Mechanical consequences of scaphotrapeziotrapezoid fusion studied by computational modelling.

Scaphotrapeziotrapezoid arthrodesis is a controversial surgical procedure for wrist disorders and its biomechanical effect remains unclear. This study investigated scaphotrapeziotrapezoid fusion based on a previously validated whole-wrist finite element model to simulate arthrodesis by creating a unified bone complex from the three bones (scaphoid, trapezium and trapezoid) in the joint. The model was analysed under physiological grasping loads to examine axial load distributions and articular contact pressures at the radioscaphoid and radiolunate interfaces. The fused complex becomes a major l

[The interosseous palmaris muscle of the thumb. Results of an anatomic study of a muscle of the first ray].

In sixteen of twenty adult human cadaveric hands a muscle fascicle was isolated from the adductor pollicis muscle. It had osseous origins from the base of the first metacarpal bone and from the trapezium bone in eleven hands. These origins were connected by a fibrous arch, which was a soft tissue origin itself. In two of the sixteen hands, muscle fibers emerged from the palmar aspect of the first dorsal interosseous muscle near the radial artery. The findings can be explained by the ontogenesis of the human hand. The described muscle is neither a part of the oblique head of the adductor pollic

Computer-simulated TOUCH prosthesis cup malposition and solutions.

INTRODUCTION: Total joint replacement has become significantly more common as a treatment for advanced trapeziometacarpal joint osteoarthritis in recent years. The latest generation of prostheses with dual-mobility designs leads to very good functional results and low rates of loosening and dislocation in the short and medium term. Biomechanical studies showed that central placement and parallel alignment of the cup with respect to the proximal articular surface of the trapezium are crucial for both cup stability and prevention of dislocation. Despite correct positioning of the guidewire, inco

Venous-trapezium coupler anastomosis in a partially congested combined ALT/TFL-flap: A case report.

BACKGROUND: Reconstruction of large soft tissue defects with extensive vascular trauma remains a challenge. Intraosseous cannulation, commonly used in emergency medicine, may offer a novel solution for venous outflow. We report the use of the trapezium bone for venous drainage in a combined anterolateral thigh and tensor fasciae latae (ALT/TFL) flap reconstruction. CASE: A patient presented with an extensive volar forearm defect, multifragmentary distal radius fracture, and transection of the radial artery, several flexor tendons and the superficial radial nerve from a circular saw injury. First Carpometacarpal Joint Motion and Proximal Migration of the First Metacarpal After Tensioning of a Suture Device Suspensionplasty Compared With Trapeziectomy: A Biomechanical Cadaver Study.

PURPOSE: The purpose of this cadaveric biomechanical experiment was to evaluate the effects of suture button suspensionplasty of the first carpometacarpal joint on thumb biomechanics and thumb position compared with an intact, arthritic specimen. METHODS: Six tendons in 8 cadaver hands were loaded to simulate 6 activities of daily living and passively moved through a circumduction motion. Proximal migration of the base of the first metacarpal was measured using optical motion sensors in the intact hand, after trapeziectomy, and following insertion of a suture button suspensionplasty with nomi

A computational approach to the "optimal" screw axis location and orientation in the scaphoid bone.

PURPOSE: The development of small cannulated screws permitted minimally invasive percutaneous fixation of acute scaphoid fractures. There are known mechanical advantages to increased screw length and central screw placement, as well as documented deleterious effects of screw malposition, including articular protrusion, proximal pole fracture, and nonunion. The purpose of this study was to compare 2 methods of calculating a screw axis accessible via a volar surgical approach. METHODS: To prevent screw protrusion through the surface of the scaphoid, we required the central screw axis to be cont

Total carpometacarpal joint dislocation combined with trapezium fracture, trapezoid dislocation and hamate fracture.

Multiple metacarpal dislocations combined with carpal fracture - dislocations are rare injuries. We report a new combination of these injuries where fracture-dislocation of the base of the 1st metacarpal bone occurred simultaneously with a comminuted fracture of the trapezium, dislocation of the trapezoid and metacarpal joints (2nd to 5th) and an avulsion fracture of the hamate. This specific carpal injury has not been previously described and our description will contribute to understanding the mechanism of these complex injuries. The injury pattern in the case featured here was multifaceted

[Amyloid arthropathy of the hand in patients on chronic hemodialysis].

Among 70 patients treated by hemodialysis for renal failure and operated for carpal tunnel syndromes, 17 presented with bone and joint lesions. Seven of them had destructive arthropathies of the distal interphalangeal joint, bilateral in 6 cases. Two radiological stages are described. Radiolucent lesions of the carpal bones were present in all the patients ("carpal amyloidosis"). The most frequent sites were around the radius-scaphoid-lunate and scaphoid-lunate joint spaces. Scaphoid-trapezoid-trapezium arthropathy was frequent with dynamic instability in one case. One patient had a pathologic

[Treatment of basal joint osteoarthritis by Swanson's trapezium implant arthroplasty].

BACKGROUND: Goal of the present study was the evaluation of clinical and radiological results after treatment of the first carpometacarpal joint by trapezium resection and implantation of a Swanson silicone prosthesis by means of a retrospetive study. However, up to now, only a few long-term data for this surgical technique are available. METHODS AND PATIENTS: Retrospectively the results of 100 trapezium resections in 72 patients with subsequent joint replacement by a Swanson silicone prosthesis have been followed up over 8.6 years on average. Besides the range of motion, the strengh in grip,

Anatomical study of the ligamentous attachments and articular surfaces of the trapeziometacarpal joint. Consequences on surgical management of its osteoarthrosis.

In the goal to optimize conservative surgical techniques of the trapeziometacarpal joint in cases of moderate osteoarthritis, we have defined the relationships between the ligamentous attachments and the articular surfaces onto the trapezium and the first metacarpal bone on the one hand, and the dorsovolar and the transverse diameters of the articular surfaces on the other hand. Thirty-six trapeziometacarpal joints (from 18 fresh cadavers) were studied. They were separated into two groups depending on the macroscopic assessment of chondral disease. Group A included stages I to III (no osteoart

Joint replacement for trapeziometacarpal osteoarthritis: implants and outcomes.

For thumb carpometacarpal osteoarthritis, trapeziectomy provides generally good results with reduction of pain and restoration of thumb mobility, but shortening of the thumb may at least in theory lead to reduction in grip and pinch strength. Furthermore, instability of the base of the first metacarpal and contact with the scaphoid may occur regardless of the several described techniques with ligament reconstruction and tendon interposition. Thus, for years alternatives to trapeziectomy have been explored and reported with disputable success. Often the road from idea to implantation in patient

Biomechanical Comparison of 5 Different Fixation Constructs in a Trapeziometacarpal Joint Arthrodesis Model.

PURPOSE: Trapeziometacarpal joint (TMC) arthrodesis has a high rate of nonunion. This biomechanical analysis sought to determine the stiffness of 5 fixation methods in a TMC joint arthrodesis model. METHODS: Five fixation constructs were tested in a validated porcine model: crossed 1.1-mm K-wires, crossed 1.6-mm K-wires, crossed headless compression screws (HCSs), compression plating (CP), and locked compression plating (LCP). The cantilever bending stiffness was measured in abduction, adduction, flexion, and extension. Samples were loaded to failure in extension, and the mode of failure was

Skeletal scintigraphy of the wrist in suggested scaphoid fracture.

Skeletal scintigraphy was performed in 187 patients with clinical suspicion of a scaphoid fracture but with normal radiographic findings. In 85 patients focally moderately (12 patients) or markedly to intensely (73 patients) increased radionuclide uptake was seen, over either the distal radius or carpal bones. A fracture corresponding to markedly increased uptake was eventually verified in 15 out of 26 patients in the distal radius, in 21 out of 28 in the scaphoid but in only 6 out of 18 in other carpal bones. One fracture of the body and one of the hook of the hamate were found, one fracture

Partial trapeziectomy for Eaton stage III thumb carpometacarpal arthritis: ligament reconstruction with tendon interposition using the entire flexor carpi radialis and interference screw fixation.

BACKGROUND: Trapeziectomy with ligament reconstruction and tendon interposition (LRTI) is currently the most popular technique for treating primary osteoarthritis of the thumb carpometacarpal joint. However, reduced trapezial height has sometimes been reported after LRTI. Longer immobilization and delayed rehabilitation times are also problematic. In this study, we evaluated the clinical and radiological outcomes of patients who underwent our modified LRTI procedure. METHODS: Retrospective study included 26 thumbs in 24 cases with advanced stages. Our modified Burton's procedure was as follow

Partial Trapeziotrapezoid Resection and Thumb Range of Movement After Trapeziometacarpal Joint Fusion-A Biomechanical Study.

PURPOSE: Trapeziometacarpal (TMC) joint arthrodesis is an effective treatment for stage III osteoarthritis. Although this procedure alleviates thumb pain and restores grip power and pinch strength, persistent limitation of thumb movement is inevitable. This biomechanical study aimed to investigate the altered kinematics of thumb circumduction motion after TMC joint arthrodesis and subsequent excision of the trapeziotrapezoid (TT) and trapezio-second metacarpal (T-2MC) joint spaces. METHODS: Eight cadaver upper extremities were mounted on a custom testing apparatus. The hand and carpal bones w

Carpal fractures.

Carpal fractures are exceedingly rare clinical entities and are often associated with concomitant injuries. In this review, we focus on fractures of the carpus, excluding the scaphoid, and provide an update on the current consensus as to mechanism, diagnosis, management, outcomes, and complications after such injuries.

De la Caffinière arthroplasty for basal thumb joint osteoarthritis.

We performed 20 de la Caffinière trapeziometacarpal arthroplasties for osteoarthritis occurring only at this joint and reviewed all patients after periods of up to ten years. Eighteen arthroplasties were satisfactory postoperatively, although all 20 patients had a satisfactory range of motion and only one experienced pain after surgery such that it impeded normal function. Failure occurred in two patients and was due to overreaming of the trapezium during surgery and a traumatic dislocation. A radiolucency between the prosthesis and bone was observed in one arthroplasty, although this patient

[Long-term results after the surgical treatment of saddle joint arthrosis with Swanson's silastic prosthesis].

The use of a silastic implant in resection arthroplasty of the carpometacarpal joint of the thumb allows the joint space to be preserved. With the Swanson type arthroplasty relief of pain, good function, as well as stability and improved pinch force can be obtained, avoiding the consequence of carpal instability by narrowing of the joint space associated with resection of the trapezium alone or in combination with tendon interposition arthroplasty. The two main problems associated with Swanson arthroplasty are prosthetic dislocation and longterm stability of the prosthesis itself. The joint su

Surgical Repair of an Avulsed Distal Flexor Carpi Radialis Tendon in a Boxer: A Case Report.

CASE: A 25-year-old professional boxer presented with a right distal flexor carpi radialis (FCR) tendon avulsion after sustaining an injury while boxing. The avulsion was identified and confirmed with magnetic resonance imaging, and the tendon was successfully reinserted into the trapezium. The patient returned to professional boxing 10 months later without complication. CONCLUSIONS: Distal FCR tendon avulsions are rare. Occasionally, this tendon can avulse after an application of excessive force. For some patients, unrepaired distal FCR tendon avulsions may prevent competitive performance. I
