A DNA nanoflowers-based microneedle patch for transdermal gene and photodynamic therapy against melanoma.

DNA nanostructures show great potential as drug carriers in biomedicine, however, biological barriers and stability issues limit their applications, particularly for aggressive melanoma shielded by the skin barrier. This study presents a hyaluronic acid-based microneedle patch integrated with DNA nanoflowers to effectively penetrate the skin barrier. The DNA nanoflowers (Z/H@DFs) are self-assembled from PD-L1 antisense oligonucleotide (ASO), AS1411 G-quadruplex units, Hemin, and ZnPc. In a B16F10 melanoma mouse model, the Z/H@DFs-based microneedle patches (Z/H@DFMNs) significantly inhibited tu

Dual-mode thermo-responsive microneedle liposome patch for adaptive transdermal delivery in postherpetic neuralgia.

Postherpetic neuralgia (PHN) presents as persistent background pain accompanied by unpredictable breakthrough episodes. Current topical therapies are poorly suited to this fluctuating pain pattern because they provide limited transdermal penetration and static drug release. Here, we developed a thermo-responsive microneedle platform integrating sustained local lidocaine delivery with externally triggered accelerated release and evaluated its material characteristics, temperature-dependent release behavior, transdermal delivery performance, and in vivo pharmacokinetic behavior. The system consi

Microarray Patch Based Transdermal Allergen Immunotherapy Prevents Gliadin-induced Anaphylaxis in a Murine Model.

PURPOSE: Gliadins are the primary triggers in wheat-dependent exercise-induced anaphylaxis. Currently, there are no officially approved immune-modulating treatments for gliadin allergy. Recent in vivo studies have shown that hyaluronic acid based dissolving microarray patch (dMAP) could deliver house dust mite allergens through transdermal pathway and protect allergic asthma and atopic dermatitis in vivo. In this study, we explored the potential of dMAP for the transdermal delivery of gliadin proteins as a strategy to mitigate gliadin allergy. METHODS: C3H/HeJ mice were sensitized to gliadin

Bisoprolol transdermal patch improves orthostatic hypotension in patients with chronic heart failure and hypertension.

β blockers (BBs) play an important role in heart failure (HF) treatment. However, orthostatic hypotension (OH) is sometimes caused by BBs. The bisoprolol transdermal patch works more slowly and is long acting compared with the bisoprolol fumarate tablet. The risk of OH may be reduced by using the bisoprolol transdermal patch. We evaluated 57 consecutive patients who were taking the bisoprolol fumarate tablet for chronic HF with hypertension from November 2016 to September 2017. We switched the patients to the bisoprolol transdermal patch. Because 12 of 57 subjects could not continue using the

[Efficacy of Transdermal Patch of Bisoprolol for Paroxysmal Atrial Fibrillation after Open Heart Surgery].

2014 American Association for Thoracic Surgery (AATS) guidelines recommend beta blocker for prevention and management of perioperative atrial fibrillation and flutter for thoracic surgical procedures. In recent years, transdermal patch of bisoprolol (TDPB) has become available in Japan. We examined the efficacy of TDPB for paroxysmal atrial fibrillation (PAF) after open heart surgery. Among 289 patients who had undergone open heart surgery in our hospital from December 2013 to April 2016, 48(16.6%)patients, for whom TDPB was used for PAF, were analyzed retrospectively. The summary of our PAF p

Galantamine transdermal patch shows higher tolerability over oral galantamine in rheumatoid arthritis rat model.

Rheumatoid arthritis (RA) is a chronic autoimmune disease of idiopathic etiology that triggers inflammatory cytokines compromising the joint mobility. Epidemiological evidences recommend the utilization of galantamine (GH) to reverse the anti-inflammatory reactions induced RA. Oral administration of GH is non-selectivity due to its association with serious gastrointestinal symptoms which, could hinder its therapeutic success. Therefore, the present study aimed to validate the therapeutic potential of GH transdermal patches as a novel application to constitute an effective and tolerable deliver

Development and in vitro tuning of empagliflozin-containing dissolvable microneedle patch for enhanced transdermal delivery.

This painless method allows drugs to penetrate the outer skin layer, offering several advantages over alternative administration routes, including ease of use and the ability to bypass enterohepatic circulation. Among transdermal drug delivery systems (TDDS), microneedle patches (MNPs) are emerging as an innovative approach for minimally invasive drug delivery, enhancing the skin permeation of substances ranging from macro to micro sizes. This study explores dissolvable microneedle patches (dMNPs) as a novel method to improve the systemic delivery of empagliflozin, an SGLT-2 inhibitor, commonl

Successful Management of Terminal Delirium With Transdermal Blonanserin Patch in a Terminally Ill Cancer Patient.

Delirium is a distressing condition in terminally ill cancer patients, often treated with antipsychotics. Administering them orally, subcutaneously, or intravenously can be challenging in severely agitated patients. Transdermal antipsychotic patches offer an alternative, but their use for terminal delirium remains underexplored. We present the case of a 73-year-old man with advanced diffuse large B cell lymphoma who developed severe mixed delirium during third-line chemotherapy. Nonpharmacological interventions and oral risperidone plus intravenous haloperidol failed to improve his condition.

Distal Radial Approach in Coronary Angiography Using a Transdermal Nitroglycerin Patch: Double-Blinded Randomized Trial.

Nitroglycerin dilates the radial artery and prevents spasm, which increases the success rate of sheath cannulation through the conventional transradial approach. However, the effects of nitroglycerin on distal radial approach (DRA) procedures are not known. The aim of this study is to elucidate whether a transdermal nitroglycerin patch improves the rate of successful DRA cannulation. A total of 92 patients scheduled for coronary angiography by means of DRA randomly received (1:1) a transdermal nitroglycerin patch preintegrated with the covering material or only the covering material on their u

Benefits of topical indigo naturalis nanofibrous patch on psoriatic skin: A transdermal strategy for botanicals.

Indigo naturalis (IN) has been extensively used as a topical treatment for psoriasis. However, clinical applications of IN in ointment were hampered by its limited transdermal efficiency and dark stains. To address the aforementioned issues, nanopatches carrying IN were fabricated using poly(ε-caprolactone, PCL)/poly(ethylene oxide, PEO) and topically applied to psoriasiform skin. The ideal ratio of 5% PCL/PEO was established to be 80:20 (w/w), and 15% IN as payload was confirmed. Investigations on the three principal active components of IN release indicated that indirubin and tryptanthrin we

Transdermal Fentanyl patch: An approach to enhance tolerance of conscious proning in COVID-19 patients.

The COVID-19 ARDS appears to have worse outcomes than ARDS from other causes. It is a predictable serious complication and the key strategy is to maintain oxygenation. Adopting the prone position for conscious COVID-19 patients requiring basic respiratory support, is shown to benefit patients in terms of improving oxygenation, reducing the need for invasive ventilation and potentially even reducing mortality. Cough and myalgia are the common and most distressing symptoms seen in conscious COVID-19 patients which can impair tolerance to awake proning. Modified awake proning with application of

Neuroprotective effects of pramipexole transdermal patch in the MPTP-induced mouse model of Parkinson's disease.

Parkinson's disease (PD) is characterized by the selective death of dopaminergic neurons. To avoid inconvenience of frequent administration caused by short half life and recurrence of symptoms such as tremor and bradykinesia incurred by drug elimination, a novel long-acting pramipexole transdermal patch has been made. In the present study, we evaluated the neuroprotective effects and underlying mechanisms of pramipexole patch (PPX patch) in a subacute PD mouse model induced by 1-Methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP). The results showed that PPX patch treatment improved dyskinesia.

Development and Evaluation of a Baicalin-Loaded Phytosomal Transdermal Patch for Type 2 Diabetes Mellitus.

INTRODUCTION: Type 2 diabetes management remains challenging because of low bioavailability, side effects, and poor compliance with oral therapies. Transdermal delivery methods offer a non-invasive therapeutic approach with sustained delivery, which avoids first-pass metabolism. METHODS: A total of 207 different phytochemicals were virtually screened using i-dock against the diabetic target, PPAR-γ. Subsequently, three matrix-type transdermal patches were developed: a patch containing pure baicalin, a standard patch containing metformin, and a baicalin-loaded phytosomal patch. Phytosomes load

Effect of perioperative transdermal nicotine patch on postoperative pain: A systematic review and meta-analysis.

BACKGROUND AND AIMS: Nicotine replacement therapy (NRT) has emerged as a potential intervention for managing perioperative pain. However, their effectiveness remains unclear, as previous studies neither considered the effect of the route of administration nor differentiated the effect between smokers and non-smokers. This systematic review and meta-analysis aimed to determine the effect of a perioperative transdermal nicotine patch on postoperative pain in both smokers and non-smokers compared with a placebo. METHODS: We performed a meta-analysis of randomised controlled trials (RCTs) compari
