Graves' disease and thyroxine-binding globulin deficiency.

Thyroxine-binding globulin (TBG) deficiency has been frequently described in single patients and in many families. Most people with abnormal TBG concentrations are euthyroid. Cases of Graves' disease and TBG deficit have rarely been reported. We describe the case of a person with Graves' disease and TBG deficiency. Because of this condition, the patient had a misdiagnosis during part of his clinical history, and therefore underwent unnecessary therapy.

Erythrocyte ouabain binding in patients receiving thyroxine.

Thyroid hormone action at the cellular level was investigated in euthyroid women who were receiving replacement thyroxine, and whose plasma T4 levels were above the upper reference limit for healthy subjects. There is evidence that erythrocyte sodium pump sites are reduced in number in patients with hyperthyroidism. These sites were measured by the ouabain binding capacity. Plasma T4, free T3 and TSH were also measured, the latter by a high sensitivity fluoroimmunoassay. Three groups of women were investigated; 30 patients receiving T4 with elevated plasma T4 concentrations, 30 age-matched hea

Serum-thyroxine levels in microwave-exposed rats.

The nature of the response of the thyroid gland in animals exposed to microwave irradiation is controversial. An enlarged thyroid and an increase of radioiodine uptake in microwave workers have been reported. Absence of thyroid disorders has also been reported in other exposed populations. Animal experimentation has contributed to the controversy because both increased and decreased thyroid functions have been reported. The thyroxine concentration in rats as representative of thyroid function in animals exposed to 2.45-GHz, 120-Hz amplitude-modulated microwaves has been studied. Comparison was

Polymorphism of human plasma thyroxine binding prealbumin.

Amyloid fibrils from an individual with heredofamilial amyloidosis were found to be composed of plasma prealbumin. To study this protein a three step procedure to isolate prealbumin from plasma was developed. It entailed ion exchange chromatography on DEAE Sephadex, affinity chromatography on Affi-gel Blue and gel filtration on AcA-34. Trypsin Digests of prealbumin were separated by reverse phase HPLC and the pattern compared to that from the normal protein. Only one unexpected peptide was found and it represented the substitution of a methionine for a valine at position 30 in the molecule. Th

Synaptosomal [125I]triiodothyronine after intravenous [125I]thyroxine.

We administered [125I]thyroxine intravenously to adult male rats and measured uptake and subcellular distribution of the hormone and its metabolites in brain. Fractional brain uptake decreased after a large dose of iodothyronine, providing evidence for saturability of the uptake mechanism. Well-defined patterns of regional and subcellular labeling were noted within 1 h after [125I]thyroxine injection. Radioactivity in synaptosomes was always greater than in any other particle separated per gram of brain, increasing linearly relative to radioactivity in brain cytosol during the 1st h. Although

Thyroxine binding globulin deficiency due to a novel HNF1B gene variant in a young girl with high thyroxine requirement.

Primary hypothyroidism is managed with oral levothyroxine (LT4) using a daily dose of 1.5 to 2 µg per kg. Elevated thyroid-stimulating hormone (TSH) despite a high dose of LT4 is a common problem and could be due to multiple etiologies, including poor drug compliance, thyroxine (T4) malabsorption, celiac disease, and consumptive hypothyroidism. A 12-year-old girl was referred with persistently elevated TSH despite using daily 100 µg LT4. Detailed evaluation, including the LT4 absorption test, was normal, and she achieved euthyroidism with a daily LT4 dose of 5.5 µg per kg. Her T4 levels were c

Metabolism of thyroxine-binding globulin in man. Abnormal rate of synthesis in inherited thyroxine-binding globulin deficiency and excess.

It has been previously suggested that inherited thyroxine-binding globulin (TBG) abnormalities in man may be due to mutations at a single X-chromosome-linked locus controlling TBG synthesis. However, abnormalities in TBG degradation have not been excluded. The availability of purified human TBG and its successful labeling with radioiodide allowed us to examine such possibility. Human TBG was purified by affinity chromatography, labeled under sterile conditions with 131I or 125I,, and mixed with [125I]thyroxine (T4) or [131I]T4, respectively, before their intravenous injection. Blood and urine

Elevation of serum free triiodothyronine, total triiodothyronine, thyroxine-binding globulin, and total thyroxine levels in combat-related posttraumatic stress disorder.

BACKGROUND: This study was designed to assess both central and peripheral aspects of thyroid function in combat-related posttraumatic stress disorder (PTSD), with the particular purpose of finding a mechanistic explanation for an imbalance between serum levels of free thyroxine (T4) and total T4 previously observed in pilot work. METHODS: A total of 96 male combat veterans with PTSD diagnosed by DSM-III-R (72 from the West Haven, Conn, Veterans Affairs Medical Center and 24 from the Menlo Park, Calif, Veterans Affairs Medical Center) were compared with 24 male control subjects. Free thyroxine assessed with three assays in sera of patients with nonthyroidal illness and of subjects with abnormal concentrations of thyroxine-binding proteins.

Three methods for estimating free thyroxine (FT4) in serum were studied: equilibrium dialysis, the SPAC-ET FT4 radioimmunoassay kit, and the Amerlite MAB FT4 luminometric assay. Serum samples from 10 subjects with above-normal thyroxine-binding globulin (TBG), 6 with low TBG, 30 with familial dysalbuminemic hyperthyroxinemia (FDH), 13 with nonesterified fatty acids (NEFA) concentrations in serum > 1.0 mmol/L, and 178 patients with various degrees of nonthyroidal illness (NTI) were measured and compared with samples from 42 euthyroid blood donors. The Amerlite MAB FT4 assay compared well with e

Hereditary abnormalities of thyroxine-binding globulin concentration. A study of 19 kindreds with inherited increase or decrease of thyroxine-binding globulin.

Nineteen families with hereditary abnormalities of thyroxine-binding globulin (TBG) have been studied, comprising a total of 15 males with absent TBG, and 15 females heterozygotic for this condition, and 14 males with excess of TBG and 44 heterozygotic females. Hereditary TBG excess was associated with thyrotoxicosis in four instances, and with myxoedema in three. The typical biochemical features of TBG abnormality are described, with the clinical histories of patients in whom diagnostic and management problems occurred.

Beyond the Gut: thyroxine malabsorption, pharmacokinetics, and delivery.

Refractory primary hypothyroidism is defined by persistently elevated thyroid-stimulating hormone despite supraphysiological oral levothyroxine dosing. The distinction between true malabsorption and pseudomalabsorption (nonadherence) is critical for management planning. In 2023, Quiroz-Aldave et al proposed a comprehensive framework for the evaluation and management of refractory primary hypothyroidism, while Liu et al provided an updated overview of both conventional and novel levothyroxine formulations. This mini-review synthesizes recent evidence published since 2023 and presents 2 illustra

Thyroxine 5'-deiodinase in human anterior pituitary tumors.

The activity of T4 5'-monodeiodinase (5'D) in the pituitary was measured in 12 patients with pituitary adenoma (3 patients with acromegaly, 2 with prolactinoma, 1 with Cushing's disease, 1 with TSH-producing tumor, and 5 with nonfunctioning tumor) and, as a control, in a patient who died of parotid cancer. The pituitaries, obtained at operation or autopsy, were homogenized in 0.1 mol/L potassium phosphate buffer, pH 7.0, and centrifuged at 800 x g. Supernatants were incubated with [125I]T4 and 20 mmol/L dithiothreitol (DTT) at 37C for 90 min. T4 5'-D was measured by the release of 125I- with t

Free-thyroxine index in psychotic and neurotic depression.

The mean free-thyroxine index value of a group of 25 psychotic depressive patients was found to be significantly lower than that of an age- and sex-matched group of 20 neurotic depressives (P less than 0.01). These findings support the view that psychotic and neurotic depression are differentiated not only with respect to their clinical expression, but also on account of their biological substrate. Comparison of free-thyroxine index values of each group of patients with a group of 240 euthyroid subjects hospitalized for various somatic illnesses revealed a lower mean free-thyroxine index value

[Once-weekly intravenous L-thyroxine replacement in hypothyroidism].

A 58-year-old female patient with gastrointestinal complications also had primary hypothyroidism which proved therapy-resistant against high-dosage oral replacement with L-thyroxine or an L-thyroxine/L-triiodothyronine combination. Administration of four intravenous injections each of 2 mg L-thyroxine given at weekly intervals brought about a clinical and chemical euthyroid state. Thereafter it was possible to adopt an oral L-thyroxine replacement procedure without complications. Intravenous replacement of L-thyroxine and later transfer to oral medication appears a superior and safer method th

Low serum thyroxine in phenothiazine-treated psychiatric patients.

Five patients treated with phenothiazines for serious psychiatric disease had markedly depressed serum thyroxine concentrations. There were no clinical or other laboratory findings to suggest hypothyroidism. Additional studies revealed normal levels of thyroxine-binding globulin as assessed by triiodothyronine resin uptake, thyroid-binding globulin capacity, and free thyroxine concentration. In contrast to the low serum thyroxine, serum triiodothyronine concentration was nornal.

Metabolism of L-thyroxine by phagocytosing human leukocytes.

Intact normal human leukocytes deiodinated L-thyroxine (T(4)) with the generation of inorganic iodide, chromatographically immobile origin material, and small quantities of L-triiodothyronine (T(3)). When phagocytosis was induced in the leukocytes through the addition of zymosan particles that had been opsonized by coating with plasma, T(4)-deiodination was greatly stimulated. In addition to the stimulation of T(4)-deiodination, the accumulation by the leukocytes of undegraded T(4) was increased. Anoxia, which has previously been shown not to interfere with phagocytosis, did not prevent the in
