Subarachnoid-pleural fistula: applied anatomy of the thoracic spinal nerve root.

Subarachnoid-pleural fistula (SPF) is a rare complication of chest or spine operations for neoplastic disease. Concomitant dural and parietal pleural defects permit flow of cerebrospinal fluid into the pleural cavity or intrapleural air into the subarachnoid space. Dural injury recognized intraoperatively permits immediate repair, but unnoticed damage may cause postoperative pleural effusion, intracranial hypotension, meningitis, or pneumocephalus. We review two cases of SPF following surgical intervention for chest wall metastatic disease to motivate a detailed review of the anatomy of neural

Thoracic nerve root hemangioblastoma: a rare cause of posterior mediastinal mass.

BACKGROUND: Hemangioblastomas are benign tumors that occur sporadically or as part of von Hippel-Lindau disease. Occasionally, they have an extramedullary location along the nerve roots and, rarely, are purely extradural. CASE DESCRIPTION: We report a rare case of a completely extradural large hemangioblastoma in a 32-year-old man presenting with radiculopathy and a posterior mediastinal mass. Screening revealed a similar tumor in the right cerebellar hemisphere. The patient underwent a right paramedian suboccipital craniotomy and total excision of the cerebellar tumor, followed by a left lat

Familial long thoracic nerve palsy: a manifestation of brachial plexus neuropathy.

Long thoracic nerve palsy causes weakness of the serratus anterior muscle and winging of the scapula. It is usually traumatic in origin. Isolated long thoracic nerve palsy has not been recognized as the major manifestation of familial brachial plexus neuropathy, but I have studied the syndrome in four members of three generations of one family. One individual suffered an episode of facial paresis. The inheritance pattern was autosomal dominant.

Guiding Device for Precision Grafting of Peripheral Nerves in Complete Thoracic Spinal Cord Injury: Design and Sizing for Clinical Trial.

BACKGROUND: In an effort to translate preclinical success in achieving spinal cord regeneration through peripheral nerve grafts, this study details the design and sizing of a guiding device for precision grafting of peripheral nerves for use in a clinical trial in complete (AIS-A) thoracic spinal cord injury (SCI). The device's design and sizing are compared to a simulation of human spinal cord sizes based on the best available data. METHODS: Spinal cord segmental sizes were generated by computer simulation based on data from a meta-analysis recently published by our group. Thoracic segments

The lateral thoracic nerve and the cutaneous maximus muscle--a novel in vivo model system for nerve degeneration and regeneration studies.

We report a novel in vivo mouse model system to study regeneration of injured motor nerve and spatiotemporal pattern of denervation in experimental nerve diseases. The lateral thoracic nerve (LTN), as a pure motor nerve, innervates the cutaneous maximus muscle (CMM) by some of the shortest and the longest motor nerve fibers in the mouse body. Its branches and nerve terminals can be imaged in whole mount preparations. Here we describe the branching pattern of the LTN and its innervation of the CMM, and characterize degeneration and regeneration over time after a LTN crush by morphological and e

A simple sonographic approach to thoracic transforaminal epidural injections for zoster-associated pain involving multiple nerves: an exploratory prospective cohort study.

BACKGROUND: A simple superoposterior approach to thoracic transforaminal epidural injections (TFEIs) under ultrasonographic guidance was proposed to reduce zoster-associated pain (ZAP) involving multiple thoracic nerves and the likelihood of transitioning to postherpetic neuralgia (PHN). METHODS: Patients were prospectively enrolled. Primary endpoints were the burden of illness (BOI) scores and epidural contrast spread. Secondary endpoints included number of needle insertion attempts, sensory blockade, hemodynamic changes, procedure time, radiation dose, adverse events, rescue analgesics, PHN

Extradural thoracic nerve root hemangioblastoma approached by a combined posterior thoracic spine and video-assisted thoracoscopic surgery: A case report.

BACKGROUND: Hemangioblastomas commonly occur in the posterior fossa and are typically attributed to sporadic or familial Von Hippel-Lindau disease. Spinal hemangioblastomas, found in 7-10% of patients, are usually located within the cord (i.e., intramedullary). Here, a 58-year-old male presented with a purely extradural hemangioblastoma involving a spinal root that was surgically excised. CASE DESCRIPTION: A 58-year-old male was admitted with a progressive paraparesis and incomplete sensory deficit. The magnetic resonance imaging documented a solid dumbbell-shaped lesion that extended through

Intracranial hypotension with a sixth cranial nerve palsy subsequent to massive thoracic CSF hygroma: a rare complication of thoracic disc excision.

BACKGROUND: Thoracic cerebrospinal fluid (CSF) hygroma is a rare and potentially devastating complication of the anterior thoracic approach to the spine. We present two cases in which this complication resulted in acute cranial nerve palsy and discuss the pathoanatomy and management options in this scenario. CASE REPORTS: Two male patients presented to our department with neurological deterioration due to a giant herniated thoracic disc. The extruded disc fragment was noted pre-operatively to be calcified in both patients. A durotomy was performed at primary disc prolapse resection in the fir

Two mammalian species in which the intercostal nerves innervate the serratus anterior or scalenus muscles together with the cervical nerves: an important clue to clarify the homology of cervico-thoracic trunk muscles in mammals.

The levator scapulae, rhomboideus, and serratus anterior muscles (as a group referred to the dorsal shoulder girdle muscles) and the scalenus muscles in mammals are usually innervated by cervical nerves. However, in koalas, the serratus anterior is additionally innervated by the lateral cutaneous branch of the first intercostal nerve. In cats, as in some other mammalian species, a part of the scalenus muscle (scalenus longus muscle) is innervated by the lateral cutaneous branches of the intercostal nerves. A precise comparison of the innervating nerves at the same segment in these two cases co

Selective Dorsal Scapular Nerve and Long Thoracic Nerve Blocks for Rescue Analgesia in Scapulothoracic Arthrodesis Surgery: A Case Report.

Scapulothoracic arthrodesis (STA) surgery is performed to stabilize the scapula in patients with facioscapulohumeral dystrophy (FSHD). Postoperative pain could be a major problem even while using erector spinae plane block (ESPB). We performed a preoperative ESPB with an intraoperative ESPB catheter, but rescue analgesia was needed for pain in the periscapular area in the postoperative period. The patient's pain score was reduced by applying an ultrasound-guided dorsal scapular nerve (DSN) and long thoracic nerve (LTN) block. Selective DSN and LTN blocks can be effective in enhancing postopera

Restoration of elbow flexion by performing contralateral lateral thoracic and thoracodorsal nerve transfers after experimental musculocutaneous nerve transection.

OBJECT: The immediate transfer of the right lateral thoracic nerve (LTN) and the thoracodorsal nerve (TDN) to the transected left musculocutaneous nerve (MCN), leading to nerve cross-neurotization, was performed in cats to evaluate reinnervation of the biceps brachii muscle (BBM). METHODS: Surgery to produce cross-neurotization of the MCN was performed in 12 cats (treatment group). Transection of the MCN was performed without attempts at neurotization in three cats (control group). Reinnervation of the BBM was assessed by performing electromyography (EMG) 6 months (14 cats) and 26 months (one

Revisiting thoracic splanchnic nerve anatomy: implications for thoracoscopic splanchnicectomy and pain management.

PURPOSE: The thoracic splanchnic nerves-greater (GSN), lesser (LSN), and least (lSN)-are important targets in thoracic surgery and image-guided interventions, yet detailed quantitative characterization of their anatomy remains limited. This study aimed to define the morphometry, segmental origins, and anatomical variability of these nerves to improve understanding of clinically relevant anatomy. METHODS: Eighty sets of thoracic splanchnic nerves from 42 formalin-embalmed donors were dissected. Segmental contributions were recorded and classified, and morphometric measurements were obtained re

Teaching neuroimages: swollen T1 nerve root in neurogenic thoracic outlet syndrome.

A 17-year-old girl without any history of considerable trauma or previous disease was admitted with weakness and wasting in the right forearm and hand, more marked on the thenar eminence (figure). EMG revealed unrecordable medial antebrachial cutaneous sensory and low-amplitude median motor responses (recorded from thenar muscles) along with chronic neurogenic changes in the hand muscles. MRI showed a fusiform focal expansion of the right T1 root (figure). Neurogenic thoracic outlet syndrome (NTOS) was diagnosed and surgically decompressed.1

Paresis of cranial nerve VI (N. abducens) after thoracic dural perforation.

Epidural anesthesia is associated with the risk of unintended dural perforation and concomitant leakage of cerebrospinal fluid (CSF) from the subarachnoidal space. This may remain asymptomatic or trigger post-dural puncture headache (PDPH). Cerebral nerve lesions after attempted epidural anesthesia are very rare. Here we report a case of unilateral paresis of the cranial nerve VI (N. abducens) after attempted thoracic epidural anesthesia. Herein, diagnosis of N. abducens paresis was probably delayed because the optical symptoms, such as blurred and double vision, were attributed to optical hal

An unusual presentation of whiplash injury: long thoracic and spinal accessory nerve injury.

Whiplash injuries from motor vehicle accidents are very common. The usual presentation and course of this condition normally results in resolution of symptoms within a few weeks. Brachial plexus traction injuries without any bone or joint lesion of the cervical spine have been reported before. We report a case where a gentleman was involved in a rear end vehicle collision, sustained a whiplash injury and was later found to have a long thoracic nerve palsy and spinal accessory nerve palsy. Although isolated injuries of both nerves following a whiplash injury have been reported, combined injury

Generalized peripheral nerve failure during thoracic spine surgery: a case report.

OBJECTIVE: Intra-operative systemic changes impairing peripheral nerve function are not commonly detected with electrophysiology. This case presentation illustrates how somatosensory evoked potential (SSEP) monitoring can detect global changes in peripheral nerve excitability during spine surgery. METHODS: A posterior thoracic spine fixation was performed on a young male with multiple traumatic injuries. Bilateral tibial nerve SSEPs were intraoperatively recorded, along with the right median nerve SSEP for control. RESULTS: A rapid, progressive loss of tibial and median nerve potentials (fol
