Pneumomediastinum From Vacuum Disc and Vertebral Trauma.

We present a case of pneumomediastinum associated with an endplate fracture adjacent to a vacuum disc. Pneumomediastinum from trauma can be due to damage to the lungs, airway, or esophagus. In this case, we present a unique complication of the vacuum disc phenomenon in which vertebral injury at the site of a vacuum disc releases gas bubbles into the mediastinum. We believe that compressive forces from the trauma produced a disruption of the annulus fibrosis and forced gas previously sequestered in the intervertebral disc space to escape into the mediastinum.

Vacuum intraoperative specimen mammography: A novel technique.

OBJECTIVE: Intraoperative specimen mammography (ISM) is a diffuse technique that allows surgeons to check specimens immediately after lumpectomy. Although the specimen is slightly compressed, the radiological image can be distorted by tissue overlap, and this may affect the evaluation of tumour borders, resulting in extension of the lumpectomy. As ISM may be less precise due to inadequate compression, a vacuum effect was applied to the specimen to increase the precision of margin detection. STUDY DESIGN: This study was conducted at St. Anna Hospital Breast Unit, Turin, Italy. Women who underw

Vacuum assisted wound closures in gynaecologic surgery.

Vacuum assisted closure and negative pressure (NP) dressings are innovative forms of wound management used in multiple fields of medicine, including surgery. We review here the current literature in this area relevant for the gynaecologist. Negative pressure dressings increase blood flow to the wound, decrease bacterial counts, decrease tension on wound edges, and increase the formation of granulation tissue. Until recently, use of NP dressings has been contraindicated in wounds that have been previously radiated or that have necrotic tissue, an infection, an open fistula, an underlying malign

Surgical vacuum drains: types, uses, and complications.

High- and low-pressure vacuum drains are commonly used after surgical procedures. High-pressure vacuum drains (ie, sealed, closed-circuit systems) are efficient and allow for easy monitoring and safe disposal of the drainage. Low-pressure vacuum drains use gentle pressure to evacuate excess fluid and air, and are easy for patients to manage at home because it is easy to reinstate the vacuum pressure. Perioperative nurses should be able to identify the various types of commonly used drains and their surgical applications. Nurses should know how to care for drains, how to reinstate the vacuum pr

Vacuum-assisted closure of complex perineal wounds.

The Vacuum-assisted Closure device decreases the time to wound healing, thus increasing the deposition of granulation tissue, and decreasing the use of wound care specialists. Perineal wounds present a special challenge. We present four cases of complex perineal wounds in which the Vacuum-assisted Closure device was used. In each case, wound care was simplified and healing accelerated. The Vacuum-assisted Closure device allows earlier wound closure, early skin grafting (with improved graft adherence), earlier hospital discharge, and earlier return to baseline functional status. Its use in the

Vacuum extraction: does duration predict scalp injury?

OBJECTIVE: To identify variables that increase the chance of neonatal scalp injury during vacuum extraction. METHODS: We conducted a prospective observational study of 134 vacuum extraction-assisted deliveries at Olive View-UCLA Medical Center in 1995. Data collected included parity, gestational age, duration of first and second stages of labor, indication for operative delivery, station and position of fetal head, duration of vacuum application, number of "pop-offs," neonatal weight, and descriptions of scalp marks or injury. Cranial imaging studies were obtained if clinically indicated. RE

Intravertebral vacuum cleft: notes on five cases.

Analysis of five personal cases of the intravertebral vacuum cleft phenomenon and a close examination of the literature, suggest that it is rather more frequent than it first appears. Intravertebral vacuum cleft tends to occur more readily in the vertebrae of the dorsolumbar hinge and mainly in the elderly, slightly more common in women than men. The main factors, from the pathogenetic point of view, are osteoporosis, a history of trauma and frequent occurrence of rheumatic problems which are often complicated by vasculitis. The comparison between excessive uptake of calcium-mimetic technetium

Plasma haptoglobin levels in vacuum extracted neonates.

In order to examine the pathogenesis of neonatal hyperbilirubinemia associated with vacuum extraction, we have compared plasma haptoglobin and bilirubin levels and blood hematocrits of vacuum extracted neonates with those of newborns who underwent a normal vaginal delivery. Plasma haptoglobin and blood hematocrits of the 12 vacuum patients measured on the third day of life were significantly lower (21.7 +/- 5.3 vs. 46.5 +/- 7.5 mg%; p value less than 0.01 and 52.3 +/- 1.3 vs. 59.7 +/- 1.8%; p value less than 0.005, respectively) than the 12 control patients (values are mean +/- SEM). Spinal vacuum phenomenon: CT diagnosis and significance.

Spinal vacuum phenomenon may be encountered in many circumstances, such as degeneration of the intervertebral disk, disk injury, vertebral metastases, ischemic vertebral collapse, Schmorl's nodes, and infectious processes. The high spatial and densitometric resolution of computed tomography (CT) makes it possible to show gas in tissues with high frequency than by conventional radiography. In certain circumstances, the CT findings may be the decisive diagnostic element.

Spinal vacuum phenomena: anatomical study and review.

"Vacuum" phenomena relate to the accumulation of gas, principally nitrogen, in crevices within the intervertebral disk or vertebra. Their appearance does not uniformly indicate "degenerative" disk disease (primary intervertebral osteochondrosis), as gaseous collections may accompany other processes (vertebral osteomyelitis, Schmorl node formation, spondylosis deformans, vertebral collapse with osteonecrosis) affecting the disk and adjacent vertebral bodies. The location and appearance of the "vacuum" phenomena are helpful indicators as to the precise nature of the spinal disorder.

[Corneal lesion by vacuum extraction (author's transl)].

The case of a 6-year-old boy is reported who was delivered out of a frontal position by application of the vacuum cup to the left part of the forehead. His left eye exhibits a pronounced somewhat irregular astigmatism. There are some vertical descemet tears. Because of the application of the vacuum pump close to the left bulbus, an increase of the tissue tension and consequently a deformation of the bulbus occurred by the vacuum in the bulbar region. The described lesions parallel the vertical descement tears which occur in forceps-delivery. Owing to the greater horizontal corneal diameter, a

Comparison of customized vacuum sealing drainage and vacuum sealing drainage in the treatment of diabetic foot ulcers: a retrospective analysis.

BACKGROUND: The prevalence of diabetic foot ulcers, a common, more serious chronic diabetes-related complication, is increasing. Vacuum sealing drainage (VSD) constitutes an effective adjunctive treatment for diabetic foot ulcers. Factors, such as poor glycemic control, ischemia, and infection prolong wound healing time, and VSD products are expensive and unaffordable for many patients. OBJECTIVE: To compare the use of customized VSD and customized VSD in the treatment of diabetic foot ulcer. METHOD: This retrospective study included 83 patients with diabetic foot ulcers in customized VSD (n

Neonatal complications in vacuum-assisted vaginal delivery: are they associated with number of pulls, cup detachments, and duration of vacuum application?

PURPOSE: To explore whether the characteristics of vacuum delivery are associated with the occurrence of head injury and neonatal complications. METHODS: Retrospectively cohort study of vacuum-assisted attempted vaginal deliveries of singletons. We studied the association of total duration of vacuum application and number of pulls and cup dislodgement with (1) primary outcome: the occurrence of major (subgaleal hemorrhage, skull fracture, and intracranial hemorrhage) or minor (cephalohematoma, scalp laceration more extensive than simple abrasions) neonatal head injuries and (2) secondary outc

Steam-Vacuum Treatment of Pig Carcass Surfaces.

The aim of this study was to evaluate the efficiency of a steam-vacuum treatment of the surface of industrially slaughtered pig carcasses after evisceration but before chilling. Both the reduction of the microbial load after contamination with stomach contents, bile, or tubular rail fat and color changes of the carcass surface were investigated. Up to 25 samples per type of contamination were examined within an experimental setting under regular slaughterhouse conditions. The steam-vacuum treatment was applied on the approximately 30 cm long carcass rind between 2 and 8 s. This led to a statis

Multiple Intestinal Fistulization Managed With Vacuum Therapy.

Intestinal fistulas are disorders of renewed interest due to their rarity, high morbidity and mortality, and difficult treatment. Their incidence has increased due to the use of surgical procedures that leave the abdomen open because of the difficulty in approximating the wound edges. In the last decade, favorable results from the use of vacuum therapy in the management of fistulas have encouraged more research on the subject. This report aimed to analyze the effects of negative pressure wound therapy applied to a patient with multiple intestinal fistulas. A 47-year-old man with 3 complex post

Vacuum-assisted closure therapy in reconstructive surgery.

In 1997, supported by experimental work, Argenta published a clinical report describing a variety of complicated wounds whose treatment responded successfully to negative pressure dressings using a vacuum-assisted closure system (VAC) (Kinetic Concepts Inc., San Antonio, TX). This system has been successfully used in the fields of orthopaedics and traumatology, general surgery, plastic and reconstructive surgery and gynaecology/obstetrics for a large variety of complicated wounds located in several regions, particularly in the torso and extremities. To the best of our knowledge, the use of the
