INTRODUCTION: Spontaneously catastrophic fracture of intact unrestored molar teeth is not common. Nevertheless, cracks do occur that progress apically, resulting in the complete splitting of the tooth and root. This report describes a catastrophic fracture that occurred in an unrestored mandibular second molar resulting in a previously unreported combination of a longitudinal and horizontal root fracture, appearing radiographically as a single horizontal root fracture.
METHODS: Tooth fragments were examined clinically, stereoscopically, and by scanning electron microscopy. Fractographic analy

Fractures of multiple permanent anterior teeth can be a traumatic experience for children, with functional, esthetic and psychological aspects. The treatment of complicated crown-root fractures (CRFs) is more challenging when the biologic width is violated. This paper presents a case of 12-year boy with complicated CRF of teeth #12 and #21, and horizontal crown fracture of tooth #11. It was managed by endodontic treatment, mucoperiosteal surgery with osteotomy to visualize the fracture line for fragment reattachment, followed by fiber-post placement and restoration with polycarbonate crowns. Teeth with traumatic crown-root fractures extending subgingivally and involving the entire width of the tooth may have problems in endodontic isolation, periodontal maintenance, and restoration. A number of techniques, such as crown lengthening, and orthodontic or surgical extrusion, have been described for treatment of crown-root fractures. However, all of these techniques have limitations in terms of the extended time required for treatment, cost, and esthetic considerations. Intra-alveolar transplantation is an alternative treatment option in the management of complicated crown-root fractur

INTRODUCTION: This study aimed to evaluate the impact of mechanical loading, induced by preparing anatomically standardized premolar and molar canal models, on the cyclic fatigue and torsional fracture resistance of heat-treated nickel-titanium (NiTi) rotary instruments.
METHODS: Sixty NiTi instruments (Race Evo) were randomly assigned to 3 groups (n = 20): unused controls (N0), instruments used to prepare 2 canals in premolar models (P2), and 4 canals in molar models (M4). Cyclic fatigue testing was performed in dynamic mode with a 4 mm axial pecking motion at 37 ± 1 °C. Time to fracture and

BACKGROUND/AIM: Anterior teeth are prone to traumatic dental injuries (TDIs). Although a number of techniques ranging from original tooth fragment reattachment (TFR) to direct composite restoration (DCR) can be used to restore uncomplicated crown fractures, there is no consensus on which method is best. The purpose of this study was to investigate the fracture resistance of bovine incisors restored by two different techniques (TFR and DCR) in three different fracture models.
MATERIALS AND METHODS: Sixty extracted bovine lower incisors were randomly divided into three groups (n = 20). Angle, o

PURPOSE: The purpose of this study was to evaluate the fracture resistance of endodontically treated teeth between those with four walls and those with three walls of remaining coronal tooth structure and the effect of the site of the missing coronal wall.
MATERIALS AND METHODS: Thirty-two endodontically treated second mandibular premolars were decoronated, leaving 3 mm above the cementoenamel junction (CEJ). A 0.5-mm-wide chamfer was prepared 1 mm above the CEJ. The teeth were randomly divided into four groups. Group 1 had four walls of coronal tooth structure, whereas groups 2, 3, and 4 had

OBJECTIVE: Thoracolumbar fractures are one of the most common fractures in clinical practice. Surgical intervention is recommended to restore spinal alignment or decompress the nerves when there are unstable fractures or neurological injuries. However, after excessive forward thrust force restoration, facet joint dislocation often occurs between the upper vertebra and the fractured vertebra, which usually leads to unsatisfactory reduction outcomes. Herein, we propose a novel spinal facet joint toothed plate to assist in fracture reduction. The purpose of this study is to evaluate the effective

OBJECTIVE: Restoration of traumatized incisors by reattachment of the original tooth fragment appears to be the most conservative treatment approach. But the measurement of forces acting on natural tooth in-vivo poses many challenges. The advent of finite element analysis (FEA) has made it possible to demonstrate the propagation of stress through each part of a tooth and its restoration. The objective of this study was to evaluate and compare the fracture resistance of reattached human tooth fragment with different types of retentive preparation techniques using finite element analysis.

The objective of this study was to determine the influence of height and thickness of the one wall remaining coronal tooth structure on the fracture resistance of an endodontically treated root with resin abutment build-up using resin composite and fiber-reinforced resin composite post. Static and dynamic fracture tests were performed by placing the remaining tooth wall on the tensile side and applying loads at an angle of 30° from the tooth axis. Superior static fracture resistance was observed when the wall remaining on the tooth had a height and thickness greater than 1.0 mm. The dynamic fa

AIM: There is an ongoing controversy about indications for prophylactic removal of third molars. The purpose of this retrospective study was to clarify the risk of preservation of lower third molars.
MATERIAL AND METHOD: In a 5-year period, 316 patients were registered who had received in-patient treatment for deep abscess formation, cyst formation or mandibular angle fracture in relation to lower third molars. A radiological analysis (panoramic radiographs) was performed to determine whether major pathological changes associated with lower wisdom teeth are related to their position. Third mo

Zirconia is widely used in full-arch restorations due to its strength and aesthetics, but failures can still affect its performance in clinical practice. In this report, a full-arch tooth-supported zirconia bridge fractured prematurely (eleven months), encouraging an investigation into its design and failure mechanisms. STL files obtained from the dental laboratory revealed regions of reduced framework thickness, falling below the manufacturer's recommendations. Fractographic analysis of the fractured pieces indicated a multifactorial failure pattern. Notable features included a thick cement l

This case report describes the successful treatment and 4-year follow-up of a maxillary central incisor with a complicated crown-root fracture that had sustained two separate traumatic injuries during the transition from adolescence to adulthood. Following the initial trauma in adolescence, the tooth was treated with root canal therapy and reattachment of the fractured crown fragment. A second trauma in adulthood led to a complicated crown-root fracture whose margin extended subgingivally. The tooth was managed using intentional replantation with 180° rotation and subsequent restoration with a

Treatment of crown-root fracture can be complicated, time-consuming and expensive. This paper presents a treatment approach that minimises intra-operatory intervention and preserves the biologic tissue. A 15-year-old boy reported for treatment of an oblique crown fracture in the left permanent maxillary central incisor (#21, Fédération Dentaire Internationale) with mature root apices. The treatment presented an endodontic, periodontal and restorative challenge as the crown fracture exposed the pulp and extended subgingivally. The patient also had accompanying generalised enamel hypoplasia due

Although it is challenging, the early diagnosis of a vertical root fracture (VRF) is crucial in order to ensure tooth preservation. The purpose of this clinical case report was to describe reparative surgery performed to treat a tooth affected by an incomplete VRF. A 26 year old male patient was suspected to have a VRF in a maxillary left central incisor, and an exploratory flap was performed in order to confirm the diagnosis. After detecting the fracture, the lesion was surgically treated, the fracture and the infected root-end were removed, and a platelet-rich plasma membrane was used to cov

BACKGROUND: Fracture of endodontic instruments during primary tooth treatment is rare, and limited evidence exists regarding the lon g-term impact of retained fragments on the development of successor teeth and surrounding alveolar bone.
CASE PRESENTATION: This report describes a 33-year-old woman in whom a radiopaque foreign body was incidentally discovered in the periapical region of the right maxillary first premolar (#14) during a routine dental examination. Cone-beam computed tomography (CBCT) revealed a well-defined, high-density object measuring 3.61 mm × 0.66 mm, located approximately

STATEMENT OF PROBLEM: Tooth-to-implant-supported fixed partial dentures (TI-FPDs) are a valuable treatment option for the shortened dental arch. For naturally healthy abutment teeth or abutment teeth with small restorations, inlays or adhesive wings present - as opposed to complete crowns - minimally invasive FPD retainer elements. Data on TI-FPDs with minimally invasive retainer designs are sparse.
PURPOSE: The purpose of this in vitro study was to compare test forces correlating with fracture (ultimate load, Fu) or formation of initial damage (force at first damage, F1d) and the failure mod

This case report describes the esthetic and functional recovery of a maxillary central incisor. The treatments used were dentin post (biological post) reinforcement and reattachment of the fragment. From the outcomes achieved, it can be concluded that this technique is promising and is yet another alternative method that can be used for the recovery of fractured anterior teeth.
