Torsades de Pointes (TdP) is a rare form of tachyarrhythmia which can potentially be fatal due to its tendency to degenerate into ventricular fibrillation. It is described as a polymorphic ventricular tachycardia characterized by twisting of the QRS complexes around the electrocardiogram (ECG) baseline in patients with a prolonged QT interval. "Torsades de pointes", a life-threatening rhythm disorder, is a polymorphic ventricular tachycardia that usually develops in association with a prolonged QT interval. Fluconazole, an anti-fungal drug, may also induce QT prolongation, in some cases subsequent torsades de pointes. Herein, we report a 16-year-old female presenting "torsades de pointes" after administration of fluconazole and rapidly improved upon cessation of the drug.

We report the case of a patient in whom dramatic T wave alternans with brief bouts of torsades de pointes and QT prolongation occurred in association with intravenously administered pentamidine. This ECG marker of a potentially fatal side effect, although uncommon, should be looked for in patients requiring the use of this drug.

Methadone is known to prolong the QT interval and precipitate torsades de pointes. A 54-year-old prescription methadone user with hypokalaemia was referred to Critical Care with acute confusion and respiratory distress. Alcohol withdrawal was the presumed precipitant. The real precipitant only became evident on analysis of a 24-h ECG (Holter monitor) attached to the patient at the time. Children with single chamber pacemakers, in adolescence and young adulthood, may be upgraded to dual chamber systems, but there are no published data about indications, timing, and complications. Brucella endocarditis is a zoonosis transmitted by contaminated dairy products. Endocarditis is a rare complication of brucellosis and mainly the aortic valve is infected. This report describes a patient with a functioning permanent pacemaker who developed torsades de pointes. Although overdrive cardiac pacing has been used to suppress this arrhythmia, cardiac pacing was not protective in this patient. Torsade de pointes is a frequent cause of sudden death of cardiac origin, with uncertain mechanisms of actions and very diverse origins. A case is presented of a patient with a bronchial condition on pharmacological treatment with macrolides, and who, as well as having atrial fibrillation, suffered an episode of self-limiting torsade de pointes that abated spontaneously with no associated clinical complications.

A 47-year-old woman with Kearns-Sayre syndrome (KSS) and an implanted pacemaker for complete heart block was admitted to the intensive care unit following a cardiac arrest due to ventricular tachycardia (torsade de pointes) in the setting of QT prolongation. Complete heart blocks and ventricular tachycardia are implicated as mechanisms of sudden deaths in KSS; such patients may require pacemaker implantation and implantation of an automatic implantable cardioverter-defibrillator.
