[Soft tissue management of severe bone defect in the maxillary anterior area: a clinical guideline on the decision-making process].

Vertical bone augmentation (VBA) in the anterior maxilla is a technically and biologically challenging technique with high complication rates and unpredictable outcomes. Selecting appropriate soft tissue management strategies is crucial to avoid short-term and long-term complications. This article aims to assist clinicians in making treatment decisions based on different soft tissue conditions during VBA. It is divided into two parts. Incision and flap design based on the quality and quantity of different soft tissues during VBA. For mucosa of good quality and quantity, a trapezoidal flap inci

Biomechanical influence of plate configurations on mandible subcondylar fracture fixation: a finite element study.

Mandible subcondylar fractures have very high complication rate, yet there is no consensus on suitable plate design for optimal patient outcomes. Our study is aimed at comparing single mini, trapezoid, lambda, strut and double mini plates. A finite-element (FE) model of intact mandible was developed based on healthy CT-scan data, which was further virtually osteotomized and fixated with plates. The cortical and cancellous bones were assigned region-specific orthotropic and heterogenous isotropic material properties respectively. The models were subjected to six load cases representing the mast

Spinous process-splitting laminoplasty using hydroxyapatite spinous process spacer.

Between February 1986 and August 1989, 45 patients with cervical myelopathy were treated by spinous process-splitting laminoplasty. Hydroxyapatite intraspinous spacers were used to maintain the enlargement of the cervical spinal canal. The shape of this spacer is trapezoidal. After sagittal splitting of the spinous process, the spacer was inserted between the two halves and affixed with the wire. Histologic study showed there was good fusion between the spacer and bone. In all cases, good enlargement of the cervical spinal canal was achieved. Spacer displacement, wire breakage, and postoperati

Diagnostic accuracy of multidetector computed tomography for patients with suspected scaphoid fractures and negative radiographic examinations.

PURPOSE: The aim of this prospective study was to evaluate the diagnostic accuracy of multidetector computed tomography (MDCT) in detecting occult scaphoid fractures. MATERIALS AND METHODS: A total of 54 patients with a clinically suspected scaphoid fracture and negative initial conventional radiographs were evaluated with 64-row MDCT wrist examinations within 1 week of the trauma. The gold standard used was the diagnosis on MRI done within 1 week after MDCT. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of MDCT were calculated. RESULTS: M

[Amyloid arthropathy of the hand in patients on chronic hemodialysis].

Among 70 patients treated by hemodialysis for renal failure and operated for carpal tunnel syndromes, 17 presented with bone and joint lesions. Seven of them had destructive arthropathies of the distal interphalangeal joint, bilateral in 6 cases. Two radiological stages are described. Radiolucent lesions of the carpal bones were present in all the patients ("carpal amyloidosis"). The most frequent sites were around the radius-scaphoid-lunate and scaphoid-lunate joint spaces. Scaphoid-trapezoid-trapezium arthropathy was frequent with dynamic instability in one case. One patient had a pathologic

Os Styloideum and Third Metacarpal Partial Coalition Nonunion After Traumatic Fracture: A Report of Three Cases.

The Os Styloideum (OS) is an accessory carpal bone located in the dorsal aspect of the wrist between the base of the second and third metacarpal bones and the trapezoid and the capitate. It is frequently partially coalited with the metacarpal bones and is usually asymptomatic. It may become symptomatic after a traumatic fracture, developing swelling, edema, and pain. We report 3 cases with a fracture of an OS partial coalition with the third metacarpal bone with signs of delayed consolidation. They all complain of persistent pain at the dorsal aspect of the hand that increased with activity. O

Collapsed scaphoid non-union with dorsal intercalated segment instability and avascular necrosis treated by vascularised wedge-shaped bone graft and fixation.

Patients with the specific problem of a collapsed Nakamura volar type, scaphoid synovial pseudarthrosis with avascular necrosis on both magnetic resonance imaging and intraoperative inspection were studied to determine the ability of a trapezoidal wedge-shaped structural bone graft vascularised by the 1,2 intercompartmental supraretinacular artery to simultaneously achieve correction of the scaphoid dimension and a high union rate. Fifteen patients with this specific problem were followed for a mean of 32.1 months and achieved union in all cases at a mean of 11.5 weeks. Improvements were seen

Skeletal scintigraphy of the wrist in suggested scaphoid fracture.

Skeletal scintigraphy was performed in 187 patients with clinical suspicion of a scaphoid fracture but with normal radiographic findings. In 85 patients focally moderately (12 patients) or markedly to intensely (73 patients) increased radionuclide uptake was seen, over either the distal radius or carpal bones. A fracture corresponding to markedly increased uptake was eventually verified in 15 out of 26 patients in the distal radius, in 21 out of 28 in the scaphoid but in only 6 out of 18 in other carpal bones. One fracture of the body and one of the hook of the hamate were found, one fracture

[Scapholunate dissociation].

Scapholunate dissociation is a rotatory subluxation of the scaphoid with widening of the gap between the scaphoid and lunate. It is caused by rupture of the ligaments between radius, scaphoid, lunate and capitate bones. The scaphoid has lost its stabilizing function as a bridge between the proximal and distal carpal row, so that the wrist shows the typical "concertina" deformity of dorsal instability. The scapholunate angle measures about 100 degrees (more than 70 degrees). The subluxation of the scaphoid and lunate with the wide gap between these bones is best treated by primary repair of the

[Scaphoid malunion].

Scaphoid fracture and healing are often difficult to diagnose. The scaphoid bone may heal in an abnormal position and this malunion is usually in flexion and shortening. Fractures of the mid or distal part of the bone are mainly concerned. This malunion may be the cause of an alteration of the carpal mechanics and finally of osteoarthritis revealed by pain and decrease of motion and strength. The intrascaphoid angle of Amadio helps to calculate the magnitude of the deformity and to plan the correction. This malunion has to be corrected only if symptomatic. Two other types of malunions exist: h

MDCT and radiography of wrist fractures: radiographic sensitivity and fracture patterns.

OBJECTIVE: The purpose of our study was to determine which wrist fractures are not prospectively diagnosed at radiography using CT as a gold standard and to identify specific fracture patterns. MATERIALS AND METHODS: Through a search of radiology records from January 1 to December 31, 2005, 103 consecutive patients were identified as having radiographic and CT examinations of the wrist. After excluding incomplete or nondiagnostic examinations and those with a greater than 6-week interval between imaging studies, the final study group consisted of 61 wrist examinations in 60 patients. Resection Arthroplasty Is a Valuable Strategy for Humeroradial Synostosis - A Case Report.

A 15-year-old girl with humeroradial synostosis since birth underwent a resection arthroplasty. A trapezoidal resection osteotomy of approximately 2 cm was performed at the anterior part of the bone flexure. This resulted at 18 months in an elbow arc of motion of 60°-110° and forearm pronation/supination of 40° and 60° without postoperative complications and improved disabilities of the arm, shoulder and hand and Hand 20 scores. Radiographic analysis revealed a humeroradial joint with a maintained pseudarthrosis and hinged motion at the humeroulnar joint. When performed by an experienced surge

The impact of lunate type on scapho-trapezio-trapezoid arthritis in trapeziometacarpal osteoarthritis patients.

BACKGROUND: Type II lunate has an extra facet with the hamate. This difference alters the biomechanics of the midcarpal joint, influencing the development of scapho-trapezio-trapezoid (STT) arthritis. We aimed to investigate whether, in patients with trapeziometacarpal (TMC) osteoarthritis (OA), involvement of the STT joint is associated with the lunate type. METHODS: A retrospective review was performed on 121 patients (mean age of 69 years) diagnosed with TMC OA. The lunate type was classified based on the presence of the facet detected on computed tomography images. The incidence of STT OA

Radiographic Characteristics of Carpal Instability Associated With Scaphotrapeziotrapezoid Osteoarthritis.

PURPOSE: Scaphotrapeziotrapezoid osteoarthritis (STT-OA) may be related to the progression of carpal instability. This study analyzed the radiographic carpal indices in wrists with varying grades of STT-OA to investigate an association between the severity of STT-OA and the occurrence of dorsal intercalated segment instability (DISI), and to identify radiographic characteristics of carpal instability associated with STT-OA. METHODS: We retrospectively measured carpal indices in 135 wrists with STT-OA (Crosby's classification grade 2, 62; grade 3, 73). Carpal indices measured included radiolun

Cleft hand.

Definition, incidence, genetics, and etiology of typical cleft hand are discussed in this paper. Cleft hand, central polydactyly, and osseous syndactyly were induced experimentally when the same teratogenic factors acted on the embryos at the same developmental stage. Typical cleft hand, central polydactyly, and syndactyly should be classified together as manifestations of the same entity, that is, failure of induction of finger rays. Typical cleft hand may be further subdivided into five types on the basis of the number of defective finger rays. Treatment of the typical cleft hand involves co

Cone-beam computed tomography for primary investigation of wrist trauma provides a new map of fractures of carpal bones.

In 2016, our primary modality for radiological examination of wrist trauma, was changed from radiography to cone-beam computed tomography (CBCT). This is a retrospective survey of carpal bone fractures detected by CBCT during 6 months in 2016/2017, compared with those found on conventional radiographs during 6 months in 2013/2014. The incidence of carpal fractures was three times higher during the CBCT period (92/100,000 per year) compared with the radiography period (29/100,000 per year) and the spectrum of anatomical locations was different between the two periods, with fractures of the luna

Vertebral pathology in an ornithopod dinosaur: a hemivertebra in Dysalotosaurus lettowvorbecki from the Jurassic of Tanzania.

A vertebral fragment of the Late Jurassic ornithopod dinosaur Dysalotosaurus lettowvorbecki from Tanzania is described. It consists of a hemivertebra that is co-ossified with a complete vertebral centrum. The hemivertebra causes a hyperkyphotic posture of the vertebral column with an angle of approximately 35 degrees between the end plates of the vertebra, that is, a dorsal bending of the vertebral column. Associated with this is a 15 degrees lateral bending, which suggests a scoliosis. Micro-CT scans reveal thickening of the cortical bone in the hemivertebra and the complete vertebral centrum
