Primary Ulnar Collateral Ligament Reconstruction in Major League Baseball Pitchers: An Analysis of Factors Associated With Successful Return to Preinjury Level of Performance.

BACKGROUND: An increasing number of Major League Baseball (MLB) pitchers are receiving ulnar collateral ligament reconstruction (UCLR). However, a substantial proportion of pitchers do not regain their preinjury level of performance. Currently, limited research has investigated the factors associated with return to level (RTL) after UCLR. HYPOTHESES: The authors hypothesized that (1) pitchers whose performance was primarily dependent on fastball effectiveness may face greater challenges in regaining their prior form, (2) those with a history of high workload were more likely to encounter diff

Lateral ulnar collateral ligament reconstruction enhancing secondary stabilizers in chronic PLRI of the elbow provides good to excellent clinical results with no recurrent instability.

PURPOSE: The adopted treatment for chronic elbow PLRI is lateral ulnar collateral ligament reconstruction. However, the most frequently reported complication after primary reconstruction is recurrent instability - up to 25 %. It was thus hypothesized that enhancing the secondary stabilizers will provide successful results with a lower rate of recurrent instability in comparison to techniques with primary reconstruction only. This study aimed to demonstrate a novel surgical technique involving secondary stabilizers. METHODS: In this retrospective study of 29 cases with chronic PLRI the mean du

Retrospective Analysis of Ulnar Collateral Ligament Reconstructions in Major League Baseball Pitchers: A Comparison of the "Tall and Fall" Versus "Drop and Drive" Pitching Styles.

BACKGROUND: Previous pilot research has investigated differences in elbow valgus torque between the "tall and fall" (TF) and "drop and drive" (DD) pitching styles. Whether one of these pitching styles is associated with a greater rate of ulnar collateral ligament reconstruction (UCLR) is currently unknown. PURPOSE: To determine the proportion of Major League Baseball (MLB) pitchers using the TF and DD pitching styles who underwent UCLR over a 10-year period. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: The demographic characteristics of pitchers who underwent UCLR bet

Side of Hamstring Harvest Does Not Affect Performance, Return-to-Sport Rate, or Future Hamstring Injuries After Ulnar Collateral Ligament Reconstruction Among Professional Baseball Pitchers.

BACKGROUND: Ulnar collateral ligament reconstruction (UCLR) is a successful procedure among professional baseball pitchers. It is unclear if hamstring tendon harvest side for UCLR affects the outcome or alters the risk for subsequent hamstring injury. HYPOTHESIS: Players with prior UCLR with ipsilateral (drive leg) hamstring autograft will have the same return-to-sport (RTS) rate and performance upon RTS but a higher number of subsequent lower extremity injuries than those with contralateral (landing leg) hamstring autograft. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: All pl

Effect of Ulnar Collateral Ligament Reconstruction on Performance in Major League Baseball Pitchers: A 2-Year Analysis of Advanced Pitching Statistics, Velocity, and Spin Rate.

BACKGROUND: Ulnar collateral ligament reconstruction (UCLR) is a common procedure for Major League Baseball (MLB) pitchers, with a rising incidence and significant financial and performance implications. However, the effect of UCLR on postinjury pitching performance remains controversial, particularly regarding advanced metrics such as spin rate and velocity. PURPOSE: To evaluate changes in pitching performance after UCLR and the association between pitch velocity, spin rate, and performance. STUDY DESIGN: Case-control study; Level of evidence, 4. METHODS: A total of 43 MLB pitchers who und

Routine diagnostic arthroscopy with elbow ulnar collateral ligament reconstruction does not reduce the need for future valgus extension overload-related surgeries: a systematic review and meta-analysis.

BACKGROUND: Valgus extension overload syndrome (VEOS) most commonly affects overhead athletes and consists of a constellation of conditions involving the medial, posterior, and lateral elbow, with the most widely discussed being ulnar collateral ligament (UCL) injuries. Many athletes with UCL tears also have findings consistent with other VEOS conditions, though these are not consistently symptomatic. Given the high rate of concomitant pathology, many authors have recommended performing arthroscopy at the time of UCL reconstruction (UCLR) to diagnose and address concomitant VEOS pathology; how

Clinical follow-up of professional baseball players undergoing ulnar collateral ligament reconstruction using the new Kerlan-Jobe Orthopaedic Clinic overhead athlete shoulder and elbow score (KJOC Score).

BACKGROUND: There are no validated outcome measures consistently used in the literature to report results of ulnar collateral ligament reconstruction in overhead athletes. HYPOTHESIS: The Kerlan-Jobe Orthopaedic Clinic Overhead Athlete Shoulder and Elbow score (KJOC score) will correlate with other validated scores for upper extremity assessment but will be more accurate in evaluating ulnar collateral ligament reconstruction outcomes in professional baseball players. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 2. METHODS: Fifty-five professional baseball players who underwent

Evolution of the Surgical Treatment of Ulnar Collateral Ligament Injuries.

Since Dr. Frank Jobe performed the initial surgery on Tommy John in 1974, the ulnar collateral ligament (UCL) reconstruction (UCLR), colloquially "Tommy John Surgery," described in 1986 has evolved as the gold standard treatment for UCL tears. The crux of technique modifications involve flexor pronator mass (FPM) management, ulnar nerve transposition (UNT), graft selection, or graft-fixation options. Jobe used a figure-of-8 graft fixation through the cubital tunnel, necessitating FPM elevation and UNT. Although 68% of patients returned to play (RTP), 25% of patients experienced ulnar neuritis,

Combined flexor-pronator mass and ulnar collateral ligament injuries in the elbows of older baseball players.

BACKGROUND: Ulnar collateral ligament reconstruction techniques have afforded baseball players up to a reported 90% return to prior or higher level of play. A subpopulation exists with less impressive clinical outcomes potentially related to the presence of a concomitant flexor-pronator mass injury. HYPOTHESIS/PURPOSE: Combined flexor-pronator and ulnar collateral ligament injuries occur in older players, and results in this group are inferior to those reported for isolated ulnar collateral ligament reconstructions. STUDY DESIGN: Case Series; Level of evidence, 4. METHODS: All baseball play

Clinical outcomes of the DANE TJ technique to treat ulnar collateral ligament insufficiency of the elbow.

BACKGROUND: Many improvements in ulnar collateral ligament reconstruction have been made since Jobe et al first described the procedure. A novel elbow ulnar collateral ligament reconstruction technique that combines interference screw fixation on the ulna with docking of the graft on the humeral side (DANE TJ) has been reported. HYPOTHESIS: Outcomes of ulnar collateral ligament reconstructions performed with the DANE TJ technique are as good as other recently published results of ulnar collateral ligament reconstruction, particularly in cases of insufficient bone stock on the sublime tubercle

A muscle-splitting approach to the ulnar collateral ligament of the elbow. Neuroanatomy and operative technique.

The standard surgical approach for repair or reconstruction of the ulnar collateral ligament of the elbow involves lifting off of the tendon of the common flexor bundle at its origin on the medial epicondyle. However, a more limited muscle-splitting approach may be feasible. A muscle-splitting approach is less traumatic to the flexor-pronator muscle mass, and it could decrease operative time and lessen immediate morbidity after surgery. A proposed muscle-split through the common flexor bundle extends from the medial humeral epicondyle to a point distal to the tubercle of the ulna such that rep

Outcome of elbow surgery in professional baseball players.

We reviewed the records of 72 professional baseball players who underwent arthroscopic or open elbow surgery. The most common diagnoses were posteromedial olecranon osteophyte (65%), ulnar collateral ligament injury (25%), and ulnar neuritis (15%). Intraarticular loose bodies were found in 39% of the patients. Fifty-nine patients (82%) were observed for a minimum of 24 months, with an average of 42 months' followup. Forty-seven players (80%) returned to play for a minimum of one season (73% at the same or higher level of play), and 17% of the players retired initially because of their elbow in

The Utility of Stress Ultrasound in Identifying Risk Factors for Elbow Ulnar Collateral Ligament Tear: A Longitudinal Study of 203 Professional Baseball Players.

BACKGROUND: Injuries to the medial ulnar collateral ligament (UCL) are common among baseball pitchers due to repetitive stress on the soft tissue stabilizers of the elbow during pitching. Dynamic stress ultrasound (SUS) can be used to evaluate the UCL and ulnohumeral joint to identify anatomic risk factors of those who will require UCL reconstruction (UCLR). PURPOSE: To determine whether any adaptive or morphological changes detectable on SUS can predict injury to the UCL in professional baseball pitchers. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A total of 203 professiona

Elbow Ulnar Collateral Ligament Injuries in Throwing Athletes: Diagnosis and Management.

Ulnar collateral ligament (UCL) injuries of the elbow are common in overhead throwing athletes. With throwing, the elbow experiences substantial valgus stress and repetitive microtrauma can lead to injury. Increasing rates of injury among both youth and professional throwers has resulted in a "UCL epidemic." Ulnar collateral ligament reconstruction ("Tommy John Surgery") became a part of the public consciousness after Tommy John returned to professional baseball after a UCL reconstruction with Dr Frank Jobe for what was once considered a career-ending injury. Partial tears and some athletes wi
