Factors Associated With Refusal of Surgery for PDAC and Its Impact: An NCDB Analysis.

BACKGROUND/AIM: Pancreatic ductal adenocarcinoma (PDAC) has poor survival despite advances in multidisciplinary care. While many patients do not undergo resection due to advanced disease or comorbidity, a small fraction refuse surgery despite recommendations. This study evaluated the association between surgical refusal and overall survival (OS) and identified factors associated with refusal of surgery. PATIENTS AND METHODS: We performed a retrospective cohort study using the National Cancer Database (NCDB), 2004-2019. Sociodemographic (age, sex, race/ethnicity, income, insurance, distance, r

Refusal of surgery in patients with laryngeal or hypopharyngeal squamous cell carcinoma.

BACKGROUND: Patients with laryngeal and hypopharyngeal cancers may refuse recommended surgery owing to concerns about voice loss. OBJECTIVES: To investigate the factors associated with surgery refusal and the survival outcomes of patients who refused surgery. PATIENTS AND METHODS: We obtained data from 24,340 surgery and 1500 refusal cases using the Surveillance, Epidemiology, and End Results database. Overall survival (OS) rates were estimated using the Kaplan-Meier method. Odds ratios for surgery refusal and hazard ratios (HRs) for death were calculated using logistic regression and Cox pr

Clinical utility of a hybrid secondary and relapse prevention program in adults with mild intellectual disability in community residential and day habilitation settings.

BACKGROUND: Despite evidence that standard substance use disorder (SUD) treatment may be less effective in people with intellectual disability (ID), there is an absence of appropriate clinical tools with which to support them. OBJECTIVES: This study examined the clinical utility of an alcohol and other drug refusal skills intervention designed to be cognitively accessible to adults with ID METHODS: Thirty individuals at high risk for or in recovery from a SUD in developmental disability services (DDS) community residential and day habilitation settings participated in the two-week refusal ski

The Ethical Obligation to Treat Infectious Patients: A Systematic Review of Reasons.

During pandemics, healthcare providers struggle with balancing obligations to self, family, and patients. While HIV/AIDS seemed to settle this issue, coronavirus disease 2019 (COVID-19) rekindled debates regarding treatment refusal. We searched MEDLINE, Embase, CINAHL Complete, and Web of Science using terms including obligation, refusal, HIV/AIDS, COVID-19, and pandemics. After duplicate removal and dual, independent screening, we analyzed 156 articles for quality, ethical position, reasons, and concepts. Diseases in our sample included HIV/AIDS (72.2%), severe acute respiratory syndrome (SAR

[School refusal].

Pediatrician are often questioned by school refusal which relies on a wide range of psychopathological features and necessitates specific approaches. This disabling condition remains underestimated and is still increasing. A poor prognosis associated with a prolonged school absence is the common hallmark of school refusals, regardless of its heterogeneity. Its seriousness warrants early identification and prompt intervention by childhood healthcare professionals, teachers and social workers. A specialized treatment is needed, closely linked with families and school. Promising developments come

Cognitive mediation of cognitive-behavioural therapy outcomes for anxiety-based school refusal.

BACKGROUND: Cognitive-behavioural therapy (CBT) has proven to be effective for anxiety-based school refusal, but it is still unknown how CBT for school refusal works, or through which mechanisms. AIMS: Innovative statistical approaches for analyzing small uncontrolled samples were used to investigate the role of self-efficacy in mediating CBT outcomes for anxiety-based school refusal. METHOD: Participants were 19 adolescents (12 to 17 years) who completed a manual-based cognitive-behavioural treatment. Primary outcomes (school attendance; school-related fear; anxiety) and secondary outcomes

A Matter of Conscience: Examining the Law and Policy of Conscientious Objection in Health Care.

Conscientious objection refers to refusal by a health care provider (HCP) to provide certain treatments, including the standard of care, to a patient based upon the provider's personal, ethical, or religious beliefs. Federal and state rules regarding conscientious objection have expanded the scope of legal protections that HCPs and institutions can invoke in support of refusal. Opponents of these rules argue that allowing refusal of care deprives patients of care that conforms to professionally established guidelines, contradicts long-standing principles related to informed consent, interferes

Understanding Surgery Refusal in Patients with Colorectal Liver Metastases.

PURPOSE: To investigate the risk factors, reasons, and survival outcomes associated with refusal of surgery for colorectal liver metastases (CRLM). Surgery is the only curative treatment for CRLM, however, some patients refuse. The risk factors and reasons for refusal remain uninvestigated in patients with CRLM. METHODS: Patients with resectable and operable CRLM referred for treatment at Copenhagen University Hospital (2013-2023), were identified through the Danish Liver Cancer Group registry. Those who refused surgery were matched 1:4 by WHO performance status with patients who accepted sur

Factors Associated With Patient's Refusal of Recommended Cancer Surgery: Based on Surveillance, Epidemiology, and End Results.

OBJECTIVES: Most non-metastatic cancer patients can harvest a preferable survival after surgical treatment, however, patients sometimes refuse the recommended cancer-directed surgery. It is necessary to uncover the factors associated with patent's decision in taking cancer surgery and explore racial/ethnic disparities in surgery refusal. METHODS: Based on the Surveillance, Epidemiology and End Results (SEER)-18 program, we extracted data of non-metastatic cancer patients who didn't undergo surgery. Ten common solid cancers were selected. Four racial/ethnic categories were included: White, bla

Refusal to take ivermectin: the associated 'risk' factors in Hoima district, Uganda.

Community-directed treatment with ivermectin (CDTI), for the control of onchocerciasis, was launched in Uganda in 1997. In East Africa, as in other endemic areas, the refusal to take the drug reduces treatment coverage and therefore poses a serious threat to attempts to eliminate onchocerciasis as a disease of public-health importance. In early 2003, an attempt was therefore made to determine the factors associated with refusal to take ivermectin during mass treatment in 2002, by interviewing the individuals, from three Ugandan villages with CDTI, who had been eligible to receive ivermectin. School Refusal Successfully Treated With Divalproex.

Divalproex was successfully used in a 7-year-old boy suffering from treatment-refractory school refusal complicated with a mood disturbance and anxiety. Divalproex had demonstrated a rapid effect in reducing school anxiety and improving school attendance after multiple failures of other agents. Divalproex may be an effective agent for treating school refusal in children with emotional and/or behavior disturbances.

An expanded self-medication hypothesis based on cognitive-behavioral determinants for heroin abusers in Taiwan: a cross-sectional study.

BACKGROUND: This study proposed and examined an expanded self-medication hypothesis (eSMH) model based on cognitive behavioral determinants, including the direct effects of negative emotional states, positive outcome expectancies and refusal self-efficacy on heroin use, and the mediating roles of positive outcome expectancies and refusal self-efficacy between negative emotional states and heroin use. METHODS: A total of 360 male heroin abusers were recruited from a drug abuse treatment center in Taiwan. Participants were asked to complete a set of questionnaires on frequency of heroin use, an

Intracranial Hemorrhage Secondary to Vitamin K Deficiency Bleeding in a Newborn.

We present a case of a 6-week-old infant who presented with seizure-like activity. Workup revealed abnormal coagulation and imaging confirmed intracranial hemorrhage. Parental refusal of vitamin K treatment at birth suggested vitamin K deficiency bleeding (VKDB) in this newborn. Though VKDB is rare in developed countries, rates have been rising which coincides with an increasing trend of parental refusal of vitamin K prophylaxis at birth.

Consolidation radiotherapy refusal in stage I-II Hodgkin lymphoma: sociodemographic determinants and impact on survival outcomes.

BACKGROUND: Consolidation radiotherapy following chemotherapy represents the standard of care for stage I-II classical Hodgkin lymphoma, yet treatment adherence varies in clinical practice. No previous study has systematically examined the patient characteristics associated with radiotherapy refusal or its impact on survival outcomes. We aimed to identify determinants of consolidation radiotherapy refusal and evaluate its consequences on cause-specific (CSS) and overall survival (OS). METHODS: We conducted a retrospective cohort study using the SEER database (2024 submission, 17 registries) a
