The development of evidence-based nursing.

This paper argues that the current conception of evidence-based medicine has its limitations in the promotion of research which effects the quality of service in any health care system. It also poses something of a difficulty for the development of evidence-based nursing in particular. This paper advocates the more broad based concept of evidence-based practice and discusses its potential for addressing theory/practice problems and the uptake of nursing research. The broader conceptualization of evidence-based practice focuses on the integration of available evidence and the tacit knowledge of

Developing a best practice model for care of patients with polydipsia.

Some psychiatric patients diagnosed with schizophrenia have a secondary diagnosis of polydipsia which is manifested by consuming excessive quantities of fluids, marked confusion, and disorientation. In most instances, these persons are less amenable to treatment and rehabilitative interventions due to the changes in cognitive and physical processes. A review of our own current practice found that we had a small group of polydipsia patients requiring a large amount of one-to-one staff time for little or no long-term benefit. Further, there was no uniform approach to identify, treat, and monitor

Cross-cultural validation of the Chinese version of the lean management scale for nursing services in hospitals.

BACKGROUND: Lean management has been increasingly applied in nursing services to improve workflow efficiency and reduce waste in care processes. This study aimed to translate, culturally adapt, and evaluate the psychometric properties of the Lean Management Scale for Nursing Services in Hospitals (LMS-N) among Chinese clinical nurses. METHODS: A total of 2,127 clinical nurses were recruited from four tertiary hospitals in Liaoning Province, China, using convenience sampling. Following Brislin's translation guidelines, the LMS-N was translated and culturally adapted into Chinese. Psychometric

The Conversion of Planned Colonoscopy to Sigmoidoscopy and the Effect of this Practice on the Measurement of Quality Indicators.

OBJECTIVES: A cecal intubation rate (CIR) of >90% is a well-accepted quality indicator of colonoscopy and is consequently monitored within endoscopy units. Endoscopists' desire to meet this target may mean that incomplete colonoscopies are recorded as flexible sigmoidoscopies. The aim of this study was to examine whether the conversion of requested colonoscopies is a clinically significant phenomenon and whether this impacts upon the measurement of quality indicators. METHODS: A retrospective review of all flexible sigmoidoscopies performed between 1 January 2015 and 31 December 2015 at Notti

[ReIntra--a model for rehabilitation of severely injured accident victims].

ReIntra is a medical/occupational counselling and reintegration service launched in 1997 with the aim of supporting seriously injured accident victims along their path towards, hopefully, social and professional rehabilitation. We have analysed 109 patients with severe injuries for whom previous attempts at reintegration left something to be desired. ReIntra's findings, collated after an average period in care of around 17 months, show that the patients' main problems have less to do with the system of medical care than with the reintegration process. Problems seem to occur, in particular, at

High-performance work systems in health care management, part 2: qualitative evidence from five case studies.

BACKGROUND: : A capable workforce is central to the delivery of high-quality care. Research from other industries suggests that the methodical use of evidence-based management practices (also known as high-performance work practices [HPWPs]), such as systematic personnel selection and incentive compensation, serves to attract and retain well-qualified health care staff and that HPWPs may represent an important and underutilized strategy for improving quality of care and patient safety. PURPOSE: : The aims of this study were to improve our understanding about the use of HPWPs in health care or

[Quality management in weight restitution in Anorexia nervosa--pathophysiology, evidence-based practice and prevention of the refeeding syndrome].

During refeeding syndrome-a well-known and dreaded complication of weight-restauration in anorexia nervosa-a shift of electrolytes and fluid can occur in malnourished patients and might therefore lead to-potentially fatal-cardiovascular, respiratory and neurological symptoms. Causes of this are metabolic and hormonal changes during re-establishment of a carbohydrate-rich diet. This syndrome is most commonly associated with hypophosphatemia, which can however be accompanied by other chemical laboratory abnormalities. Standardized guidelines for the prevention and management of the refeeding syn

Paying for quality improvement: compliance with tobacco cessation guidelines.

BACKGROUND: Identification and treatment of routine tobacco use in medical practice is an effective intervention but is not used consistently. A study was conducted at HealthPartners, a large network-model health plan in Minnesota, to determine the effect of an outcomes recognition strategy that involved bonus funds and the rates at which network physicians document that tobacco users are identified and advised to quit. METHODS: Audits of 14,489 ambulatory patient records from 19-20 medical groups were conducted to determine the proportion of charts from each medical group that demonstrated i

Improving the care of patients with non-ST-elevation acute coronary syndromes in the emergency department: the CRUSADE initiative.

Although acute coronary syndromes (ACS) represent a well-recognized source of morbidity and mortality for patients with cardiovascular disease, evidence-based therapies shown to improve outcomes for ACS are frequently underused in appropriate patients, especially in the emergency department (ED). Despite dissemination of expert recommendations from the American College of Cardiology/American Heart Association (ACC/AHA) and ED-focused recapitulation of them in the emergency medicine literature, significant barriers continue to limit the adoption of guidelines in clinical practice and appear to
