Ischemic cerebrovascular symptoms occuring in patients with essential thrombocythaemia are usually attributed to platelet or platelet-fibrin emboli. A patient is described in whom transient ischemic attacks (TIA) had some features - namely the presence of headache and the progressive onset of symptoms - unusual for an embolic phenomenon but suggestive of a migrainous event. No further attack occured when the patient was treated by an antiplatelet drug ticlopidine, though platelet count was unchanged. The relationship between platelets, TIA and migraine are discussed.

Aspirin is the cornerstone of therapy for unstable angina and acute myocardial infarction and the foundation on which other therapies are added, both in the short term and the long term. Yet, despite clear data, aspirin is woefully underused or is often used late. Prompt administration of aspirin could save thousands of lives each year. Ticlopidine and clopidogrel have a synergistic effect when used with aspirin, and can also have a role in treating patients who are aspirin-resistant or have diffuse atherosclerosis.

The inhibitory effects of ticlopidine and acetylsalicylic acid (ASA) on thrombus formation in rat mesenteric microvessels were studied. The results were compared with the effect of the drugs on platelet aggregation in citrated whole blood. We have experienced two cases of drug-induced lupus erythematosus caused by ticlopidine in the last three years. Both were late-onset cases (1 and 4 years) that occurred in elderly men (76 and 81 years old). The common features were fever, arthralgia, myalgia, serositis, and the presence of anti-histone autoantibodies. Because ticlopidine is widely used in elderly people with ischemic vascular disease, ticlopidine-induced lupus should be considered when patients taking ticlopidine present lupus-like symptoms.
