Primary jugular foramen fibrous dysplasia: surgically nuanced video of extradural infratemporal transjugular approach. Illustrative case.

BACKGROUND: Fibrous dysplasia of the bone is a disease caused by a somatic GNAS mutation that affects craniofacial bones and can have a mass effect on different neurovascular structures. The authors present the first case of primary jugular foramen fibrous dysplasia with occlusion of the transverse and sigmoid sinuses. OBJECTIVE: A 33-year-old man presented with a history of dizziness and occasional dysphagia over the past year. Magnetic resonance imaging showed a uniform enhanced mass in the left jugular foramen, with complete blockage of the adjacent transverse and sigmoid sinuses. The comp

Pharynx and oesophagus evaluation during the swallow using helical computerized tomography.

PURPOSE: Videofluorography (VF) and endoscopy are commonly used for dynamic imaging (DI) of pharyngeal swallowing but do not offer transverse plane (TP) information. The aim of the present study was to evaluate helical computerized tomography (HCT) to measure the DI capability pharyngeal swallowing in the TP. METHODS: The HCT scan technique used was a single-slice cine mode with scan times of 100 ms. All 15 subjects were studied supine during dry swallow, swallowing of barium sulphate jelly and 3, 10, 15 or 20 ml of a 40% barium sulphate solution. Nine subjects repeated the test twice at more

Brain herniations into the dural venous sinuses or calvarium: MRI of a recently recognized entity.

Brain herniations into dural venous sinuses (DVS) are rare findings recently described and their etiology and clinical significance are controversial. We describe five patients with brain herniations into the DVS or calvarium identified on MRI, and discuss their imaging findings, possible causes, and relationship to the patient's symptoms. All patients were examined with MRI including high resolution pre- and post-contrast T1- and T2-weighted sequences. With respect to brain herniations we documented their locations, signal intensities in different sequences, and size. We then reviewed clinica

Endovascular treatment of dural sinus thrombosis with rheolytic thrombectomy and intra-arterial thrombolysis.

BACKGROUND AND PURPOSE: Cerebral venous thrombosis is a rare entity that can be difficult to manage. Intrasinus thrombolysis is an increasingly applied intervention, but this modality carries an increased risk of hemorrhage. We describe for the first time an option with a potentially lower incidence of intracranial bleeding, the combination of the AngioJet rheolytic thrombectomy catheter with intra-arterial thrombolysis, in 2 patients with extensive dural sinus thromboses, preexisting intracranial hemorrhage, and severe progressive neurological deficits despite heparin therapy. METHODS: Four

[Idiopathic intracranial hypertension: From physiopathological mechanisms to therapeutic decision].

Clinical features include visual disturbances, headaches, and pulsatile tinnitus that can be associated with reduced quality of life, and a risk of irreversible visual impairment in some cases. Obese women of childbearing age represent the main at-risk population, and the incidence of the disease is increasing because of rising prevalence of obesity worldwide. In addition, an imbalance in sex hormones is reported as a contributing risk factor. The pathophysiology of idiopathic intracranial hypertension involves a disturbance of the evacuation pathway of intracranial fluids caused by the increa

How I do it: Management of venous bleeding from the superior petrosal vein during endoscopic microvascular decompression.

BACKGROUND: A repair strategy for venous bleeding from the superior petrosal vein (SPV) is essential during endoscopic microvascular decompression. METHOD: Sliced oxycellulose seats are rounded off, making balls around 10 mm in diameter. When venous bleeding arises from the SPV, the first oxycellulose ball is placed just behind the SPV in the surgical view. A second ball is then applied in front of the SPV. The SPV is thus immediately and entirely covered by oxycellulose, and hemostasis is safely achieved with the preservation of the SPV. CONCLUSION: This oxycellulose ball technique offers s

Objective Pulse-Synchronous "Essential" Tinnitus due to Narrowing of the Transverse Dural Venous Sinus.

Subjective tinnitus is a common problem with many etiologies. Objective tinnitus, in which the sound is perceived by both the patient and the examiner, is less common. Objective tinnitus of the vascular type, in which a pulse synchronous bruit is heard by an independent observer, is frequently related to an underlying arterial or arteriovenous malformation, most commonly a dural arteriovenous fistula (DAVF) involving the transverse and sigmoid sinuses. The remaining cases are usually termed "essential" vascular tinnitus, and are presumed to have a venous etiology. In these cases, the audible n

Comparative Analysis of Dural Venous Sinus Pressures, Gradients, and Dimensions in Idiopathic Intracranial Hypertension Between Stented and Unstented Patients.

BACKGROUND AND OBJECTIVES: Venous sinus stenting (VSS) for idiopathic intracranial hypertension (IIH) improves venous flow in patients with high trans-stenosis pressure gradients. Variability in sinus pressure profiles prompts the need for exploration of venous sinus physiology for better understanding of these findings. Stented patients exhibit characteristic pressure patterns aiding assessments of intervention success. We investigated heterogeneity in venous anatomy and pressures to establish venous profiles that may indicate the benefit of VSS for IIH. METHODS: Records of patients who unde

Endovascular Management of Intracranial Dural Arteriovenous Fistulas: Transarterial Approach.

In this second of 3 review articles on the endovascular management of intracranial dural AVFs, we discuss transarterial treatment approaches. The treatment goal is to occlude the fistulous point, including the most distal portion of the arterial supply together with the most proximal portion of the draining vein (ie, the "foot" of the vein), which can be accomplished with liquid embolic agents via transarterial access. Anatomic factors to consider when assessing the safety and efficacy of a transarterial approach using liquid embolic agents include location, angioarchitecture, and proximity of

Postpartum superior sagittal, left transverse sinus and right internal jugular vein thrombosis with left parietal infarct: A case report.

Cerebral venous sinus thrombosis is a rarely occurring condition. Pregnancy and postpartum are both known risk factors for cerebral venous sinus thrombosis. Early detection and treatment are critical, as CVST can be potentially life-threatening. Here, we present a case of a patient who developed left transverse and superior sagittal sinus thrombosis 15 days after normal vaginal delivery. The patient presented to the emergency department with complaints of irritability and an altered state of consciousness for two days. The patient also developed seizures extending from the lower limb to the up

Bilateral proptosis, dilatation of conjunctival veins, and papilloedema: a neuro-ophthalmological syndrome caused by arteriovenous malformation of the torcular herophili.

A patient with an occipital dural arteriovenous malformation (AVM) developed signs of a carotid-cavernous sinus fistula and raised intracranial pressure. Bilateral transverse sinus occlusion associated with the AVM produced these signs by rerouting intracranial venous drainage anteriorly through the cavernous sinuses and superior ophthalmic veins. Angiography and computerised tomographic reformation techniques were used to define these extraordinary cranio-orbital venous pathways.

Stent Retriever and Aspiration Catheter Biopsy of an Intraluminal Transverse Sinus Meningioma: Case Report.

BACKGROUND AND IMPORTANCE: Intracranial intraluminal mass lesions are rare and challenging to biopsy using noninvasive methods. Obtaining tissue for diagnosis is critical for guiding treatment, yet traditional approaches such as craniotomy pose significant risks. In this case report, we describe the first use of an aspiration catheter and stent retrieval system to safely and successfully perform an endovascular biopsy in a patient with an exclusively intraluminal meningioma. CLINICAL PRESENTATION: The patient had a history of medulloblastoma, which was resected and treated with craniospinal r

Thrombosed Fetal Dural Sinus Malformation: A Case Report.

BACKGROUND: Fetal dural venous sinus thrombosis is an uncommon disorder in which a blood clot forms in the developing fetus's dural venous sinuses. A detailed ultrasound examination, including neurosonography, and color Doppler are usually used to identify it, and fetal brain magnetic resonance imaging (MRI) is used to confirm the diagnosis. Although few case reports exist, the incidence of fetal occurrence remains unknown. CASE DESCRIPTION: A 25-year-old woman, primigravida, with no comorbidities at 20 weeks and 1 day, underwent a detailed ultrasound followed by MRI. The imaging revealed a t

Occipital Screw Placement Using a Navigation System for a Pathological Odontoid Fracture With a Dural Venous Sinus Variation.

Conventional fluoroscopic guidance can provide enough information to precisely insert an occipital screw in ordinary cases. However, the occipital screw creates a potential risk of dural venous sinus injury or thrombosis. In some cases, with dural sinus variation, surgeons must especially be cautious to avoid its injury. We present a rare case of proper occipital screw placement using a navigation system for a pathological odontoid fracture with a high risk of dural venous sinus injury because of anatomical variations in the transverse and occipital sinuses. A 60-year-old man who underwent thy

Venous sinus stenting for pulsatile tinnitus secondary to venous sinus stenosis: a dual-center retrospective cohort study.

Pulsatile tinnitus (PT) can emerge due to stenosis in the venous sinus, which leads to turbulent venous flows and increased pressures within the veins. Stenting of the venous sinus (VSS) is a potential intervention to relieve PT and is considered minimally invasive, as it restores proper venous flow. To evaluate the clinical, hemodynamic, and radiological outcomes of venous sinus stenting (VSS) in a real-world cohort of patients with pulsatile tinnitus (PT) and venous sinus stenosis, and to describe the symptom-imaging-manometry framework used for procedural selection in routine practice.
