Early Relational Trauma and the Development of Internalizing Symptoms in Females with Postural Orthostatic Tachycardia Syndrome.

Postural orthostatic tachycardia syndrome (POTS) is a disorder of the autonomic nervous system (ANS) that primarily impacts females. This study explored the relationship between early relational trauma, internalizing symptoms, and autonomic dysfunction in females with POTS. Previous research has established a link between childhood trauma and long-term health outcomes, but the specific impacts of relational trauma, particularly its physical and emotional manifestations, are less understood. Our findings revealed statistically significant correlations between childhood relational trauma, POTS-r

Assessment and management of carotid artery trauma associated with mild head injury.

Management of patients with trauma to the central nervous system is a challenge for the neuroscience nurse. When trauma to the carotid artery is present, the management of closed head injury becomes even more complex. Although the incidence of carotid artery dissection is small, the risks of morbidity and mortality are high. This article discusses the mechanism of injury, associated symptoms, and theories of management for victims of carotid artery injury associated with blunt craniocervical trauma. New assessment considerations for nursing diagnosis and intervention are provided.

Anatomical features of the developing brain implicated in pathogenesis of hypoxic-ischemic injury.

The developing nervous systems is subject to damage from lack of vital substances necessary for normal maturation and function as well as from trauma or a variety of toxins and infectious agents. By far, the most important of these is inadequate oxygen delivery to the fetus in utero, and/or during the intrapartum and/or early neonatal period. Many types of lesions have been described under the rubric of hypoxic-ischemic encephalopathy, a major proportion of which are found only in the immature nervous system and essentially are never seen later in life. Moreover, a large number are primarily h

Scapulothoracic dissociation secondary to major shoulder trauma.

Three skeletally immature patients with scapulothoracic dissociation were reviewed. A 5-year-old child's arm, caught in a conveyor belt, led to complete upper extremity amputation. Multiple fractures, muscular damage, and cutaneous and subcutaneous tissue disruption (degloving) were present throughout the avulsed extremity. Replantation was considered, but not carried out because of these extensive injuries. He was treated with a myoelectric prosthesis. Two older boys had scapulothoracic dissociation (one open, one closed) associated with clavicular diaphyseal fractures following blunt trauma.

Biological mechanisms underlying adverse mental health outcome after trauma.

By now it is well known that trauma-related psychiatric disorders are associated with a broad range of biological dysregulations. This special issue focused on research aiming to advance our understanding of the biological mechanisms underlying adverse mental health outcome after trauma, in particular the development of trauma-related disorders and differences in individual susceptibility (i.e. risk or resilience) to their development. We included 51 papers published in EJPT between 2021 and 2023, either directly submitted to the special issue or whose focus clearly fitted the aim of the speci

The biological effects of occlusal trauma on the stomatognathic system - a focus on animal studies.

The aim of this review of the literature was to assess the biological effects of occlusal trauma on the stomatognathic system focusing on animal studies. However, there are no conclusive explanations on the association between occlusal trauma and disease of the stomatognathic system. A literature survey was performed using the Medline database, covering the period from 1967 to 2012. Over 300 abstracts were reviewed, and 70 manuscripts were selected. Additional references from citations within the articles were obtained, and current textbooks were also used. This review does not include the eff

Advances in Exosome-Based Therapies for the Repair of Peripheral Nerve Injuries.

Peripheral nerve injuries (PNIs) are the term used to describe injuries that occur to the nerve fibers of the peripheral nervous system (PNS). Such injuries may be caused by trauma, infection, or aberrant immunological response. Although the peripheral nervous system has a limited capacity for self-repair, in cases of severe damage, this process is either interrupted entirely or is only partially completed. The evaluation of variables that promote the repair of peripheral nerves has consistently been a focal point. Exosomes are a subtype of extracellular vesicles that originate from cellular s

Cerebrospinal Fluid Leaks of the Skull Base: Physiology and Pathophysiology.

Cerebrospinal fluid (CSF) is a colorless body fluid present within the central nervous system. CSF carries out essential functions in the central nervous system through maintenance of homeostasis, protection from trauma, and control of development. Classic experiments on CSF production postulated that CSF is produced in the choroid plexus and absorbed in the arachnoid granulations, but modern studies suggest that CSF is produced and absorbed through the entirety of the neuroaxis, with its movement primarily dependent on the cardiac cycle. This review will cover current concepts of CSF physiolo

Spontaneous Clostridium perfringens Meningitis and Brain Abscess in a Neonate.

This case describes a neonate who presented with spontaneous Clostridium perfringens meningitis and brain abscess. The abscess was drained, and the infant completed a 6-week course of antibiotics. Throughout this time the infant remained well with no need for intensive care. C. perfringens central nervous system infections are associated with trauma and poor outcomes. This case highlights that the spectrum of disease can include spontaneous infection with a relatively mildly clinical course demonstrating the importance of 16s polymerase chain reaction in culture-negative cases and its role in

Paresthesias: a practical diagnostic approach.

Paresthesias may be caused by central or peripheral nervous system abnormalities. Central nervous system-induced paresthesias are most commonly caused by ischemia, structural or compressive phenomena, infection, inflammation or degenerative conditions. Peripherally induced paresthesias can be caused by entrapment syndromes, metabolic disturbances, trauma, inflammation, connective tissue diseases, toxins, hereditary conditions, malignancies, nutritional deficiencies and miscellaneous conditions. Confirming the diagnosis and establishing an etiology may require appropriate laboratory and radiolo

Sudden death: Neurogenic causes, prediction and prevention.

Sudden death is a major health problem all over the world. The most common causes of sudden death are cardiac but there are also other causes such as neurological conditions (stroke, epileptic attacks and brain trauma), drugs, catecholamine toxicity, etc. A common feature of all these diverse pathologies underlying sudden death is the imbalance of the autonomic nervous system control of the cardiovascular system. This paper reviews different pathologies underlying sudden death with emphasis on the autonomic nervous system contribution, possibilities of early diagnosis and prognosis of sudden d

The neurobiology of childhood trauma, from early physical pain onwards: as relevant as ever in today's fractured world.

Background: The situation in the world today, encompassing multiple armed conflicts, notably in Ukraine, the Coronavirus pandemic and the effects of climate change, increases the likelihood of childhood exposure to physical injury and pain. Other effects of these worldwide hardships include poverty, malnutrition and starvation, also bringing with them other forms of trauma, including emotional harm, neglect and deliberate maltreatment. Objective: To review the neurobiology of the systems in the developing brain that are most affected by physical and emotional trauma and neglect. Method: The re

Primary meningioma of the scalp as a late complication of skull fracture: case report and literature review.

A case of primary meningioma of the scalp is presented in which the anatomical association with a previous skull fracture is demonstrated and pathogenesis discussed. The investigations and surgical treatment are briefly described. Review of the literature reveals a strong association between trauma of the central nervous system and intracranial meningioma. Occasional reports also exist of extra-cranial meningioma following trauma. This is, however, the first report of such an association in the scalp.

Systems of Neurotrauma HealthCare.

The delivery of neurotrauma care exists in the context of a system addressing the continuum of injury from prevention to postacute care and community reintegration. Given this complexity, there is persistent high variability in care nationally. To understand the need for improved standardization, it is important to discuss the history trauma system evolution. There are currently established efforts in the field of neurotrauma intended to harmonize data collection and practice patterns. However, to fully develop into mature systems with nuanced outcome measures specific to brain and spine injur

Elevation of serum plasminogen activator inhibitor-1 predicts postoperative delirium independent of neural damage: a sequential analysis.

Older adult surgical patients are susceptible to developing delirium. Early intervention can be initiated if a potential biomarker associated with delirium can be identified during the acute phase of surgery. Therefore, we investigated the changes in the levels of serum inflammatory mediators responsible for delirium. Serum biomarkers were measured preoperatively to postoperative day 3 in 96 patients who underwent esophageal cancer surgery and compared between patients who did and did not develop delirium. Serum concentrations of the brain-derived phosphorylated neurofilament heavy subunit rem

Stem cell sprays for neurological injuries: a perspective.

Injuries to the brain and spinal cord have major clinical consequences with high costs for healthcare systems. Neural cell transplantation therapies have significant translational potential to promote regeneration post-injury with clinical trials commencing for various pathologies. However, there are challenges associated with current clinical approaches used for systemic or direct delivery of transplant cells to neural tissue in regenerative applications. These include risks associated with surgical microinjection into neural tissue (e.g. haemorrhage, cell clumping) and high cell loss due to

[Intraoperative neuromonitoring during endocrine surgery].

Recurrent laryngeal nerve (RLN) injury is a known complication of thyroid surgery. Patients undergoing reoperations or extensive resections of malignancies carry an increased risk of nerve damage. The gold standard for preventing RLN injury is identification of the RLN. The development of a new technique has enabled intraoperative testing of RLN function. Intraoperative neuromonitoring has an excellent specificity and negative predictive value in which an unchanged positive signal is highly predictive of intact nerve function. This technique facilitates RLN identification in altered anatomy as

Human cadaveric study of subscapularis muscle innervation and guidelines to prevent denervation.

The upper and lower subscapular nerves provide innervation to the subscapularis muscle. However, the axillary nerve may provide a significant innervation to the lower portion of the muscle. The prevalence and patterns of anomalous innervation of the subscapularis muscle were studied to determine if these variations increased the risk of muscle denervation during open shoulder surgery. Twenty human cadaveric shoulders were dissected, and the innervation to the subscapularis was defined. The distance from the nerve insertion to the shoulder joint was measured in neutral and maximal external rota

Stripping and ligation of the saphenous vein.

Successful varicose vein surgery depends on accurate preoperative assessment and individualized treatment for various combinations of venous insufficiency. Noninvasive duplex scanning is currently the gold standard in varicose vein evaluation. Flush ligation and division of the great saphenous vein and its tributaries, inverted downward stripping to below the knee combined with stab avulsion of varicosities, have yielded excellent results for patients with great saphenous vein reflux. Additionally, identifying and correcting incompetent calf perforating veins is necessary to achieve a satisfac
