[Markers of connective tissue dysplasia in patients with idiopathic atrioventricular valve prolapse and anomalously sited chordae tendineae].

Phenotype was investigated in 86 patients with idiopathic prolapses of atrioventricular valves and in 21 patients with displaced chordae. 10 and 4 markers of connective tissue dysplasia were distinguished for patients with the prolapses and anomalous chordae, respectively. For patients with dysplasia of connective tissue of the heart a modified version of phenotypical length is proposed: norm-anomalous chordae--prolapse of the 1st degree--prolapse of the 2nd degree. For patients with 3 and more markers echo-CG registration is advisable to reject mitral and/or tricuspid valves.

[Successful repair of tricuspid regurgitation due to blunt trauma].

A 19-year-old man developed paralysis of the left arm as a result of left brachial plexus injury by a traffic accident. He underwent operation for the brachial plexus paralysis, and then severe heart failure developed postoperatively. Echocardiography revealed severe tricuspid valve regurgitation. Tricuspid valve plasty was performed 14 months after the traffic accident. The anterior leaflet of the tricuspid valve was torn and the chordae attached there were torn as well. The torn anterior leaflet was sutured directly, and the prolapsed portion of this leaflet was collected by transfer of the

Echocardiographic findings in Uhl's anomaly. Demonstration of diastolic pulmonary valve opening.

The echocardiographic findings in a case of Uhl's anomaly, or congenital hypoplasia of the right ventricular myocardium, are reported. Diastolic opening of the pulmonary valve is described. Comparison is made with echocardiograms in Ebstein's disease of the tricuspid valve, and it is suggested that echocardiography can help in differentiating these two entities. In addition to the pulmonary valve finding, increased right ventricular dimension, delayed tricuspid closure and abnormality (prolapse) of the mitral valve were noted. The echocardiographic findings are compared with cardiac catheteriz

Iatrogenic Tricuspid Regurgitation Associated With Pacemakers: A Case Compounded by Tricuspid Stenosis.

A patient presented with severe tricuspid regurgitation 20 years after dual-chamber pacing. Transesophageal echocardiography suggested ventricular pacing wire adherence to the tricuspid valve (TV) and atrial wire prolapse across the tricuspid annulus. Surgical extraction of the pacing wires revealed TV commissural fusion and subvalvular thickening causing tricuspid stenosis, requiring TV replacement. (Level of Difficulty: Intermediate.).

Severe tricuspid regurgitation with chordae tendinae rupture in CABG surgery.

A 58-year-old male patient was admitted with chest pain and was diagnosed with coronary heart disease. He was scheduled for coronary artery bypass grafting (CABG) under cardiopulmonary bypass (CPB). Intraoperative real-time transesophageal echocardiography (TEE) showed that the tricuspid valves were well-aligned and subtle regurgitation. Real-time TEE after separation from CPB showed severe tricuspid regurgitation and prolapsed chordae tendineae. The tricuspid chordae tendineae rupture due to a right atrial venous return cannula. The use of negative pressure to improve venous drainage during C

Edge-to-edge repair with ring annuloplasty for post-transplant biopsy-related tricuspid regurgitation.

We experienced an early biopsy-related tricuspid regurgitation post-transplant. Transthoracic and transoesophageal echocardiography showed a frail anterior leaflet with a possible ruptured chorda. The tricuspid valve was inspected, and a ruptured chorda was noted in the anterior leaflet with a single torn chorda on the septal leaflet. The corresponding papillary muscle was difficult to identify. The patient underwent successful tricuspid valve repair with edge-to-edge repair and ring annuloplasty. Compared to a complex repair procedure such as the replacement of multiple artificial chordae, th

Real-time 3-dimensional echocardiography provides new insight into mechanisms of tricuspid valve regurgitation in patients with hypoplastic left heart syndrome.

BACKGROUND: Tricuspid regurgitation in hypoplastic left heart syndrome has an impact on outcome, but its mechanisms remain unclear. METHODS AND RESULTS: Real-time 3-dimensional echocardiography was performed in 35 patients with hypoplastic left heart syndrome (age, 1 month to 10 years; 10 after first-stage Norwood, 12 after superior cavopulmonary shunt, 13 after Fontan). From the 3-dimensional data set, we marked the annulus in systole and diastole. At mid systole, we marked the location of the papillary muscle tip and point of chordal attachment to the leaflet. We traced the surfaces of the

Significance of Effective Height and Mechanism of Regurgitation in Tricuspid Aortic Valve Repair.

BACKGROUND: Isolated repair of the regurgitant tricuspid aortic valve has become an increasingly practiced alternative to replacement, even though durability data are scarce. We analyzed the midterm results of tricuspid aortic valve repair to determine whether the mechanism of regurgitation or operative technique influences the results. METHODS: Between December 1997 and August 2014, 264 patients underwent isolated tricuspid aortic valve repair in our institution. The mean age was 59 ± 16 years; 77% (n = 203) were male. Clinical and operative data were recorded. The patients were observed cli

When should prophylactic maze procedure be considered in patients undergoing mitral valve surgery?

BACKGROUND: Some patients are at high risk for late atrial fibrillation (AF) after mitral valve surgery, even without a prior history of arrhythmias. We examined the incidence and predictors of late AF in patients with functional tricuspid regurgitation (TR) undergoing mitral valve repair. METHODS: From March 1995 to December 2005, 573 patients (375 men) with severe mitral regurgitation owing to leaflet prolapse and functional TR underwent mitral valve repair; 75 patients (13%) had greater than mild TR preoperatively. We excluded patients with preoperative AF or other cardiac disease. We asse

[Cardiovascular aspects of Martin-Bell syndrome].

Cardiovascular anomalies have been studied in 13 subjects (8 males and 5 females, average age 15 +/- 7 years) affected from fragile X syndrome. This group has been examined by standard-ECG, Holter-ECG, echocardiography (M-mode, B-mode, Doppler and color-Doppler). The results have been compared with a control group of 39 subjects (20 males and 19 females, average age 15 +/- 5 years), with non genetic mental retardation. Clinical examination, ECG and Holter did not show any significant pathological alteration compared with the results of the control group. In the study group echocardiography sho

Tricuspid valve repair concomitant with the Norwood operation among babies with hypoplastic left heart syndrome.

OBJECTIVES: Among patients with hypoplastic left heart syndrome (HLHS), tricuspid valve regurgitation (TR) portends a poor prognosis. Our goal was to describe the outcomes of tricuspid valve reconstruction (TVR) concomitant with the Norwood operation and using two-dimensional echocardiography and evaluate the structural factors associated with successful functional correction. METHODS: We performed a retrospective, single-centre study of patients with HLHS undergoing TVR at the time of the Norwood operation. Structural echocardiographic parameters were compared between patients with successfu

The other side of the coin in primary tricuspid valve disease: The incremental value of 3D echocardiography.

Primary tricuspid valve regurgitation may be encountered in daily practice as a result of multiple etiologies. Described herein are the cases of 2 patients with severe primary tricuspid regurgitation. The underlying mechanism was posterior leaflet prolapse due to spontaneous chordae rupture in 1 case, and iatrogenic posterior leaflet tissue loss during removal of a permanent pacemaker in the other. Transthoracic and transesophageal echocardiography, which permit assessment of the tricuspid valve with multilevel imaging, are the techniques of choice for accurate detection and understanding of t

Free margin length and geometric height in aortic root dilatation and leaflet prolapse: implications for aortic valve repair surgery.

OBJECTIVES: Our goal was to assess the aortic leaflet free margin length (FML) and geometric height (gH) in a normal aortic valve (AV), aorta dilatation and aortic leaflet prolapse. METHODS: We measured the FML and gH intraoperatively in 132 patients operated on for aortic insufficiency, aortic dilatation, endocarditis or fibroelastoma. Patients were divided into 3 groups: normal tricuspid AV (group 1, n = 12), aortic dilatation (group 2, tricuspid = 43, bicuspid = 18) and leaflet prolapse (group 3, tricuspid = 32, bicuspid = 27). The FML and gH were compared between the groups and between th

Echocardiographic determination of the prevalence of primary myxomatous degeneration of the cardiac valves.

BACKGROUND: There is a paucity of data on the prevalence of primary myxomatous degeneration (PMD) of the cardiac valves. Because the gold standard for the diagnosis is pathology, its preoperative detection rate is relatively low. The purposes of this study were to determine the capability of echocardiography to identify PMD compared with surgical pathological findings and to determine the echocardiographic features of PMD. METHODS: Echocardiograms were retrospectively compared with pathologic findings in 1,080 patients undergoing surgery for moderate or severe cardiac valve regurgitation. PMD

Development of valve dysfunction in adolescents and young adults with Down syndrome and no known congenital heart disease.

We performed examinations and echocardiographic studies in 35 patients with Down syndrome (aged 20 +/- 4.2 years) with no known intracardiac disease. Sixteen patients (46%) had mitral valve prolapse; two of these also had tricuspid valve prolapse. Two had aortic regurgitation. Valve regurgitation was present in 4 (17%) of 23 patients more than 18 years of age but in none of the 12 patients 18 years of age or younger. We recommend screening of adolescent and young adult patients with Down syndrome for the development of valve dysfunction, especially before dental or surgical procedures.

Echocardiographic evaluation of tricuspid valve endocarditis: an M mode and two dimensional study.

Standard M mode echocardiography and a new real time two dimensional echocardiographic imaging system were utilized to follow the progressive anatomic destruction of the tricuspid valve in a patient with bacterial endocarditis. The initial two dimensional echocardiographic study revealed large vegetative masses attached to severely prolapsing tricuspid leaflets. Serial studies demonstrated eventual disruption of the chordal attachments of the anterior tricuspid leaflet resulting in frank leaflet flail. This technique was seen to complement both M mode echocardiography and cardiac angiography b

Value and limitations of color Doppler flow mapping in the detection and semiquantification of valvular regurgitation.

We compared color Doppler flow mapping data to angiographic data in 294 patients with suspected valvular regurgitation. Thirty-one patients had rheumatic mitral regurgitation and 37 had mitral regurgitation due to mitral valve prolapse by angiography. Ten patients had no angiographic regurgitation (4 rheumatic, 6 prolapse). The remaining patients included 86 with suspected aortic regurgitation and 130 with suspected tricuspid regurgitation. Angiographically 74 had aortic regurgitation and 111 tricuspid regurgitation. The maximum size of regurgitant jets was evaluated in each patient by color f

Secundum atrial septal defect repair: long-term surgical outcome and the problem of late mitral regurgitation.

This study examines the clinical and surgical outcome of a group of 55 patients (mean age 33 years) with secundum atrial septal defect who underwent surgical repair of this defect between 1981 and 1990. A group of 25 of these patients underwent late echocardiographic follow-up. Fifty-two patients underwent repair by direct suturing and three by patch closure. Surgical mortality was nil. There was one late death of a 58 year old who died from cardiac failure 4 years after surgery. Late postoperative morbidity consisted of two patients; one, age 63 at the time of surgery, required mitral and tri
