The thymus is a complex, highly specialized organ that is derived from the third and fourth pharyngeal pouches and contains elements that originate from all three germinal layers. As such, it shows certain specific and distinctive reaction patterns to injury that are frequently encountered in a variety of reactive and inflammatory conditions. These reaction patterns, however, can also often be associated with neoplastic diseases arising from this organ, and in some instances, their presence may contribute to obscure the malignant nature of the underlying process. We present a review of the mai

Massive thymic hyperplasia (MTH) is rare in the pediatric age group, especially in infants. However, because of a wide variation in size and weight of the thymus, an enlarged gland is often resected because of suspicion of a neoplasm or a cyst. Some cases of thymoma resembling pulmonary acute infection occur less frequently than MTH, but if respiratory problems are accompanied by a large thymus immediate diagnosis is often necessary to differentiate between these two conditions. Four infants (14 days to 4 months of age) with MTH were recently studied, all having an acute onset of a severe resp

Spindle cell lesions of the thyroid gland (T-SCL) are not encountered routinely in clinical practice or in the context of thyroid pathology. They commonly are classified as primary or secondary to metastatic disease. Primary T-SCL can be derivedfromfollicular, C-cell (parafollicular), or mesenchymal components and may be the result of reactive or neoplastic processes, including post-fine-needle aspiration spindle cell nodules, Riedel thyroiditis, solitary fibrous tumor, leiomyoma, peripheral nerve sheath tumor, hyalinizing trabecular tumor, spindle epithelial tumor with thymus-like differentia

Thymoma, the most common tumor mediastinum, is a neoplasm arising from the epithelial cells of thymus. Nearly all thymomas present in adult life. Thymomas in children are exceptional. Chest X-ray, CT, magnetic resonance and fine-needle aspiration help for diagnosis. Surgical resection represents the treatment of choice and the outcome of surgery has been shown to depend on the local invasiveness of the tumor.

Carcinomas of the thymus are now well recognized as distinctive but rare entities, and several clinicopathologic variants of such neoplasms have been described. These include keratinizing squamous cell carcinoma, nonkeratinizing squamous cell carcinoma, lymphoepithelioma-like carcinoma, neuroendocrine carcinoma, adenosquamous carcinoma, mucoepidermoid carcinoma, clear-cell carcinoma, papillary adenocarcinoma, adenocarcinoma not otherwise specified, basaloid carcinoma, and sarcomatoid carcinoma. The application of electron microscopy, immunohistology, and other adjunctive pathological technique

BACKGROUND: Most thymic neoplasms fall under the designation of thymoma, consisting of well-differentiated epithelial cells, resembling normal thymus. At the opposite spectrum are thymic carcinomas; the cell of origin while similar is malignant. Recently a third category of thymic neoplasms, atypical thymomas, has been recognized representing thymic neoplasms manifesting atypia although without overt cytomorphologic criteria of malignancy.
METHODS: Seven patients with a diagnosis of atypical thymoma were encountered over a 6-year period from the patient files of the cardiothoracic division of

Neuroendocrine neoplasms (NENs) are a heterogeneous group of epithelial neoplastic proliferations that irrespective of their primary site share features of neural and endocrine differentiation including the presence of secretory granules, synaptic-like vesicles, and the ability to produce amine and/or peptide hormones. NENs encompass a wide spectrum of neoplasms ranging from well-differentiated indolent tumors to highly aggressive poorly differentiated neuroendocrine carcinomas. Most cases arise in the digestive system and in thoracic organs, i.e., the lung and thymus. A correct diagnostic app

Primary mediastinal tumors, with the exception of lymphoma, are generally rare neoplasms. The diagnosis, classification, and treatment of these tumors still cause some degree of difficulty due to their low incidence and morphologic heterogeneity. This is particularly true for thymic epithelial neoplasms, that is, thymoma and thymic carcinoma, and also applies to primary neuroendocrine carcinomas of the thymus and primary mediastinal germ cell tumors. The appropriate staging of these tumors, likewise, has been a matter of debate over the years and numerous proposals for the staging of thymic ep

Thymomas are rare neoplasms of the thymus and are often associated with immune-mediated paraneoplastic syndromes, most commonly, myasthenia gravis. The same underlying mechanism can produce antibodies to other self-antigens in various organ systems. Autoimmune hepatitis is a rare complication of thymoma. We present a 35-year-old healthy male, initially thought to have drug-induced liver injury, who was subsequently diagnosed with thymoma-induced autoimmune hepatitis, a rare syndrome of which only two previous cases have been reported.

Lymphoepithelial neoplasms are a heterogeneous group of biphasic tumors that contain both epithelial and lymphoid components. Most of these tumors are primarily epithelial with a secondary lymphoid infiltrate and illustrate the basic cell trafficking that occurs in the skin. Included are benign cutaneous lymphadenoma, malignant lymphoepithelioma-like carcinoma, heterotopic dermal thymus, and several miscellaneous lesions that may have a prominent lymphoid infiltrate. Familiarization with their characteristic features will aid in the recognition of these relatively uncommon but often distinct e

Thymic epithelial neoplasms are malignant lesions that originate from the thymus and include thymoma, thymic carcinoma, and thymic neuroendocrine tumors. Although computed tomography (CT) is typically considered the imaging modality of choice for identifying thymic tumors, characterizing the primary neoplasm, and staging of disease, the role of magnetic resonance (MR) imaging continues to expand. MR imaging is effective in distinguishing thymic epithelial neoplasms and other malignant tumors from benign lesions in the prevascular mediastinum, can be used to characterize and stage thymic tumors

In this report, a case of metaplastic thymoma arising in a thymic cyst wall is discussed. The patient was a 61-year-old male whose chief complaint was sweating and chest pain. Imaging study revealed a mediastinal mass with right hemithoracic extension. He underwent surgical resection of the mass and remnants of the thymus. Histologic studies showed a primary thymic neoplasm with a biphasic histologic pattern composed of 2 distinct epithelial and stromal components arising in a thymic cyst wall. Immunohistochemically, the epithelial component was cytokeratin positive and focally reactive for EM

Primary thymic adenocarcinoma is extremely rare. Moreover, thymic pure epithelial benign neoplasms are extremely rare. We encountered a cystic tumor almost purely composed of goblet cell-like mucus-producing cells of the thymus. A mass lesion of the mediastinum was detected in a 54-year-old man. The gross specimen presented a unilocular cystic lesion containing abundant mucin, measuring 8 × 5.5 × 4.5 cm. Microscopic examination revealed a cystic tumor consisting of bland mucus-producing cells resembling goblet cells and forming tiny daughter cysts within the dense fibrous capsule. No destructi

Masses within the thymus and mediastinum are frequently biopsied to establish a diagnosis. The specimens sent for histopathological analysis are typically small (needle) biopsy specimens on which the pathologist is challenged to provide a classifying diagnosis. Despite the fact that the pathology of the anterior mediastinum is predominantly centred on the thymus and it thus might be expected that the range of possible diagnoses is limited, the list of disease entities is in fact extensive, encompassing hyperplastic conditions and both benign and malignant neoplasms. In addition to primary thym

Aggregates of benign nevus cells occurring in lymph nodes are a well-described incidental finding. Nevus cell aggregates (NCAs) can mimic foci of metastatic carcinoma or other disease processes, so the surgical pathologist should be familiar with this lesion. The purpose of this report is to describe the potential diagnostic difficulties created by benign NCAs within the thymus of a 32-year-old man with dysplastic nevus syndrome and malignant melanoma involving mediastinal lymph nodes and the right lung. Morphologically, the NCAs in this case elicited the differential diagnoses of metastatic m

Thymomas are rare anterior mediastinal tumors that originate in the epithelial cells of the thymus and have a rare propensity to metastasize to extrathoracic locations unless it is a histologic high-grade neoplasm (type B and above). We describe a case of 50-year-old woman diagnosed with type AB thymoma and the role of 18F-fluorodeoxyglucose positron emission tomography-computed tomography in accurate delineation of extrathoracic metastases during initial staging.

Thymolipoma is an uncommon benign neoplasm of the thymus composed of mature adipose and thymic tissue. The diagnosis of thymolipoma should be considered in the case of a mediastinal mass with fat density, especially if it is interspersed with strands of soft tissue attenuation on computed tomography scans. However, it is sometimes difficult to differentiate radiologically between a thymolipoma and other mediastinal fatty tumors. Here we present a patient with a huge mediastinal mass that proved to be a thymolipoma.

Thymic hyperplasia is a rare and benign cause of mass in the anterior mediastinum in adults. This pathology is characterized by an increase in the size and weight of the thymus while preserving its architecture and histology. In most cases, it is asymptomatic, and routine imaging exams are used to make the diagnosis. Thymic hyperplasia can be divided into reactive and true hyperplasia based on morphological characteristics. The main differential diagnosis of this pathology is thymoma, an epithelial neoplasm of the thymus. True thymic hyperplasia is the rarest subtype, and, unlike reactive or l

UNLABELLED: Marginal zone lymphoma is a neoplasm affecting the lymphatic system, including the bone marrow, thymus, spleen, and lymph nodes. This type of non-Hodgkin's lymphoma affects B cells and is estimated to comprise between 5% and 17% of all non-Hodgkin's diseases. The incidence of finding any neoplasm within the foot and ankle has been estimated to be only 2.0% to 3.5% of all patients. However, despite the low incidence of cancer found within the foot and ankle, the clinician must be mindful that the possibility does exist and should thus consider surgically excised soft tissue and bone

Lymphoepithelial carcinoma (lymphoepithelioma) is a neoplasm that may be found in nasopharynx, nasal fossae, maxillary sinus, base of the tongue, tonsil, thymus and parapharyngeal area. Such a tumor is extremely rare in the hypopharynx and the case described is the second so far reported in the world literature, after the first case by Dockerty et al. (1968). The tumor is morphologically identical to those arising in other organs. Our patient died ten months after diagnosis.
