Malignant fibrous histiocytoma arises most commonly de novo and rarely from sites of chronic inflammation. The authors present a case of malignant fibrous histiocytoma arising in a thoracotomy hypertrophic scar. The patient required large local excision of the tumor down to and including the muscle layer with postoperative radiation treatment. Factors associated with the occurrence of this sarcoma are discussed along with salient management principles.

The authors report their experience with transhiatal esophageal resection accumulated during the period between January 1978 and March 1990. Indications for the procedure included cancer of the gastric cardia (26.3%), cancer of the hypopharynx (3.8%), cancer of the esophagus (59.2%), and benign esophageal disease (9.8%). Transcatheter plugging of patent ductus arteriosus with modified Porstmann's method was performed in children of school age and adults (130 cases) in recent 10 years. This paper describes our experience in performing saphenous vein bypass grafts to the circumflex coronary artery system with a left thoracotomy in 9 patients. A 53-year-old man underwent surgical repair and drainage of a spontaneous esophageal rupture through a left anterolateral intercostal thoracotomy. We report the case of a 15-year-old boy with a single left ventricle who underwent total cavopulmonary connection (Fontan circulation). One hundred and ten patients underwent gastroesophageal resections performed transdiaphragmatically by way of a left thoracotomy. Eighty-one patients had tumors distal to the inferior pulmonary vein and had en bloc dissections, while the remaining 29 had midthoracic tumors, which required a two-rib thoracotomy. There were 73 men and 37 women with a median age of 64 years (a range of 40 to 75 years). The postoperative mortality rate was 2.7 per cent, and morbidity, mainly caused by respiratory complications, occurred in 21 patients (19 per cent), with no patients requiring ventilatory support.

A group of patients with carcinoma of the oesophagus and cardia has been studied. Subtotal oesophagectomy without thoracotomy has been performed. The operation is briefly described, the complications and results are discussed and it is concluded that the operation of subtotal oesophagectomy without thoracotomy does have a place in the treatment of selected patients with carcinoma of the oesophagus.

The surgical management of tumors of the left main bronchus with involvement of the lower trachea is one of the most difficult problems of tracheobronchial surgery. Two cases of adenocystic carcinoma in this location are presented, where resection of the tumor and reconstruction of the airway were performed through a left thoracotomy. METHODS: We retrospectively reviewed 53 neonates who underwent thoracotomy from January 2017 to December 2019 for a period of 3 years.
