A delay in diagnosis and management of syndesmotic injuries can result in a complex treatment challenge, leading to poorer clinical results and higher rates of unplanned, syndesmotic-related primary operations and reoperations. This article presents a simple and reproducible algorithm with case examples for the management of syndesmotic injuries with delay in diagnosis to help guide surgeons when determining the best treatment strategies for this complex injury.

BACKGROUND: The high variability in clinical manifestations of Fabry disease can lead to delays between symptom onset and correct diagnosis, and between correct diagnosis and initiation of enzyme replacement therapy. We investigated whether these delays have improved in recent years.

BACKGROUND: Most patients with advanced prostate cancer (PCa) develop bone metastases (BM) and present with bone complications like fracture. Bone-targeted agents that prevent metastasis-induced bone complications can cause adverse events. Understanding how patients view treatment options may optimize care. This study aimed to quantify how PCa patients value a hypothetical treatment that delays BM but can cause osteonecrosis of the jaw (ONJ). A motorcyclist was involved in a motor vehicle accident and presented with respiratory distress and neck swelling with surgical emphysema. He sustained gross tracheal injury, severe pneumothoraces and lung contusions. As intubation was successful, the tracheal injury was not addressed immediately in view of the other severe respiratory problems. Delayed and non-union of isolated fractures of the capitate are rare injuries. Initial plain radiographs do not always show the fractures. Especially fractures through the neck of the capitate can lead to delayed union. We report on a patient with persistent localised pain over the capitate 5 months after a fall on the hand. OBJECTIVE: To summarize the experience in treatment and diagnosis of popliteal artery trauma and to determine the factors for amputation.
METHODS: From February 1995 to January 2006, 28 patients with popliteal artery trauma were treated. The disease course was more than 8 hours. Of them, there were 25 males and 3 females, aging from 3 to 53 years. Trauma was caused by traffic accident in 12 cases, by falling from height in 3 cases, by firearm in 2 cases, by sharp instruments in 3 cases, by strangulation in 2 cases and by others in 6 cases. PURPOSE: To evaluate the influence of smoking on comorbidity, treatment, visual and general outcome in patients with scleritis.
METHODS: The smoking habits of 103 patients with a diagnosis of episcleritis or scleritis were evaluated. These patients were treated by one ruling protocol at the Leiden University Medical Center between 1997 and 2000. Medical records of each patient were evaluated in detail. A case of full-blown neurosyphilis in a 40 year old university man is demonstrated. The disease was initially misdiagnosed as Creutzfeldt-Jakob disease. This study presents the results of the treatment of 92 unilateral fractures of the ankle. Of these, 72 patients were operated on immediately after the injury and 20 patients had delayed operations. Both groups are comparable in age, injury, fracture type and treatment. From the results it can be concluded that delayed open reduction and internal fixation 5-8 days after injury will give similarly good functional results. However, postoperative immobilization and the time spent in hospital by patients treated by delayed operation is significantly longer.

So called "premature ejaculation" is a symptom of several different conditions. Almost all of these respond well to short term techniques that are fully described in this paper. Considering the contrast between the degree of marital disruption that is often caused by inability to delay ejaculation and the relatively little time and effort needed by the understanding family physician to clear up the symptom, the point is made that it is a rewarding experience to treat these patients in family practice.
