Outlook for Tissue Engineering of the Tympanic Membrane.

Tympanic membrane perforation is a common problem leading to hearing loss. Despite the autoregenerative activity of the eardrum, chronic perforations require surgery using different materials, from autologous tissue - fascia, cartilage, fat or perichondrium - to paper patch. However, both, surgical procedures (myringoplasty or tympanoplasty) and the materials employed, have a number of limitations. Therefore, the advances in this field are incorporating the principles of tissue engineering, which includes the use of scaffolds, biomolecules and cells. This discipline allows the development of n

An unusual dark pigmentation on the tympanic membrane.

OBJECTIVE: To report an extremely rare case of dark pigmentation on the tympanic membrane due to alkaptonuria, and to discuss the probable association between this condition and hearing loss. CASE REPORT: A 58-year-old man with alkaptonuria was admitted with tinnitus and hearing loss in both ears. Physical examination showed bluish-black pigmentation on the helixes of both ears and both sclerae. Otoscopic examination revealed dark discolouration of both tympanic membranes. Audiological evaluation revealed mixed high frequency hearing loss in both ears. Tympanometric examination revealed type

Diagnosis and management of the lateralized tympanic membrane.

OBJECTIVE: Lateralization of the tympanic membrane (TM) is associated with significant morbidity. In a series of 14 patients, we make observations to illuminate this condition for the diagnosing and treating physician. STUDY DESIGN: Chart review of 14 consecutively treated patients. METHODS: We analyzed the presenting signs and symptoms, etiology, audiometric data, and operative findings of patients with a lateralized tympanic membrane (TM). RESULTS: The etiology was postsurgical in 13 patients (there were four aural atresia repairs and nine tympanoplasties), with 2 patients having had mult

Tympanic membrane temperature in a patient with vertigo.

Tympanic membrane temperature (TMT) is greatly influenced by the internal carotid artery and the jugular vein. Average human brain temperature is influenced by the differences between the temperature of the internal carotid artery and of the jugular vein. TMT has been used extensively as an indicator of brain temperature. Tympanic membrane temperature was measured in six healthy men (control group) and one patient with vertigo. In the control group, TMT showed considerable circadian changes parallel to the core temperature. In the patient with vertigo, TMT varied widely after the attack of ver

Errors in the assessment of tympanic membrane perforations.

It is commonly stated that operative closure of large perforations of the tympanic membrane is less successful than closure of small perforations. Few authors mention the exact size of the perforations studied, and fewer still give their method of measuring perforations. We describe a photographic method of assessing the size of perforations relative to the total drumhead, measuring them with a computerized digitizing pad. Thirteen temporal bones were selected at random. Perforations of different sizes, shapes and at different sites were fashioned in the tympanic membranes. These were assessed

EMLA in local anaesthesia of the tympanic membrane.

An ideal agent for local anaesthesia of the tympanic membrane has been missing so far. Recently, however, a eutectic mixture of lignocaine and prilocaine (EMLA, Astra, Södertälje, Sweden) has proved promising. We compared the anaesthetizing efficacy of EMLA, Bonain's solution (cocain, menthol, phenol ana partes) and Xylocain-spray (Astra, Södertälje, Sweden) in 42 voluntary subjects. EMLA was applied on one tympanic membrane and either one of the other two agents in the other ear of each subject. Small cotton pledgets were used for application. Sensitivity of the ear drum was tested under otom

Role of the mastoid in tympanic membrane reconstruction.

Otologists have long debated the importance of the mastoid in determining the success or failure of tympanic membrane reconstruction. The pneumatic spaces within the mastoid represent an "air reservoir" which can be drawn upon during periods of eustachian tube dysfunction and buffer the middle ear against the development of detrimental negative pressures. Mastoid inflammatory disease, if untreated, may result in recurrent suppuration and graft failure. Small mastoid volume, aside from its well known association with chronic infectious middle ear disease, has been shown to effect adversely graf

The use of sclera in tympanic membrane reconstruction.

Sclera appears to be an appropriate grafting material because of its high density, high tensile strength, and low antigenicity. Preserved donor sclera has been used for myringoplasty in 125 cases over a 4 year period with an overall success rate for complete closure of the perforation of 96%. Several simple techniques for creating flanges to hold the graft in place were used. The size of the perforations ranged from several millimeters to 80% of the eardrum surface. Most of the cases have been done as outpatients, the adults with local anesthesia and the children with general anesthesia. The a

Assessment of tympanometry abnormalities of the tympanic membrane.

Isolated abnormalities of the tympanic membrane have been postulated to affect tympanometry. The authors tested 33 abnormal tympanic membranes in 19 patients with tympanosclerosis and/or thin membranes (having no middle or fibrous layer) against a control group of 32 normal ears in 16 age- and sex-matched patients. All selected patients were free of middle ear disease on the basis of normal hearing, normal pneumatic otoscopic findings, and the presence of crossed acoustic reflexes or normal findings at paracentesis. All patients in the control group had normal type A tympanograms, while 20 (61

Hearing loss in perforations of the tympanic membrane.

70 patients with dry central perforations have been studied to assess the hearing loss. The hearing loss is greater at the lower frequencies and increases with the size of the perforation. Malleolar perforations cause more hearing loss than the non-malleolar unless the perforation involves less than 10 per cent of the tympanic membrane surface area. Perforations of the postero-inferior quadrant cause more hearing loss than those in the antero-inferior quadrant.

Artificial intelligence to detect tympanic membrane perforations.

OBJECTIVE: To explore the feasibility of constructing a proof-of-concept artificial intelligence algorithm to detect tympanic membrane perforations, for future application in under-resourced rural settings. METHODS: A retrospective review was conducted of otoscopic images analysed using transfer learning with Google's Inception-V3 convolutional neural network architecture. The 'gold standard' 'ground truth' was defined by otolaryngologists. Perforation size was categorised as less than one-third (small), one-third to two-thirds (medium), or more than two-thirds (large) of the total tympanic m

Success of tympanic membrane repair following palatoplasty.

INTRODUCTION: Eustachian tube dysfunction, ubiquitous in patients with cleft palate, is a known risk factor for failure of tympanic membrane (TM) repair. Age may also influence the success rate. We describe a cohort of patients with cleft palate who underwent TM repair at a tertiary academic medical center. METHODS: Record query of International Classification of Diseases, Ninth Revision and Current Procedural Terminology codes for cleft palate and otologic procedures (myringoplasty, tympanoplasty, mastoidectomy, and/or ossicular chain reconstruction) was performed for patients treated betwee

Animal Model of Chronic Tympanic Membrane Perforation.

Chronic tympanic membrane perforations (TMP) can be a source of significant morbidity from hearing loss, recurrent middle ear infections, changes in lifestyle, and risk of cholesteatoma formation. Laboratory experiments of TMP have been fraught by the rapid and high rate of spontaneous healing observed in animal models. There is controversy on the minimal time that perforations in animal models must have in order to be considered chronic TMP and thus have clinical relevance, with authors suggesting time periods of perforation patency of 8-12 weeks. In this article, we sought to create a clinic

Hemorrhage within the tympanic membrane without perforation.

BACKGROUND: Hemotympanum refers to both the presence of blood in the middle ear cavity and to ecchymosis of the tympanic membrane (TM), and a systematic study of intra-TM (iTM) hemorrhage without bleeding in the middle ear cavity has not been conducted. The goals of our study were to analyze the causes of iTM hemorrhage without TM perforation or bleeding in the middle ear cavity, and to demonstrate the clinical characteristics of the disease. METHODS: This Case series study included five patients with iTM hemorrhage between August 2014 and August 2017. An iTM hemorrhage was diagnosed when oto
