The spine journal : official journal of the North American Spine Society2016eng
BACKGROUND CONTEXT: Total disc arthroplasty is an alternative therapy to spinal fusion for the treatment of neck or low back pain and is hypothesized to reduce the risk of disease progression to the adjacent spinal levels. Radiographic and retrieval analyses of various total disc replacements (TDRs) have shown evidence of impingement damage. Impingement of TDRs can occur when the device reaches the limits of its functional range of motion, causing contact between peripheral regions of the device.
PURPOSE: Impingement can be associated with increased wear and mechanical damage; however, imping

BACKGROUND: Lumbar pedicle screw fixation and fusion are major procedures for treating discogenic low back pain (DLBP). However, due to its advantages of preserving the segmental motion and biomechanical simulation, artificial total lumbar disc replacement (TDR) is increasingly popular.
METHODS: From 2007 to 2010, 68 DLBP patients were enrolled. TDR were performed on 34 patients and the other 34 controls underwent the traditional fixation procedure. Qualitative and quantitative evaluations were followed including the changes in range of motion (ROM) and interpedicle height (IPH) at the poster

The study design includes a prospective, randomised controlled study comparing total disc replacement (TDR) with posterior fusion. The main objective of this study is to compare TDR with lumbar spinal fusion, in terms of clinical outcome, in patients referred to a spine clinic for surgical evaluation. Fusion is effective for treating chronic low back pain (LBP), but has drawbacks, such as stiffness and possibly adjacent level degradation. Motion-preserving options have emerged, of which TDR is frequently used because of these drawbacks. How the results of TDR compare to fusion, however, is unc

STUDY DESIGN: This is a level III retrospective study.
OBJECTIVE: The authors aim to review the outcomes and complications of ball and socket total disk replacements (TDRs).
SUMMARY OF BACKGROUND DATA: TDR is a motion-preserving technique that closely reproduces physiologic kinematics of the cervical spine. However, heterotopic ossification and spontaneous fusion after implantation of the total cervical disk have been reported in several studies to decrease the range of motion postulated by in vitro and in vivo biomechanical studies.
METHODS: The medical records of 117 consecutive patients

PURPOSE: Although cervical total disc replacement (CTDR) is perceived as a safe procedure, no review to date has quantified the complication rates. Of note, heterotopic ossification (HO), one of the complications of CTDR, is hypothesised to cause adjacent segment degeneration (ASDegeneration). This association has not been proven in meta-analysis. Hence, this systematic review and meta-analysis aims to investigate the pooled prevalence of complications following CTDR among studies that concomitantly reported the rate of HO, and the associations between HO and other complications, including ASD

Since the launch of cervical total disc replacement (CTDR) in the early 2000s, many clinical studies have reported better outcomes of CTDR compared to those of anterior cervical discectomy and fusion. However, CTDR is still a new and innovative procedure with limited indications for clinical application in spinal surgery, particularly, for young patients presenting with soft disc herniation with radiculopathy and/or myelopathy. In addition, some controversial issues related to the assessment of clinical outcomes of CTDR remain unresolved. These issues, including surgical outcomes, adjacent seg

STUDY DESIGN: In vitro biomechanical study.
OBJECTIVE: To quantify the effects of uncinatectomy on cervical motion after total disc replacement (TDR).
SUMMARY OF BACKGROUND DATA: The effect of uncinatectomy on TDR motion is unknown. Partial uncinatectomy may be required to decompress the foramen; however, the residual uncinates can potentially limit TDR motion and serve as a source of progressive spondylosis. Complete resection of the uncinates may decrease this risk yet endanger destabilizing the segment.
METHODS: Seven human cervical spines (C3-C7) (age, 63.4 +/- 6.9 years) were tested fi

STUDY DESIGN: Data for this study were collected as part of a prospective Food and Drug Administration regulated IDE trial for the ProDisc total disc replacement.
OBJECTIVE: The purpose of the study was to compare the results of total disc replacement (TDR) in patients with previous discectomy to patients who had no such previous surgery.
SUMMARY OF BACKGROUND DATA: Discectomy is a well accepted surgical procedure for appropriately selected patients. However, removal of a portion of the disc may lead to further disc degeneration and collapse of the disc space. In the past, treatment options

INTRODUCTION: The role of fusion of lumbar motion segments for the treatment of axial low back pain (LBP) from lumbar degenerative disc disease (DDD) without any true deformities or instabilities remains controversially debated. In an attempt to avoid previously published and fusion-related negative side effects, motion preserving technologies such as total lumbar disc replacement (TDR) have been introduced. The adequate extent of preoperative DDD for TDR remains unknown, the number of previously published studies is scarce and the limited data available reveal contradictory results. The goal

BACKGROUND CONTEXT: Anterior cervical discectomy and fusion (ACDF) is the leading surgical treatment for cervical radiculopathy. However, ACDF surgery has been suggested for to accelerate the degeneration of the adjacent cervical discs, which causes so-called adjacent segment disease (ASD). Over the past 2 decades, total disc replacement (TDR)/cervical disc arthroplasty (CDA) has become an increasingly common method for treating degenerative cervical diseases. The rationale is that a synthetic disc prosthesis may preserve motion at the operated level, which is expected to lead to reduced stres

STUDY DESIGN: Prospective randomized clinical trial.
OBJECTIVE: Determine the reasons for, and rates of, secondary surgical intervention up to 5 years at both the index and adjacent levels in patients treated with cervical total disc replacement (TDR) or anterior cervical discectomy and fusion (ACDF). Patients undergoing TDR received ProDisc-C.
SUMMARY OF BACKGROUND DATA: Several outcome-based prospective, randomized clinical trials have shown cervical TDR to be equivalent, if not superior, to fusion. The ability of TDR to allow decompression while maintaining motion has led many to suggest

Cervical disc arthroplasty (CDA) is a well-established motion-preserving treatment for cervical degenerative disc disease. The Bryan® Cervical Disc is among the most widely used implants with supportive long-term outcome data; however, late implant migration is a rare occurrence and can result in significant neurologic compromise. A female in her mid-50s with a history of C5-6 Bryan Cervical Disc arthroplasty performed 20 years prior presented with progressive cervical myelopathy. She reported a fall from a horse two years before evaluation. Neurologic examination demonstrated bilateral upper-
