Tooth extractions in Orthodontics: first or second premolars?

Tooth crowding and protrusions demand rigorous attention during orthodontic planning that includes the extraction of first and second premolars. Some characteristics, such as dentoalveolar bone discrepancies, maxillomandibular relations, facial profile, skeletal maturation, dental asymmetries and patient cooperation, are important elements of an orthodontic diagnosis. This study discusses the options of treatments with extractions and describes the correction of a Class I malocclusion, bimaxillary protrusion, severe anterior crowding in both dental arches and tooth-size discrepancy, using firs

Clinical insights into tooth extraction via torsion method: a biomechanical analysis of the tooth-periodontal ligament complex.

Traditionally, extracting single, flat- or curved-rooted teeth through twisting is unfeasible. However, our clinical practice suggests that such teeth can be extracted efficiently through moderate twisting in a minimally invasive manner. Given the lack of studies on biomechanics of the tooth-periodontal ligament (PDL) complex during torsion, which has further constrained its application, we assessed the feasibility of the torsion method for extracting single-rooted teeth and evaluated its minimally invasive potential. Using three-dimensional finite element analysis, we examined the stress dist

Management strategies for unanticipated remnants following tooth extraction: A case report.

RATIONALE: Tooth extraction is a common oral surgical procedure, yet rare complications such as retained tooth fragments may occur. These remnants - including crown fragments, root tips, or other materials - can delay healing, provoke infection, and impair oral function, posing challenges for clinicians. Because fragments may migrate extra-socket and be masked by fibrotic tissue, routine examination can be falsely negative and diagnosis delayed. This case report clarifies when and why early CBCT is warranted to localize retained crown fragments and demonstrates a micro-incision retrieval strat

Digital robot-assisted minimally invasive impacted tooth extraction: A case report.

OBJECTIVE: This study investigated the clinical effects and applicability of minimally invasive impacted teeth extraction using digital robots. METHODS: A marker was bonded to the non-surgical area before surgery. A Cone-Beam Computed Tomography (CBCT) scan was obtained and uploaded to the robot software to determine the drilling position of the ring drill. During the surgery, the robot arm automatically navigated to a predetermined position, and the ring drill removed part of the bone tissue and exposed and extracted the impacted teeth. Finally, the surgeon tightly sutured the wounds to the

Effect of maxillary anterior supernumerary tooth extraction on the underlying malocclusion.

INTRODUCTION: Orthodontic discrepancies are a common finding in patients with supernumerary teeth (ST). The presence of a ST can cause a number of orthodontic discrepancies, including delayed eruption or retention of adjacent teeth, crowding, spacing, and abnormal root formation. The aim of the present study was to assess the effect of extraction of an anterior supernumerary tooth on the underlying orthodontic discrepancies without additional treatment for a 6-month period. METHODS: This was a prospective, longitudinal, observational, study. It included 40 participants with orthodontic malocc

Observational study investigating tooth extraction and the shortened dental arch approach.

The shortened dental arch (SDA) provides a cost-effective dentition, considering the population is ageing and retaining teeth for longer. The aims were to observe the reasons and sites of tooth extraction and assess the functional dentition over 15 years in dental practice. Subjects were recruited who required permanent tooth extractions between 2000 and 2015. The reasons for extractions were chosen from twelve extraction codes. Data were also collected for demographics, tooth position, root treated teeth and functional pairs remaining. Patient-centred factors on reasons for tooth extraction a

Timeframe of socket cortication after tooth extraction: A retrospective radiographic study.

OBJECTIVES: To assess the timeframe between tooth extraction and radiographically detectable socket cortication in humans. METHODS: Two hundred and fifty patients with a CT scan ≤36 months after tooth extraction were included. First, three orthoradial multiplanar reconstruction slices, representing the major part of the extraction socket, were scored regarding the degree of bone healing as (i) healed, that is, complete/continuous cortication of the socket entrance, or (ii) non-healed. Thereafter, based on the results of all three slices, the stage of cortication of the extraction socket, as o

Applied feline oral anatomy and tooth extraction techniques: an illustrated guide.

PRACTICAL RELEVANCE: Tooth extraction is one of the most commonly performed surgical procedures in small animal practice. CLINICAL CHALLENGES: The clinician must be familiar with normal oral anatomy, utilize nomenclature accepted in dentistry and oral surgery, use the modified Triadan system for numbering teeth, identify normal structures on a dental radiograph, understand the tissues that hold the teeth in the jaws, know the biomechanical principles of tooth extraction, be able to choose the most appropriate instrument for removal of a tooth, extract teeth using closed and open techniques, a

Posttraumatic use of dental implants immediately after tooth extraction: clinical study.

PURPOSE: Root fracture is a combined injury of cementum, dentin, and pulp. Many of these traumas remain untreated, mistreated, or overtreated. It leads to a more complicated treatment in case of tooth loss. Many different treatment procedures, with a very changeable success rate, have been proposed for years to treat teeth with root fractures. The objective of the following clinical studies was to evaluate the clinical effectiveness of implants placed in fresh extraction sites to treat teeth with horizontal root fracture. METHODS: The study group included 25 patients (15 men and 10 women) bet

Effects of alendronate on bone healing after tooth extraction in rats.

OBJECTIVES: Tooth extraction has been identified as an important risk factor for bisphosphonate-induced osteonecrosis of the jaw. Therefore, the main goal of this study was to determine the effects of alendronate on healing of the extraction socket and on interdental alveolar bone after tooth extraction in rats. MATERIALS AND METHODS: Animals were injected subcutaneously with vehicle or alendronate for 3-4 weeks before the first mandibular molar was extracted and these treatments were continued during post-extraction periods of 10, 21, 35 and 70 days. Mandibles were processed to evaluate heal

Staphylococcal scalded skin syndrome and toxic shock syndrome after tooth extraction.

Staphylococcus aureus may cause cutaneous and systemic infections such as staphylococcal scalded skin syndrome (SSSS) and toxic shock syndrome (TSS). Although exfoliative toxins A and B, which cause SSSS, and TSS toxin-1 may be produced by different strains of S aureus, the two syndromes rarely occur simultaneously. We describe a patient admitted to the intensive care department with an exfoliative generalized erythroderma, signs of shock, and biopsy specimen findings consistent with SSSS. This patient presented after a tooth extraction. Symptoms recurred after dismissal, and he was readmitted

Predicting Pain After Tooth Extraction With Easy-to-Collect Preoperative Variables.

OBJECTIVES: To examine the determinants of acute pain following tooth extraction, within a set of factors belonging to different domains: biometry, dental nosology and anatomy, psychology toward pain and surgery, and surgical procedure. METHODS: Participants were recruited in two centres in France and Lebanon. A 'simple extraction' (161 patients) and a 'wisdom teeth/tooth' group (at least mandibular) (115 patients) were studied separately. Postoperative pain and total analgesic drug intake (over 5 days) were both expressed as a composite score. Both scores were ranked within each surgery grou

Discoidin Domain Receptor 2 Is Required for Tooth Extraction Socket Healing.

Discoidin Domain Receptor 2 (DDR2) is a collagen-activated tyrosine kinase required for bone and tooth development as well as regeneration of calvarial and long bones. Although DDR2 is expressed in alveolar bone, its functions in this site have not been previously examined. The present study used global and conditional knockout approaches and lineage tracing to examine the functions of DDR2 in alveolar bone formation during tooth socket healing. First molars were extracted from the maxillae of wild-type and globally Ddr2-deficient (Ddr2LacZ/LacZ) mice, and socket healing was measured after 1,
