Microenvironments of tuberculous granuloma: advances and opportunities for therapy.

The hallmark tissue lesions of tuberculosis (TB) are granulomas. These multicellular structures exhibit varying degrees of cellular complexity, are dynamic, and show considerable diversity within and between hosts. Categorization based on gross pathologic features, particularly caseation and necrosis, was historically coined prior to the identification of mycobacteria as the causative agent of TB. More recently, granuloma zonation based on immune cell composition, metabolite abundance, and physical characteristics has gained attention. With the advent of single-cell analyses, distinct microenv

Radiation-Induced Macrovessel/Microvessel Disease.

Radiation therapy (RT) is a cornerstone in cancer treatment (used in 50% of cases), yet challenges persist because damage to normal tissue through direct impact of radiation or bystander effects is inevitable. Injury of macrovessels by RT manifests as obstructive disease, which is akin to atherosclerotic disease. Historically observed in coronary arteries of patients treated for breast cancer and lymphoma, it also affects patients receiving contemporary therapy for lung and chest cancers. Moreover, radiation at various sites can lead to peripheral vascular disease. An aspect of radiation-induc

Massive soft tissue infections: necrotizing fasciitis and purpura fulminans.

Necrotizing fasciitis and purpura fulminans are two destructive infections that involve both skin and soft tissue. Necrotizing fasciitis is characterized by widespread necrosis of subcutaneous tissue and the fascia. Historically, group A beta-hemolytic streptococcus has been identified as a major cause of this infection. However, this monomicrobial infection is usually associated with some underlying cause, such as diabetes mellitus. During the last two decades, scientists have found that the pathogenesis of necrotizing fasciitis is polymicrobial. The diagnosis of necrotizing fasciitis must be

New Insights Into the Role of Neutrophils in Lymphoma.

Under physiological conditions, neutrophils exhibit remarkable tissue surveillance capabilities, infiltrating interstitial spaces within secondary lymphoid organs such as lymph nodes and spleen, and even actively patrolling nodal tissues. These observations provide groundbreaking insight into the functional heterogeneity of neutrophils while offering new perspectives on the tumor microenvironment (TME) in lymphomas. A critical question arises: what roles might these specialized, lymphoid tissue-resident neutrophils play in lymphomagenesis through their unique phenotypic and functional adaptati

Stem-cell-based therapies for retinal degenerative diseases: Current challenges in the establishment of new treatment strategies.

Various advances have been made in the treatment of retinal diseases, including new treatment strategies and innovations in surgical devices. However, the treatment of degenerative retinal diseases, such as retinitis pigmentosa (RP) and age-related macular degeneration (AMD), continues to pose a significant challenge. In this review, we focus on the use of embryonic stem cells (ESCs) and induced pluripotent stem cells (iPSCs) to treat retinal diseases by harnessing the ability of stem cells to differentiate into different body tissues. The retina is a tissue specialized for light sensing, and

Malignant syphilis with ocular involvement in an HIV-infected patient.

Malignant syphilis is now considered a rare disease, more commonly affecting individuals with poor health, malnutrition or HIV infection. We present a 34-year-old man with HIV infection who developed multiple atypical cutaneous ulcerations, leonine facies, a scleral nodule and keratitis with visual loss. The diagnosis of malignant syphilis was delayed due to the insidious presentation, but was confirmed via immunohistochemical (IHC) staining with anti-Treponema antibodies of a skin biopsy. Significant clinical improvement was observed following a 15-day course of penicillin and tigecycline the

Consequences of continuous mebendazole therapy in alveolar hydatid disease--with a summary of a ten-year clinical trial.

The assessment of a ten-year clinical trial of continuous therapy in eight patients revealed further evidence of a significant therapeutic effect of mebendazole on alveolar hydatid disease. Life-expectancy was increased when compared to untreated historical controls, especially in the patients over 55 years of age. All symptomatic patients showed subjective improvement. In four patients, three had a 50% or greater reduction in the diameter of massive hepatic lesions, and in the fourth, progressively enlarging metastases were arrested. Fall in the IHA titre suggested that the causative organism

Serial circulating tumour DNA monitoring in first-line advanced NSCLC (COPE): a randomized, two-arm non-comparative phase II trial.

PURPOSE: COPE evaluated serial circulating tumor DNA (ctDNA) profiling with molecular tumor board (MTB) review during first-line therapy for advanced non-small cell lung cancer (NSCLC). EXPERIMENTAL DESIGN: COPE was a multicenter, randomized (2:1), noncomparative phase II trial. Arm A underwent serial plasma profiling with every report reviewed at a weekly virtual MTB; Arm B received standard management. The primary endpoint was 18-month overall survival (OS) in Arm A versus a prespecified 40% historical benchmark; no between-arm comparison was planned. RESULTS: Among 176 patients, 117 were

Plasma and intracellular pharmacokinetics of tenofovir disoproxil fumarate and emtricitabine in transgender women receiving feminizing hormone therapy.

BACKGROUND: Transwomen have an increased risk of HIV acquisition compared with other adults. Drug-drug interactions between pre-exposure prophylaxis (PrEP) and gender-affirming therapy are cited as a reason for poor PrEP uptake among transwomen. We evaluated plasma tenofovir and emtricitabine pharmacokinetics and their active intracellular anabolites, tenofovir-diphosphate and emtricitabine-triphosphate, in transwomen receiving feminizing hormones. METHODS: We enrolled HIV-negative transwomen (≥19 years) not receiving PrEP. Participants took oral tenofovir disoproxil fumarate/emtricitabine 30

Destructive Erysipelothrix rhusiopathiae Endocarditis Confirmed on Valve Tissue Culture After Persistent Blood Culture Negativity.

Erysipelothrix rhusiopathiae is a zoonotic, gram-positive bacillus that is a virulent cause of infective endocarditis (IE) and incurs a high mortality rate. Its virulence is compounded by intrinsic resistance to common empirical agents including vancomycin and aminoglycosides. Given it is historically associated with a high rate of bacteraemia, its presentation in this case report as blood culture-negative but tissue culture positive infective endocarditis is highly uncommon. We report the case of an immunocompetent female in her 60s with relevant environmental exposure presenting with acute h

Quantitative Evaluation of Dystrophin Expression Using SDS-PAGE Western Blot Methods.

SDS-PAGE Western blots allow for the identification of the presence and relative quantity of proteins of interest on the basis of their molecular weight and their recognition by specific antibodies. This technique has historically been useful in the diagnosis and study of Duchenne muscular dystrophy (DMD) and Becker muscular dystrophy (BMD), as it allows the recognition of clinically relevant abnormalities in the amount or size of dystrophin. While this technique is used infrequently for diagnosis at this point (as genetic testing for pathogenic variants of the DMD gene has become standard pra

Renaissance of laser interstitial thermal ablation.

Laser interstitial thermal therapy (LITT) is a minimally invasive technique for treating intracranial tumors, originally introduced in 1983. Its use in neurosurgical procedures was historically limited by early technical difficulties related to the monitoring and control of the extent of thermal damage. The development of magnetic resonance thermography and its application to LITT have allowed for real-time thermal imaging and feedback control during laser energy delivery, allowing for precise and accurate provision of tissue hyperthermia. Improvements in laser probe design, surgical stereotac

Ventricular structure and function in children with sickle cell disease using conventional and tissue Doppler echocardiography.

Conventional 2-dimensional, M-mode, and spectral Doppler echocardiographic techniques have documented abnormal ventricular function in adults with sickle cell disease (SCD), but assessments in children are conflicting. Tissue Doppler echocardiography (TDE) provides additional information about myocardial function. Two-dimensional, M-mode, tricuspid regurgitation jet velocity (TRJV) data, and tissue Doppler echocardiographically derived myocardial velocity measurements of left ventricular (LV) and right ventricular function were taken from children with SCD compared to those of similar healthy

Proton Radiation for Locally Advanced Breast Cancer: Evolution and Techniques.

Proton therapy has emerged as an important technique for locally advanced breast cancer (LABC), driven primarily by the need to reduce cardiopulmonary dose during comprehensive nodal irradiation, particularly when the internal mammary chain is targeted. Since the first clinical proton experience in patients with LABC and unfavorable cardiac anatomy in 2010, proton radiation therapy for LABC has evolved considerably: delivery technique has progressed from passive scattering and uniform scanning to pencil beam scanning and intensity modulated proton therapy, whereas breast radiation oncology ove

A historical perspective and evolution of the treatment of male urinary incontinence.

AIMS: To describe the historical changes from antiquity until present in the presentation and treatment of male urinary incontinence. METHODS: A literature review of PubMed articles in English pertaining to male incontinence was performed. RESULTS: Male urinary incontinence was first mentioned in Egyptian manuscripts in 1500 B.C. In 1564, Ambroise Pare designed portable urinals for incontinent males. Wilhem Hildanus created the first condom catheter with pig bladder in the 1600s and was also credited with fashioning the first penile clamp. Lorenz Heister introduced a perineal bulbar urethral

Tumor Treating Fields (TTFields) Therapy Concomitant with Taxanes for Cancer Treatment.

Non-small cell lung cancer, ovarian cancer, and pancreatic cancer all present with high morbidity and mortality. Systemic chemotherapies have historically been the cornerstone of standard of care (SOC) regimens for many cancers, but are associated with systemic toxicity. Multimodal treatment combinations can help improve patient outcomes; however, implementation is limited by additive toxicities and potential drug-drug interactions. As such, there is a high unmet need to develop additional therapies to enhance the efficacy of SOC treatments without increasing toxicity. Tumor Treating Fields (T

Diagnostic and Therapeutic Challenges in Multifocal MRSA Pyomyositis: A Case Report.

Background: Pyomyositis is a rare bacterial infection of skeletal muscle, historically associated with tropical climates but increasingly observed in temperate areas, particularly among immunocompromised individuals. Its nonspecific early symptoms often overlap with other soft-tissue infections, causing diagnostic delay. Case Presentation: We report the case of a 64-year-old lumberjack with poorly controlled diabetes and a remote history of lymphoma, who developed multifocal MRSA pyomyositis with septic pulmonary emboli following orthopedic trauma and surgery. The condition rapidly evolved int

Imaging Capabilities of Patients with Early-stage Collagenoses.

The general term collagenosis covers a group of connective tissue diseases triggered by systemic autoimmune processes and as yet unknown trigger mechanisms. Systemic sclerosis is one of the five types of collagenosis. Of the systemic rheumatic diseases, it has the highest mortality rate, partly due to the historical lack of disease-modifying therapies. Early diagnosis of systemic sclerosis can be challenging if the typical symptoms and diagnostic or classification criteria are not yet evident. However, given the disease's often heterogeneous course, early diagnosis and treatment are crucial fo

Traditional and New Routes of Trophoblast Invasion and Their Implications for Pregnancy Diseases.

Historically, invasion of placental trophoblasts was thought to be extremely specific, only invading into the connective tissues of the maternal uterus and finally reaching and transforming the uterine spiral arteries. Only recently, identification of new routes of trophoblast invasion into different structures of the maternal uterus has been achieved. Thorough morphological analysis has resulted in the identification of trophoblasts invading into glands, veins, and lymph vessels of the uterine wall. These new routes pave the way for a re-evaluation of trophoblast invasion during normal placen
