"Slow" Trauma to Families with the Demise of Family-Centered Care and Trauma-Informed Care.

AbstractIn the article "Slow Trauma: Reframing Concepts of Moral Injury in Trauma-Informed Ethics Consultation," Charles Love argues that family members are at significant risk for "moral injury and trauma" through the experience of the hospitalization of their loved ones. But Love adds an important insight about the type of moral injuries experienced by family members, distinguishing between what psychologists refer to as "fast" trauma and "slow" trauma. Love's detailed exposition of slow trauma is an important refinement of my article "TIEC, Trauma Capacity, and the Moral Priority of Surroga

Age and trauma center-specific disparities in splenectomy rates for pediatric trauma patients.

BACKGROUND: Non-operative management (NOM) of blunt splenic injury (BSI) is the target for pediatric trauma care. Yet, the rates of splenectomy have historically been higher at non-pediatric trauma centers (PTCs). We sought to determine if this difference improved contemporaneously. METHODS: Using the National Trauma Data Bank, 26 707 pediatric patients (age ≤18 years, 2013 to 2021) with BSI were identified and analyzed based on care at adult trauma centers (ATCs), dual trauma centers (DTCs), or pediatric trauma centers (PTCs) using descriptive statistics and logistic regressions. Management of pediatric blunt abdominal trauma in a Dutch level one trauma center.

PURPOSE: Most children with intra-abdominal injuries can be managed non-operatively. However, in Europe, there are many different healthcare systems for the treatment of pediatric trauma patients. Therefore, the aim of this study was to describe the management strategies and outcomes of all pediatric patients with blunt intra-abdominal injuries in our unique dedicated pediatric trauma center with a pediatric trauma surgeon. METHODS: We performed a retrospective, single-center, cohort study to investigate the management of pediatric patients with blunt abdominal trauma. From the National Traum

Association of Trauma Center Level and Patient Volume with Outcomes for Penetrating Thoracic Trauma.

BACKGROUND: We investigated the potential link between trauma center American College of Surgeons verification level and institutional volume of penetrating thoracic trauma with outcomes for patients with penetrating thoracic trauma. METHODS: Penetrating thoracic injuries were identified in the National Trauma Data Bank from 2013 to 2016. Primary exposures were trauma center American College of Surgeons verification level and annual penetrating trauma caseload by center. Cox models were used to evaluate the association between primary exposures and mortality. Poisson regression was used to ev

Impact of Device-Associated Infection on Trauma Patient Outcomes at a Major Trauma Center.

BACKGROUND: Catheter-associated urinary tract infection (CAUTI) and ventilator-associated pneumonia (VAP) are considered performance measures. We analyzed the incidence, prevalence, and risk of CAUTI and VAP in trauma patients, as well as the demographic and injury factors related to these infections and their relative risks of negative outcomes (prolonged length of stay [LOS], sepsis, and death). METHODS: Trauma registry data were analyzed (age >18 y; LOS >24 h) from January 1, 2007, to December 31, 2011. Demographics and injury location, severity, and type were analyzed relative to outcomes

Infectious complications of noncombat trauma patients provided care at a military trauma center.

Infectious complications are reported frequently in combat trauma patients treated at military hospitals. Infections in 4566 noncombat related trauma patients treated at a military trauma center were retrospectively reviewed from 1/2003 to 5/2007 using registry data. Burns, penetrating, and blunt trauma accounted for 17%, 19%, and 64%, respectively; the median age was 38 and 22% were female. Pulmonary infections were present in 4.2% of patients, 2.4% had cellulitis and wound infections, 2.2% urinary infections, and 0.7% sepsis. On univariate analysis, infected patients were more likely to be a

The efficacy of a two-tiered trauma activation system at a level I trauma center.

BACKGROUND: By using current American College of Surgeons trauma center triage criteria, 52% of patients transported to our level I trauma center are discharged home from the emergency department (ED). Because the majority of our trauma transports were based solely on mechanism of injury, we instituted, in 1990, a two-tiered trauma team activation system. Patients are triaged into major and minor trauma alert categories based on prehospital provider information. For minor trauma patients, respiratory therapy, operating room staff, and blood bank do not respond. The current study evaluated this

Assessment of severity, causes, and outcomes of hospitalized trauma patients at a major trauma center.

BACKGROUND AND AIM: Trauma is a major cause of hospitalization in both civilians and military personnel (MP). It constitutes one of the most common causes of work layoffs, lowered efficiency, and handicaps in the civilian as well as the military population. Assessment of injury parameters and severity is useful. It allows for grading, grouping, classification, and assessment of outcomes of trauma patients for comparison with world standards. This study aimed to assess the Injury Severity Scores (ISS), recognize the source, and types of injuries leading to hospitalization of civilian and milita

[Closed traumas of the thorax. Assessment of two years activity at the Dakar Trauma Center].

Two years after opening the Dakar Trauma Center has received 179 cases of blunt chest trauma without prehospital treatment. Road accidents were the leading cause. The initial mortality was 29 patients (16.20%) and 150 patients arrived alive and were treated. The initial mortality was primarily due to the associated extrathoracic injuries mainly hemorrhagic with hemoperitoneum in 44.82% and brain hemorrhage in 24.14% of cases. Secondly, it was due to the thoracic injuries with 41.37% of anterior flail chest and massive hemothorax in 65.51% of cases. Among the 150 patients treated, 42% had extra

Audit of Motor Vehicle Accidents at a Trauma Center.

Background Motor vehicle accidents (MVAs) are the leading cause of accidental deaths in India. An audit of trauma cases is required in order to improve hospital policy and patient care, as well as to change the attitude and perspective of healthcare staff. Methods A retrospective observational study was performed on MVA trauma victims admitted to a tertiary trauma care center. Parameters included mean age with range, gender distribution, length of hospital stay (LOS), anatomical location of injuries, and the percentage of age groups and number of patients undergoing a nonoperative approach ver

Operative Rib Fixation at a Rural Trauma Center.

INTRODUCTION: Operative rib fixation is becoming more common in the management of rib fractures, with studies showing benefits of surgery in decreasing time on mechanical ventilation, ICU and total hospital length of stay, and mortality. This paper will review the data from our institution to determine if these benefits were seen in our trauma population. METHODS: A retrospective review of the institution's trauma database was conducted, including operative patients (n = 36), control patients (nonoperative patients from October 2018 to October 2019, n = 207), and selected control patients bas

Trauma center-related biases in injury research.

OBJECTIVES: Most studies from trauma centers analyze and present combined data on patients from their surrounding communities and patients referred for specialized services from service areas of other hospitals. Information is needed about the effect of combining data from the two groups on conclusions about injury in the community. METHOD: All injured patients seen in a trauma center emergency department of 30% of days over one year were studied concerning referral status, age, sex, type of activity when injured, injury type and severity, hospitalization, and prior medical history. RESULTS:

Femoral venous trauma in a university referral center.

Therapeutic results from extremity vascular injuries have been improved by early definitive care by surgeons trained in peripheral vascular techniques. Serious venous injuries are uncommonly seen other than in large urban referral centers or military installations. Seventeen patients with major femoral venous injuries were evaluated over the past 25 years at the North Carolina Memorial Hospital. The records from these patients were analyzed regarding mechanisms of injury, time delay for definitive care, clinical presentation, significant associated injuries, operative management of the venous

Vascular trauma observed at an urban hospital center.

The records of 50 patients with 67 major civilian type vascular injuries treated in a large urban center from 1967 to 1975 were reviewed in terms of nature of the injury, form of surgical management and early and late results. Civilian injuries differ in many aspects from those of their military counterparts and often require different treatment. In both, however, treatment protocols should be periodically re-evaluated for a number of reasons, chiefly among which are changing patterns of delivery of the injured patient to the medical center and advances in technical skills. Notable in the pres

Criteria for the qualification of geriatric patients to the trauma center.

BACKGROUND: The growth of the geriatric population, resulting in an exponential increase in the number of injured older people, determines the necessity of ensuring adequate access to specialized units such as trauma centers (TCs). The specificity of pathophysiological processes that progress with age, worsening the body's response to trauma, makes it difficult to develop an optimal triage protocol for geriatric patients, reducing high undertriage, i.e., underestimation of injuries and referral to lower-reference units. AIM: To verify the TC qualification criteria for 65+ patients and analyze

The effects of Hurricane Sandy on trauma center admissions.

BACKGROUND: Hurricane Sandy was a particularly unusual storm with regard to both size and location of landfall. The storm landed in New Jersey, which is unusual for a tropical storm of such scale, and created hazardous conditions which caused injury to residents during the storm and in the months following. This study aims to describe differences in trauma center admissions and patterns of injury during this time period when compared to a period with no such storm. METHODS: Data were collected for this study from patients who were admitted to the trauma center at Morristown Medical Center dur

Clival fractures in a level I trauma center.

OBJECT: Clival fracture (CF) is rare among head traumas. The aim of this study was to explore how radiological features observed in CF reflect the clinical picture and mechanism of injury in such cases. METHODS: Radiological data for patients with skull base fracture admitted to the Montreal General Hospital between February 2002 and October 2012 were obtained from the Quebec Trauma Registry and reviewed for CF. Identified CF was categorized by orientation and quality. Injury mechanism, clinical presentation, and follow-up outcome were obtained through retrospective chart review. RESULTS: Of
