Conscious and unconscious brain responses to food and cocaine cues.

Visual presentation of appetitive and negative cues triggers fast responses in the human brain. Here we assessed functional MRI (fMRI) responses to food, cocaine, and neutral cues presented at a subliminal ("unconscious", 33 ms) and supraliminal ("conscious", 750 and 3000 ms) level in healthy, cocaine naïve volunteers. Because there is evidence of circadian variability in reward sensitivity, our second aim was to assess diurnal variability in the brain's reactivity to cues. Sixteen participants completed two randomly ordered fMRI sessions (once 9-11 AM and another 5-7 PM). in which food, cocai

Rescue of a saturation diver, unconscious due to an explosion underwater.

Evolution of "safe diving practices" introduced by the diving industry and regulatory authorities in the North Sea over the past 3 or 4 decades has improved the safety records of oilfield commercial diving considerably. However, accidents still occur. In an accident occurring underwater, the outcome is often fatal. For a diving doctor providing emergency medical advice to the industry, when a diving superintendent calls to say "I have a problem" indicates an emergency and a life and death situation for a diver in water. Making a quick decision after comprehending the situations is important to

No cognitive processing in the unconscious, anesthetic-like, state of sleep.

We review evidence challenging the hypothesis that memories are processed or consolidated in sleep. We argue that the brain is in an unconscious state in sleep, akin to general anesthesia (GA), and hence is incapable of meaningful cognitive processing-the sole purview of waking consciousness. At minimum, the encoding of memories in sleep would require that waking events are faithfully transferred to and reproduced in sleep. Remarkably, however, this has never been demonstrated, as waking experiences are never truly replicated in sleep but rather appear in very altered or distorted forms. Gener

Cerebral Tuberculoma of Right Frontal Lobe in an Unconscious Patient.

Cerebral tuberculoma of the brain are uncommon presentation of tuberculosis (1%). A 17-year female presented to the emergency with a Glasgow Coma Score of (eye-1, verbal-1 and motor-3), 5/15 bilateral fixed pupils with laborious breathing. The radiological features were suggestive of aintracerebral abscess/ glioma. Intraoperatively the brain was very tense and a large, vascular, mass was present that was excised completely. Her histopathology revealed features of tuberculoma and she was started on anti-tubercular treatment. She is discharged home with regular follow-up for the last 10 months.C

Strongyloides stercoralis hyperinfection in an unconscious diabetic patient with dermatomyositis.

A case of Strongyloides stercoralis hyperinfection in a patient with dermatomyositis and diabetes mellitus is herein reported. The case was a 60-year-old female admitted due to watery diarrhea and unconsciousness. She had a 10-year history of chronic immunosuppressive therapy including methotrexate and prednisolone for dermatomyositis. Stool parasitological examination revealed numerous rhabditiform larvae of threadworm "S. stercoralis." Larva in stool sample was characterized by sequencing of mitochondrial DNA. After treatment with ivermectin, the patient recovered without evidence of S. ster

Towards an unconscious neural reinforcement intervention for common fears.

Can "hardwired" physiological fear responses (e.g., for spiders and snakes) be reprogramed unconsciously in the human brain? Currently, exposure therapy is among the most effective treatments for anxiety disorders, but this intervention is subjectively aversive to patients, causing many to drop out of treatment prematurely. Here we introduce a method to bypass the subjective unpleasantness in conscious exposure, by directly pairing monetary reward with unconscious occurrences of decoded representations of naturally feared animals in the brain. To decode physiological fear representations witho

Unconscious selection drove seed enlargement in vegetable crops.

Domesticated grain crops evolved from wild plants under human cultivation, losing natural dispersal mechanisms to become dependent upon humans, and showing changes in a suite of other traits, including increasing seed size. There is tendency for seed enlargement during domestication to be viewed as the result of deliberate selection for large seeds by early farmers. However, like some other domestication traits, large seeds may have evolved through natural selection from the activities of people as they gathered plants from the wild, or brought them into cultivation in anthropogenic settings.

Conscious vision for action versus unconscious vision for action?

David Milner and Melvyn Goodale's dissociation hypothesis is commonly taken to state that there are two functionally specialized cortical streams of visual processing originating in striate (V1) cortex: a dorsal, action-related "unconscious" stream and a ventral, perception-related "conscious" stream. As Milner and Goodale acknowledge, findings from blindsight studies suggest a more sophisticated picture that replaces the distinction between unconscious vision for action and conscious vision for perception with a tripartite division between unconscious vision for action, conscious vision for p

[The role of unconscious emotional-motivational stimuli in heroin addiction].

Eighty patients with heroin addiction, aged from 15 to 20 years (most of them over the age of 16), were examined. The duration of illness was from 6 months to 3 years. The examination was conducted 2-3 weeks after arresting of withdrawal syndrome. In that period, patients had different mental disorders: depressive-dysphoric, neurosis-like, anxiety-hypochondriac, hysteriform and others. The aim of the examination was to find peculiarities of functioning of different cortical regions in the process of appraisal of conscious and unconscious stimuli using the method of visual evoked potentials wit

Unconscious pregnancy fantasies as an underlying dynamism in panic disorder.

Unconscious pregnancy fantasies are outlined as an underlying dynamic organizer to the panic experience in some patients with panic disorder. Detailed case material from the treatment of two childless panic patients, one male and one female (nonpregnant) is presented to illustrate this. A literature review found reports of nine nonpregnant patients, none exposed to a pregnant analyst, in whom these fantasies are described as central. Four of these patients had a psychiatric syndrome consistent with panic and agoraphobia. Some mechanisms that may underlie these connections are explored.

Increased Global and Regional Connectivity in Propofol-induced Unconsciousness: Human Intracranial Electroencephalography Study.

BACKGROUND: The conscious state is maintained through intact communication between brain regions. However, studies on global and regional connectivity changes in unconscious state have been inconsistent. These inconsistencies could arise from unclear definition of unconsciousness, spatial and temporal limitations of neuroimaging modalities, and estimating only single connectivity measure. This study investigated global and regional changes in amplitude and phase-based functional connectivity in propofol-induced unconsciousness, which is widely recognized as unconsciousness. METHODS: Amplitude

Severe Hypernatraemic Dehydration and Unconsciousness in a Care-Dependent Inpatient Treated with Empagliflozin.

A 66-year-old Caucasian male became unconscious 2 weeks after initiation of add-on therapy with empagliflozin for poorly controlled type 2 diabetes mellitus. The inpatient had recently suffered focal pontine stroke, rendering him bedridden and requiring increased nursing care, including assistance with drinking. The patient had received empagliflozin 10 mg once daily for glycaemic control. Investigations revealed hypernatraemia (164 mmol/l), a urine glucose level of 3935 mg/dl, and a creatinine level of 2.1 mg/dl. The patient was diagnosed with severe hypernatraemic dehydration due to iatrogen

Fronto-parietal connectivity is a non-static phenomenon with characteristic changes during unconsciousness.

BACKGROUND: It has been previously shown that loss of consciousness is associated with a breakdown of dominating fronto-parietal feedback connectivity as assessed by electroencephalogram (EEG) recordings. Structure and strength of network connectivity may change over time. Aim of the current study is to investigate cortico-cortical connectivity at different time intervals during consciousness and unconsciousness. For this purpose, EEG symbolic transfer entropy (STEn) was calculated to indicate cortico-cortical information transfer at different transfer times. METHODS: The study was performed

Freud, Sebald and the head of Medusa: The unconscious sense of guilt.

This paper explores Freud's concept of unconscious guilt (Freud, S. 1923. "The Ego and the Id". In The Standard Edition of the Complete Psychological Works of Sigmund Freud, Vol. 19. 1-66), as illuminated in Sebald's masterpiece, Austerlitz (Sebald, W. G. 2001. Austerlitz, Translated by Anthea Bell. London: Penguin Books). Both writers examine the human impulse to look away from suffering, personal and collective, and the guilt inherent both in avoidance and in bearing witness. Sebald, preoccupied with Germany's post-war "self-amnesia" (Sebald, W. G. 2003. On the Natural History of Destruction

The Mystical Unconscious: Holding the Sublime, the Spectral, and the Sacred in Psychoanalysis.

This paper explores the mystical unconscious as a dimension of psychoanalysis that moves beyond psychoanalysis's emphasis on repression and interpretation. Drawing from psychoanalytic insights, mystical traditions, and personal clinical encounters with states of spirit possession, the author argues for a psychoanalytic stance of witnessing. Through concepts such as the mystical vertex, internal third, and the analyst as a double, the author presents psychoanalysis as an art of profound attunement-one that welcomes spectral, sublime, and sacred psychic experiences not as pathology, but as meani

Unconscious affective communication in the formation and transformation of emerging subjectivity.

The author addresses "primitive states of mind", a significant point of current development in contemporary psychoanalysis, with the aim of recognising and elaborating such configurations in the analytic relationship. The article revisits the views of Loewald (1980), who saw minds as organising experiences gradually over the course of development according to different principles and changeable organisational structures which continue to operate simultaneously in adult experience on a continuum that runs from consciousness to unconsciousness. The author uses a clinical example to describe vari

The Poetics of Estrangement and the Cinematic Unconscious in All of Us Strangers.

This essay examines how Andrew Haigh's 2023 film, All of Us Strangers, creates an immersive encounter with the workings of the unconscious mind. The film parallels a psychoanalytic process in its exploration of the central character, Adam, who progressively gains access to his memory, childhood trauma, and inner life. Viewers watch the evolving nature of Adam's psychic experience, voyeuristically participating in his deepening insights. The essay examines the "cinematic unconscious," the narrative and visual strategies that usher spectators into Adam's unconscious, affording a dreamlike experi

Update on the neurodevelopmental theory of depression: is there any 'unconscious code'?

Depression is currently one of the most common psychiatric disorders and the number of patients receiving antidepressant treatment is increasing every year. Therefore, it is essential to understand the underlying mechanisms that are associated with higher prevalence of depression. The main component leading to the change in functioning, in the form of apathy, anhedonia, lack of motivation and sleep disturbances, is stress. This is the factor that in recent decades-due to the civilization speed, dynamic technological development as well as competitiveness and competition in relationships-signif

Imaging the Unconscious "Found Down" Patient in the Emergency Department.

Unconsciousness may be due to severe brain damage or to potentially reversible causes. Noncontrast head computed tomography (CT) helps identify acute ischemic and hemorrhagic lesions as well as certain patterns of toxic encephalopathy. MR imaging plays an important role in the assessment of acutely encephalopathic patients who may show no significant abnormality on CT. This review describes some of the common and infrequent entities that can lead to unconsciousness, including epilepsy and vascular, traumatic, metabolic, and toxic disorders.
