiological abnormalities of lung mechanics associated with pleural effusion. Applying pleural manometry, we found small pleural pressure curve oscillations resembling the pulse tracing line. The aim of our study was to characterize the oscillations of pleural pressure curve (termed here as the pleural pressure pulse, PPP) and to establish their origin and potential significance. This was an observational cross-sectional study in adult patients with pleural effusion who underwent thoracentesis with pleural manometry. The pleural pressure curves rec

Post-Thoracentesis Ultrasound versus Chest Radiography for the Evaluation of Effusion Evacuation and Lung Reexpansion: A Multicenter Study.

Rationale: Post-thoracentesis chest radiography (CXR) is often used to evaluate the degree of residual fluid after thoracentesis. Whether post-drainage ultrasound examination is comparable to CXR in the evaluation of pleural space evacuation is unknown. Objectives: How do post-thoracentesis ultrasound and CXR compare in assessing the effectiveness of pleural space evacuation? Methods: In this prospective, multicenter study, patients with free-flowing pleural effusions with minimal to no septations requiring thoracentesis were recruited. Post-thoracentesis ultrasound was performed immediately p

Evaluation of the risk of intercostal artery laceration during thoracentesis in elderly patients by using 3D-CT angiography.

OBJECTIVE: Our study was undertaken to determine the location of the tortuous intercostal artery in elderly patients by using 3D-CT angiography in order to prevent laceration during thoracentesis. METHODS: We evaluated the data of 3D-CT angiography of the intercostal artery in consecutive patients who had undergone contrast chest CT scan in our hospital from December 2007 to April 2008. We considered the "percent safe space" (the shortest lower rib-to-intercostal artery distance/the upper rib-to-lower rib distance) to be an index of safety that can be used to prevent laceration of the interco

Suction pressures generated during thoracentesis using wall suction-based automated drainage: an in vitro and in vivo analysis.

Equipoise exists regarding the optimal method to drain pleural fluid during thoracentesis. While several institutions use wall-based automated suction, others point to the risk of excessively high suction pressures and therefore elevated barotrauma risk as a reason to avoid it. We first performed in vitro experiments involving drainage of a 1-liter saline bag using standard thoracentesis apparatus, a digital manometer, and either manual drainage (using a 60 mL syringe) or automated drainage (using wall suction at the maximum setting). The proceduralist was blinded to measurements during manual

Impact of therapeutic thoracentesis and pleural pressure changes on breathing pattern, dyspnea, lung function, and arterial blood gases.

INTRODUCTION: Therapeutic thoracentesis is highly effective in providing symptomatic improvement in patients with large volume pleural effusion (PE). However, some physiological effects of pleural fluid (PF) withdrawal are still not fully elucidated. OBJECTIVES: The study aimed to evaluate alterations in the breathing pattern, pulmonary function, and arterial blood gases (ABG) in relation to both withdrawn PF volume and pleural pressure (Ppl) changes in patients undergoing therapeutic thoracentesis. PATIENTS AND METHODS: This prospective, observational, cross‑sectional study included 37 pati

Accuracy of Fully Automated and Human-assisted Artificial Intelligence-based CT Quantification of Pleural Effusion Changes after Thoracentesis.

Quantifying pleural effusion change at chest CT is important for evaluating disease severity and treatment response. The purpose of this study was to assess the accuracy of artificial intelligence (AI)-based volume quantification of pleural effusion change on CT images, using the volume of drained fluid as the reference standard. Seventy-nine participants (mean age ± SD, 65 years ± 13; 47 male) undergoing thoracentesis were prospectively enrolled from October 2021 to September 2023. Chest CT scans were obtained just before and after thoracentesis. The volume of pleural fluid on each CT scan, w

Incremental value of pocket-sized imaging device for bedside diagnosis of unilateral pleural effusions and ultrasound-guided thoracentesis.

OBJECTIVES: The present study aimed to assess the additional value of a pocket-sized imaging device (PSID) as an adjunct to plain chest X-rays in the diagnosis of pleural effusion (PE), mainly for those requiring pleural thoracentesis. METHODS: We performed a thoracic ultrasound examination using a PSID in 73 patients with an abnormal chest X-ray diagnostic for unilateral PE. Abundant PE was defined as an interpleural distance between the diaphragm and visceral pleura (VP) of ≥ 30 mm at the apex of the 50 mm bisector line of the costodiaphragmatic recess at end expiration. RESULTS: According

Henry Ingersoll Bowditch: The Prominent Physician Who Introduced Thorax Examination and Trocar Thoracentesis in the United States of America.

Born in a wealthy family with a tradition in science, Henry Ingersoll Bowditch (1808-1882) with studies at Harvard Medical School and in Europe had succeeded in leaving his mark in the American history of medicine. He had been a pioneer in the stethoscope's use, which was promoted and suggested to all physicians of his era. He had widely used thoracentesis, an ancient procedure, for pleuritic effusions, diagnosed with a stethoscope. Inside his most popular treatise "The Young Stethoscopist," he had given a plethora of data concerning the auscultation of the lungs, heart, and vessels; obstetric

Preliminary Evaluation of a Novel Neural Network-Based Hybrid Simulator for Surgical Training and Performance Assessment of Neonatal Thoracentesis.

INTRODUCTION: Tension pneumothorax is a rare and life-threatening situation in neonates requiring immediate intervention through thoracentesis. Significant complications can arise while performing thoracentesis in the case of inadequate skill level or exposure to the condition. Although simulation-based training (SBT) has proven to be effective in learning surgical skills, training sessions are long, subjective, and expensive, because of which they cannot be held regularly. This article attempts to improve traditional SBT for neonatal thoracentesis through an autonomous simulator that can prov

Reexpansion Pulmonary Edema Following Postoperative Thoracentesis After Combined Coronary Artery Bypass Grafting and Mitral Valve Surgery: A Case Report.

Reexpansion pulmonary edema (REPE) is an uncommon but potentially life-threatening complication following rapid evacuation of air or fluid from the pleural space. Diagnosis can be challenging in postoperative cardiothoracic patients because clinical and radiographic findings may overlap with more common causes of acute respiratory failure, including cardiogenic pulmonary edema and fluid overload. A 55-year-old female underwent urgent triple-vessel coronary artery bypass grafting and mitral valve repair for acute ST-segment elevation myocardial infarction. Her postoperative course was complicat

The Impact of Gravity vs Suction-driven Therapeutic Thoracentesis on Pressure-related Complications: The GRAVITAS Multicenter Randomized Controlled Trial.

BACKGROUND: Thoracentesis can be accomplished by active aspiration or drainage with gravity. This trial investigated whether gravity drainage could protect against negative pressure-related complications such as chest discomfort, re-expansion pulmonary edema, or pneumothorax compared with active aspiration. METHODS: This prospective, multicenter, single-blind, randomized controlled trial allocated patients with large free-flowing effusions estimated ≥ 500 mL 1:1 to undergo active aspiration or gravity drainage. Patients rated chest discomfort on 100-mm visual analog scales prior to, during, a

Thoracentesis with pleural fluid cytology versus thoracoscopy with pleural biopsy for the diagnosis of malignant pleural effusion: a cost-effectiveness study.

BACKGROUND: Thoracentesis with pleural fluid (PF) cytology has a sensitivity of ∼60% to diagnose malignant pleural effusion (MPE). Medical thoracoscopy with pleural biopsy has a higher diagnostic sensitivity. This study evaluates the cost-effectiveness of thoracentesis versus thoracoscopy for diagnosis of MPE in patients with prior or active malignancy. RESEARCH QUESTION: When is thoracentesis or thoracoscopy cost-effective for the diagnosis of malignant pleural effusion in patients with prior or active malignancy? STUDY DESIGN AND METHODS: A cost-effectiveness model was developed comparing

Ultrasound-guided thoracentesis for diagnosing Nocardia beijingensis-related empyema in a patient with systemic lupus erythematosus: A rare case report.

Infections caused by Nocardia species are relatively rare and primarily affect immunocompromised patients. There are few reported cases of Nocardia beijingensis causing empyema in patients with systemic lupus erythematosus (SLE). We present a case of a 49-year-old female patient with SLE on immunosuppressive therapy (corticosteroids and azathioprine), admitted with dyspnea and left-sided pleuritic chest pain. A diagnosis of large parapneumonic left pleural effusion was established based on blood tests, blind thoracentesis findings, and bronchoalveolar lavage results. The patient received empir
