Asymptomatic uterine perforation and IUD migration to the broad ligament: A case report.

RATIONALE: Uterine perforation is a serious complication of intrauterine contraceptive device (IUD) placement. However, as complete uterine perforation and extrauterine migration may remain asymptomatic, thorough localization of the IUD is important prior to reinsertion. PATIENT CONCERNS: A 33-year-old patient who has had 4 IUD insertions, wherein the location of the first IUD (inserted 14 years ago) was not identified prior to reinsertion and replacement of the subsequent three. She presented to hospital with a 6-month history of abdominal pain. Pelvic ultrasonography (US), radiography, hyst

IUD use among parous women and risk of uterine perforation: a secondary analysis.

OBJECTIVE: The objective of the study was to determine if delivery and lactation are risk factors for complete intrauterine device perforations. STUDY DESIGN: We performed a reanalysis of the European Active Surveillance Study on Intra-Uterine Devices data set using complete penetration of the myometrium as the definition for perforation. RESULTS: Of the 61,448 women enrolled (70% levonorgestrel, 30% copper devices), we identified 58 complete perforations, 30 of which occurred in lactating women. Incidence per 1000 insertions was 4.5 (95% confidence interval [CI]: 3.0-6.4) for lactating and

Incarceration of the appendix complicating a uterine perforation following surgical abortion: CT aspects.

A 39-year-old patient was evaluated for pelvic pain and infection following a surgical abortion performed via aspiration curettage. A pelvic computed tomography (CT) revealed an invagination of fat in the uterine wall accompanied by an abscess in the rectouterine pouch. Laparoscopic exploration revealed an incarcerated cecal appendix within the myometrium with necrosis of the appendiceal base as the culprit. A posteriori, CT multiplanar reconstruction allowed identification of the incarcerated appendix within the myometrium. To our knowledge, this CT description is the only case described in t

Conservative management of spontaneous uterine perforation associated with placenta accreta: a case report.

BACKGROUND: Placenta accreta occurring in an unscarred uterus is exceedingly rare. Previous cases of spontaneous uterine perforation associated with placenta accreta were treated with hysterectomy. CASE: A nulliparous woman was clinically diagnosed with placenta accreta when spontaneous vaginal delivery was complicated by postpartum hemorrhage and a retained placenta. Magnetic resonance imaging subsequently revealed focal areas of placenta accreta. Acute-onset abdominal pain and cul-de-sac fluid prompted diagnostic laparoscopy, which revealed a spontaneous uterine perforation in the right pos

Uterine perforation on intrauterine device insertion: is the incidence higher than previously reported?

The objective of the study was to determine the rate of uterine perforation associated with insertion of Multiload Cu375 in 'real-life' clinical use. It was a prospective observational cohort study using Prescription Event Monitoring methodology. There were 17,469 Multiload Cu375 insertions in 16,159 women between 1991 and 2001. The insertions were performed by 1,699 different doctors (mostly general practitioners) in surgeries, clinics, and hospitals throughout New Zealand. The results found 28 reports suggesting complete or partial uterine perforation, giving an incidence of 1.6 per 1000 ins

[Uterine rupture without preexisting caesarean section after perforation of the uterus during evacuation].

Uterine rupture is a serious complication, associated with perinatal and maternal morbidity and mortality. This case report describes uterine rupture in a patient who did not have any previous caesarean section. The patient had acute abdominal pain, hypertonic uterus, blood in the amniotic fluid and abnormal cardiotocographic values. An abruption of the placenta was suspected, and an acute caesarean section was performed during which a rupture in fundus uteri was found. It was later experienced that the patient ten years earlier had suffered a uterine perforation during evacuation after a late

Perforation of transverse colon: a catastrophic complication of uterine artery embolization for fibroids.

We report a case of a 43-year-old woman who underwent uterine artery embolization (UAE) for a symptomatic large fibroid uterus and had spontaneous perforation of the transverse colon 3 months after embolisation with near-fatal consequences. We believe this is the first reported case in the literature of this serious complication of UAE. We briefly review the literature on bowel complications after UAE and discuss lessons to be learned regarding patient selection and postprocedure follow-up.

Uterine perforation by a cystoperitoneal shunt, an unusual cause of recurrent vaginal discharge.

BACKGROUND: We describe the first reported case of uterine perforation by a cystoperitoneal shunt. The mechanism of this unusual complication is unclear. CASE: A 17-year-old patient had a cystoperitoneal shunt for a porencephalic cyst. She presented with recurrent watery vaginal discharge. A pelvic ultrasound examination showed that the uterus had been perforated by the distal tip of the shunt. The cystoperitoneal shunt was converted to a ventriculo-atrial shunt, and the vaginal discharge subsequently resolved. CONCLUSION: The appearance of light and clear vaginal discharge in a patient with

Spontaneous uterine perforation of choriocarcinoma with negative beta-human chorionic gonadotropin after chemotherapy.

OBJECTIVE: To report an extremely rare case of spontaneous uterine perforation of choriocarcinoma with negative beta-human chorionic gonadotropin (β-hCG) post-chemotherapy. CLINICAL PRESENTATION AND INTERVENTION: We present a 35-year-old choriocarcinoma patient whose serial serum β-hCG levels following a fifth course of chemotherapy had been within the normal range, but who developed spontaneous uterine perforation with intra-abdominal hemorrhage after eight courses of combined chemotherapy. The patient then underwent an emergency hysterectomy and survived. CONCLUSION: Patients with persiste

Uterine Perforation as a Complication of the Intrauterine Procedures Causing Omentum Incarceration: A Review.

OBJECTIVE: Omentum involvement resulting from uterine perforation is a rare complication following intrauterine procedures that might require immediate intervention due to severe ischemic consequences. This review examines the prevalence of this complication, risk factors, the mode and timing of diagnosis, the proper management and the outcome. METHODS: A systematic literature search was conducted on PubMed, PubMed Central and Scopus using uterine perforation, D&C, abortion and omentum as keywords. The exclusion criteria included the presence of the uterus or placenta's malignancy and uterine

Conservative laparoscopic approach to a perforated pyomyoma after uterine artery embolization.

We describe a conservative laparoscopic approach to treatment of a perforated pyomyoma after uterine artery embolization. A 36-year-old woman came to our emergency department 8 weeks after undergoing uterine artery embolization. She reported painful abdominal cramps. Physical examination revealed fever, tachycardia, hypotension, abdominal pain with rebound tenderness and pain during bimanual examination. Ultrasonography showed a fundal/subserosal leiomyoma and a moderate amount of fluid in the abdominal cavity. Because the patient desired preservation of the uterus, a laparoscopic approach was

Is lactation a risk factor of IUD- and sterilization-related uterine perforation? A hypothesis.

A recent article reported a high risk of IUD-related uterine perforation in lactating women in the U.S. This article prompted the authors to examine the large international datasets on IUDs and tubal sterilizations collected by Family Health International. The findings are somewhat suggestive of such a positive association in IUD users as well as in women undergoing laparoscopic or minilaparotomy sterilization. More definitive studies are urged.

A Rare Occurrence of Uterine Perforation Following the Dilation and Curettage for Missed Abortion.

Uterine perforation represents an uncommon yet potentially significant complication that may arise during the process of dilation and curettage (D&C), a frequently conducted procedure employed for a variety of gynaecological indications, including missed abortion. This report delineates an atypical case of uterine perforation in a 30-year-old female patient who underwent D&C subsequent to a missed abortion at 10 weeks of gestation and subsequently experienced acute abdominal discomfort accompanied by clinical features of internal haemorrhage shortly after the intervention. Diagnostic imaging c

Scar endometriosis after a laparotomy for uterine perforation as a complication of dilatation and curettage.

BACKGROUND: Ectopic endometriosis is an uncommon disease. Abdominal scar endometriosis is especially rare. CASE: A 38-year-old gravid 1, para 1 woman underwent dilatation and curettage due to an intrauterine infection. During the procedure, uterine perforation occurred and an emergency laparotomy was performed to provide hemostasis and suturing. Three years later, she complained of pain and swelling at the lower end of her abdominal scar during menstruation. A biopsy of the abdominal scar demonstrated endometriosis. The patient elected to receive conservative management rather than a surgical

Fish bone foreign body causing rectal, uterine, and bladder perforation: a rare case report.

Fish bones are one of the most common gastrointestinal foreign bodies resulting from accidental ingestion. While the majority can pass through the gastrointestinal tract via peristalsis, a small proportion may become lodged in the digestive tract, potentially causing injury to the gastrointestinal wall or adjacent tissues. However, visceral perforation remains extremely rare. Preoperative abdominal imaging examinations provide crucial evidence for the diagnosis and localization of fish bone foreign bodies. This article reports a case of a patient presenting with painless hematuria who was init

Incidental Diagnosis of Uterine Perforation During Lower Segment Caesarean Section: A Rare Obstetric Complication.

This case report describes the incidental finding of uterine perforation during lower segment cesarean section (LSCS) in a 30-year-old woman, G2A1 with a history of a missed abortion. The antenatal period of the current pregnancy was uncomplicated. The patient presented at 39 weeks with complaints of leaking. A detailed examination revealed thick meconium, leading to the diagnosis of premature rupture of membranes. In response to the urgency posed by potential fetal distress, an emergency LSCS was performed. Post-delivery, an incidental intraoperative finding revealed a 2 cm circular uterine p
