One-Lung Ventilation in a Pediatric Tracheostomy Patient: A Case Report.

A 20-month-old male with congenital lobar emphysema presented for right middle lobectomy. He had previously undergone right upper lobectomy and tracheostomy placement and remained ventilator-dependent. Due to recent accidental decannulation necessitating emergent tracheostomy revision, otolaryngology advised that stoma patency be maintained throughout the perioperative period. As such, orotracheal intubation techniques were not practical options for lung isolation. This case report describes a unique method of achieving one-lung ventilation in a young pediatric patient with a tracheostomy.

Early Rehabilitation Reduces Time to Decannulation in Patients With Severe Acquired Brain Injury: A Retrospective Study.

Purpose: Early decannulation is considered a main rehabilitative goal in tracheostomized patients. Our aim is to evaluate whether a very early rehabilitation protocol helps to reduce the tracheostomy duration in patients affected by an Acquired Brain Injury (ABI). Methods: Data about consecutive tracheostomized patients admitted in our Neuro-Rehabilitation Unit (NRU) were retrospectively collected. We defined two groups: Early Rehabilitation Group patients came from our ICU, where they started the rehabilitative treatment; Delayed Rehabilitation Group patients arrived from external ICUs and st

Unmasking of tracheomalacia following short-term mechanical ventilation in a patient of adult respiratory distress syndrome.

Patients with chronic obstructive pulmonary disease (COPD) are susceptible to airway malacia, which may be unmasked following mechanical ventilation or tracheostomy decannulation. Dynamic imaging of central airways, a non-invasive test as effective as bronchoscopy to diagnose airway malacia, has increased the recognition of this disorder. We describe a 70-year-old woman admitted with adult respiratory distress syndrome. She had cardiorespiratory arrest on admission, from which she was successfully resuscitated. She had obesity, hypertension, diabetes mellitus, recurrent ventricular tachycardia

Endoscopic posterior cordotomy for treatment of dyspnea due to vocal fold immobility.

INTRODUCTION: Several surgical procedures have been described for the treatment of respiratory distress secondary to vocal fold immobility (VFI), but the contribution of posterior cordotomy (PC) to tracheostomy weaning or prevention has not been studied in depth, particularly in the acute setting. The objective of this study was to show the effectiveness of PC to relieve dyspnea, prevent the need for tracheostomy, and enable decannulation in patients with VFI. METHODS: We conducted a retrospective study and reviewed the medical records of all patients whose dyspnea warranted surgical interven

Laser arytenoidectomy.

An endoscopic laser arytenoidectomy was performed on 20 patients for bilateral abductor vocal cord paralysis due to various causes. Ten patients had a previous tracheostomy prior to the laser arytenoidectomy and all were subsequently decannulated. One patient without a previous tracheostomy required an immediate tracheostomy after the completion of the laser arytenoidectomy due to a severe laryngeal edema and was also subsequently decannulated. All of these patients had a satisfactory voice and an adequate laryngeal airway. The advantages of the laser for endoscopic arytenoidectomy are facilit

Internal mandibular distraction to relieve airway obstruction in infants and young children with micrognathia.

Micrognathia may cause upper airway obstruction requiring complex medical interventions and sometimes tracheostomy. The role of distraction techniques to lengthen the mandible is yet to be clarified. The aim of this paper is to present a series of five cases in which mandibular lengthening by osteotomy and internal distraction was used to relieve airway obstruction. Five patients whose ages ranged from 4-39 months (mean, 15 months) were managed at our center with internal distraction osteogenesis to relieve airway obstruction. Three patients had a tracheostomy, and two patients had refractory

Tracheostomy in children--the Red Cross War Memorial Children's Hospital experience 1980-1985.

During the 6 years (1980-1985) at The Red Cross Children's Hospital 293 children required a tracheostomy during treatment of a variety of disorders. Of these children 44% were under 1 year of age. Indications are discussed of which the commonest was LTB. Of the 3500 children seen with laryngotracheobronchitis (LTB) 4.6% had a tracheostomy--28% of those requiring airway intervention. Overall 67% of the children were decannulated within 10 weeks and 92% within a year. For 56% one or more further procedures prior to decannulation were required, including 34 children who required a laryngotracheop

Tracheostomy in the pediatric trisomy 21 population.

INTRODUCTION: Tracheostomy in children is often performed to alleviate airway obstruction (AO) or to facilitate long-term ventilator support due to respiratory failure of various etiologies, such as heart failure, and postoperative respiratory failure. Although many of these pathologies are common among trisomy 21 patients, tracheostomy rates among this population have not previously been reported. The aim of our study was to determine the incidence of trisomy 21 patients undergoing tracheostomy. Secondary objectives include decannulation rates and mortality associated with tracheostomy. MATE

Revealing the needs of children with tracheostomies.

INTRODUCTION: Small children with tracheostomy are at potential risk and have very specific needs. International literature describes the need for tracheostomy in 0.5% to 2% of children following intubation. Reports of children submitted to tracheostomy, their characteristics and needs are limited in developing countries and therefore there is a lack of health programs and government investment directed to medical and non-medical care of these patients. The aim of this study was to describe the characteristics of these children and identify problems related to or caused by the tracheostomy. M

Effect of speaking valve use on decannulation in tracheostomized patients.

This study investigates the impact of speaking valve use, in addition to routine treatment and rehabilitation, on decannulation outcomes and related physiological functions in tracheostomized patients. This retrospective cohort study included adult inpatients who underwent tracheostomy from January 2023 to September 2025. Patients were divided into 2 groups based on whether they used a speaking valve. Propensity score matching was applied to control confounding factors, and 80 patients were ultimately included (40 in each group). Baseline data, primary outcomes (decannulation time, success rat

The combination use of a Japanese-ready-made fenestrated speaking cannula and speaking valve in children with tracheostomy.

OBJECTIVES: A Japanese-ready-made fenestrated speaking cannula (SC) and a one-way speaking valve (SV) were actively used in children with tracheostomy in our hospitals. We reviewed our experience, focusing on the laryngeal function. METHODS: Eleven children who underwent tracheostomy using an SC and SV were retrospectively analyzed. Confirmation of steady spontaneous breathing and, when feasible, a patent upper airway and the position of the side holes of the SC in the trachea by rigid bronchoscopy were necessary. RESULTS: Tracheostomy reasons included upper airway stenosis (six cases) or re

Granuloma Following Tracheostomy Closure: An Overlooked Cause of Chronic Cough.

Tracheostomy is a life-saving procedure often performed in cases of airway obstruction, prolonged mechanical ventilation, or neurological conditions affecting respiratory function. Although decannulation is generally safe, delayed complications such as tracheal granulomas can occur and may present with nonspecific symptoms, leading to delays in diagnosis. This report describes the case of a 45-year-old previously healthy female who developed a persistent dry cough and positional shortness of breath six months after tracheostomy closure. The patient was initially admitted with bilateral vocal c

Results of the Azizkhan procedure for long-term suprastomal tracheomalacia.

One of the reasons of failure to decannulate patients after airway surgery or long-term tracheostomy is suprastomal tracheomalacia. We present 7 cases with long-term tracheostomies treated by an anterior cricoid suspension described by Azizkhan in 1993. Prior to surgery, other causes of airway obstruction, as well as the presence of gastroesophageal reflux disease should be ruled out. All of them are decannulated. This procedure is a safe surgical technique, easily reproducible and with low costs.

[Tracheotomy in brain injured patients: which patients? Why? When? How?].

The aim of this study is to determine, from the data available in the literature, the indications of tracheostomy in brain injured patients, the incidence and risk factors for complications and the follow-up required until decannulation. The incidence of tracheostomy is 10% in TBI and 50 to 70% in subpopulations with a Glasgow Coma Scale (GCS) below 9. Early complications are not specific. The most frequent late complication is laryngotracheal stenosis, which occurs in 15% and is more frequently observed in the most severe patients with major hypertonia. It is likely that tracheostomy, if need
