Diagnostic Value of Elastography for Thyroid Nodules in Hashimoto's Thyroiditis.

Objective To evaluate the diagnostic performance of elastography in the diagnosis of thyroid nodules in the context of Hashimoto's thyroiditis(HT). Methods The study evaluated 43 thyroid nodules by conventional ultrasound (CU) and elastography in 38 patients co-existed with HT who were referred for operation. The patients underwent CU and elastography before operation. The sensitivity, specificity, positive and negative predictive values, and accuracy for CU,elastography, and combination of these two techniques were assessed by using histopathological results as the gold standard. Results Among th

Thyroid nodules and thyroid autoimmunity in the context of environmental pollution.

Evidence suggests that in most industrialized countries autoimmune disorders, including chronic lymphocytic thyroiditis, are increasing. This increase parallels the one regarding differentiated thyroid cancer, the increment of which is mainly due to the papillary histotype. A number of studies have pointed to an association between chronic lymphocytic thyroiditis and differentiated thyroid cancer. The upward trend of these two thyroid diseases is sustained by certain environmental factors, such as polluting substances acting as endocrine disrupting chemicals. Herein we will review the experime

Ultrasonographic features of thyroid nodules accompanied by Hashimoto thyroiditis.

OBJECTIVE: To compare the ultrasound imaging characteristics of benign and malignant thyroid nodules on the background of Hashimoto's thyroiditis (HT). METHODS: Totally 120 consecutive patients with pathologically confirmed benign (n=60) or malignant (n=60) thyroid nodules in Peking Union Medical College Hospital since January 2013 were retrospectively analyzed, and their preoperative ultrasound images were reviewed. RESULTS: Sixty cases (100%) of the malignant nodules were papillary thyroid cancers. In the benign group, 33 cases (55.0%) had nodular changes of HT, 26 (43.3%) had nodular goit

Thyroid storm triggered by thyroid nodule fine needle aspiration biopsy: a case report.

BACKGROUND: Thyroid storm is a potentially fatal thyrotoxicosis triggered by an event, such as manipulation of the thyroid gland, acute iodine load, trauma, or infection. Prior to deciding on fine needle aspiration (FNA) biopsy, patients who have been diagnosed with hyperthyroidism or low thyroid stimulating hormone and multinodular goiter (MNG) should be imaged via radionuclide thyroid scan. CASE DESCRIPTION: We present a case of a 62-year-old female patient with history of MNG, who had thyrotoxicosis on presentation due to medication noncompliance and was found to have Graves' disease. Investigating the Effect of Thyroid Nodule Location on the Risk of Thyroid Cancer.

Background: Thyroid nodules are routinely evaluated with ultrasound. Our aim was to determine if thyroid nodule location was a useful feature to predict thyroid cancer. Materials and Methods: Retrospective review of patients with thyroid nodules from six referral centers from 2006 to 2010. A total of 3313 adult patients with thyroid nodules and confirmed benign or malignant thyroid diagnoses were included. Results: Mean patient age was 54.2 (18-97) years, and the majority were women (n = 2635, 79.8%). A total of 3241 nodules were analyzed, 335 (10.3%) of which were malignant. Thyroid nodule lo

The Impact of Hashimoto Thyroiditis on Thyroid Nodule Cytology and Risk of Thyroid Cancer.

CONTEXT: The impact of Hashimoto thyroiditis (HT) on the risk of thyroid cancer and its accurate detection remains unclear. The presence of a chronic lymphocytic infiltration imparts a logical mechanism potentially altering neoplastic transformation, while also influencing the accuracy of diagnostic evaluation. METHODS: We performed a prospective, cohort analysis of 9851 consecutive patients with 21,397 nodules ≥1 cm who underwent nodule evaluation between 1995 and 2017. The definition of HT included (i) elevated thyroid peroxidase antibody (TPOAb) level and/or (ii) findings of diffuse hetero

Microcalcifications without a thyroid nodule as the sole sign of papillary thyroid carcinoma.

SUMMARY: Our objective is to demonstrate the importance of considering microcalcifications even without evidence of nodules as a potential sign of malignancy. Current guidelines, such as those of the British Thyroid Association, acknowledge the clinical significance of microcalcifications only when found within nodules. In this case, they are considered a suspicious feature, classifying the nodules as U5 (i.e. high risk) where fine-needle aspiration biopsy (FNAB) is warranted, following the high likelihood of cancer in these nodules. In addition, there is a dearth of evidence of ultrasound sca

Rapid thyroid nodule growth is not a marker for well-differentiated thyroid cancer.

BACKGROUND: Rapid growth of thyroid nodules is described as being associated with thyroid cancer. The objective of the study was to determine how the growth rate of thyroid nodules during follow-up is associated with the risk of thyroid cancer. METHODS: Retrospective analysis of patients undergoing thyroid surgery for nodular disease and a repetitive preoperative ultrasound work-up of at least 6 months was done. Nodule growth was considered relevant when a volume increase >49% was detected. Growth patterns were described as rapid for a volume increase present over 6 to 24 months. RESULTS: Of

Long-term prognosis of thyroid nodule cases compared with nodule-free controls in atomic bomb survivors.

CONTEXT: Radiation exposure is associated with development of thyroid nodules. The long-term risk of thyroid cancer development in irradiated people with thyroid nodules, however, has not been clarified. OBJECTIVE: The objective of this study was to assess the long-term risk of cancer development in irradiated individuals with thyroid nodules. DESIGN, SETTING, AND PARTICIPANTS: This prospective study comprised 2637 atomic bomb survivors (mean age, 59 yr; 1071 men and 1566 women) who participated in the baseline thyroid study of the Nagasaki Radiation Effects Research Foundation from 1984 thr

Solitary benign thyroid nodule in an elderly female cadaver.

A thyroid nodule (TN) is a structurally discrete lesion within the thyroid gland that can be readily distinguished from the adjoining normal thyroid parenchyma. TNs are more frequently detected these days because of the widespread use of high-resolution imaging techniques for various neck pathologies. A well-circumscribed macroscopic lesion measuring 17 mm ×14 mm ×12 mm was observed in the left lobe of the thyroid gland during routine dissection performed on an embalmed elderly female cadaver for undergraduate teaching. It was firm to the touch and greyish brown in color. In the histological e

Dual Autonomously Functioning Thyroid Nodule: An Unusual Imaging Finding.

A 48-year-old female, a known case of ulcerative colitis, on routine workup, was incidentally detected to have biochemical hyperthyroidism. 99mTc pertechnetate thyroid scintigraphy performed for the evaluation of hyperthyroidism revealed two foci of increased tracer uptake in the right thyroid lobe with suppressed uptake in the rest of the thyroid gland consistent with dual autonomously functioning thyroid nodules.

[Use of clinical ultrasound in primary care: Thyroid nodule].

The thyroid nodule is a frequent lesion within the thyroid. Its incidence is between 50%-60% and most of them are asymptomatic and benign (90-95%) and its diagnosis is frequently made as an incidental finding. The main objective of their study is to rule out thyroid cancer. Cervical ultrasound is the non-invasive diagnostic technique of choice to study the thyroid gland and its pathology, including the thyroid nodule, as it is the technique with the best cost-benefit ratio. Together with the anamnesis, physical examination, personal and family history and laboratory tests, it enables the first

Diagnosis of Thyroid Nodule with New Ultrasound Imaging Modalities.

Introduction: B-mode ultrasound (US) technology is an integral part of diagnosing and assessing risk stratification of thyroid nodules (TNs). The addition of shear wave elastography and three-dimensional (3D) US imaging may improve risk stratification for thyroid cancer (TC). Materials and Methods: The patient was evaluated in our clinic with US imaging including B-mode, shear wave elastography, 3D-US, and fine needle aspiration biopsy (FNAB). Laboratory measurements were performed at LabCorp. The patient gave informed consent. Case: A 20-year-old female referred for hypothyroidism who was on

Simple core-needle biopsy for thyroid nodule, complicated tinnitus.

BACKGROUND: Fine-needle aspiration is the procedure of choice for evaluating thyroid nodules. Core-needle biopsy (CNB) is not included in the American Thyroid Association recommendations for evaluating such nodules. CNB complications are classically bleeding and hematomas. To our knowledge, no case of arteriovenous fistula (AVF) secondary to a CNB has been reported, nor has any case of tinnitus secondary to a post-CNB AVF. OBJECTIVES: To make the clinician aware of possible vascular complications caused by CNB and the possibility of difficult pathology reading caused by previous CNB. METHODS

Autonomously functioning thyroid nodule associated with thyrotoxic periodic paralysis.

Thyrotoxic periodic paralysis (TPP) is mainly associated with Graves' disease but rarely with autonomously functioning thyroid nodule (AFTN). We herein report a case of AFTN associated with TPP in which the latter resolved after (131) I therapy for the former. We analyzed the genes encoding thyrotropin receptor (TSHR), the alpha-subunit of the stimulatory G protein (Gsalpha), calcium channel CACNA1S and potassium channel KCNE3, and found that the patient does not carry the known mutations in these genes. Whereas the pathogenesis of TPP and AFTN remains to be understood, the present case sugges

Percutaneous ethanol injection therapy for autonomously functioning thyroid nodule.

Four patients with solitary autonomously functioning thyroid nodule (AFTN; 2 toxic and 2 subclinically toxic) received ultrasonography (US)-guided percutaneous ethanol injection therapy (PEIT). The pretreatment scintigraphic appearance of the nodule was hot, and radioactivity in the extranodular tissue was completely suppressed throughout. Ninety-nine percent ethanol was slowly injected under US guidance. As a rule, the injection was performed in fractionated sessions and the treatment was repeated until the total amount of ethanol exceeded the baseline nodular volume. The therapy was successf

Coccidioidomycosis masquerading as thyroid nodule: a rare diagnostic challenge.

We present a compelling case of disseminated coccidioidomycosis involving the thyroid gland, a remarkably uncommon manifestation of this infection. The gravity of this sporadic disease is underscored by its high mortality rate, primarily because of challenges in timely diagnosis and treatment initiation. Accurate diagnosis hinges upon utilizing various techniques, including the culture of a fine-needle aspirate, biopsy and direct microscopy. However, the medical community is still grappling with the optimal treatment strategy, encompassing considerations such as duration and dosage of medicati
