Stimulation parameters for directional vagus nerve stimulation.

BACKGROUND: Autonomic nerve stimulation is used as a treatment for a growing number of diseases. We have previously demonstrated that application of efferent vagus nerve stimulation (eVNS) has promising glucose lowering effects in a rat model of type 2 diabetes. This paradigm combines high frequency pulsatile stimulation to block nerve activation in the afferent direction with low frequency stimulation to activate the efferent nerve section. In this study we explored the effects of the parameters for nerve blocking on the ability to inhibit nerve activation in the afferent direction. The overa

Laryngology Outcomes Following Implantable Vagus Nerve Stimulation.

IMPORTANCE: Vagus nerve stimulation (VNS) devices have gained widespread acceptance for treatment of resistant epilepsy and depression. The increasing number of procedures has resulted in an increasing number of iatrogenic injuries to the vagus nerve, which can have a significant effect on vocalization and quality of life. OBJECTIVE: To determine the relative frequency of laryngeal adverse effects reported to the US Food and Drug Administration (FDA) after VNS implantation and to analyze associated VNS device problems. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cross-sectional ana

Neurosarcoidosis Directly Involving the Cervical Vagus Nerve.

OBJECTIVES: To present a novel location in which neurosarcoidomatous inflammation is identified and its accompanying presentation. METHODS: The authors present a case of bilateral vocal fold paresis associated with non-caseating granulomatous inflammation of the cervical and intra-axial portions of the vagus nerve masquerading as a cranial nerve tumor. RESULTS: Examination revealed bilateral vocal fold paresis and asymmetric palate elevation. MRI demonstrated enhancing bilateral jugular foramen masses, and neck ultrasound demonstrated bilateral thickened appearance of the vagus nerves. Vagus

Vagus nerve stimulation for drug-resistant epilepsy.

Vagus nerve stimulation (VNS) is a neuromodulatory therapeutic option for drug-resistant epilepsy. In randomised controlled trials, VNS implantation has resulted in over 50% reduction in seizure frequency in 26%-40% of patients within 1 year. Long-term uncontrolled studies suggest better responses to VNS over time; however, the assessment of other potential predictive factors has led to contradictory results. Although initially designed for managing focal seizures, its use has been extended to other forms of drug-resistant epilepsy. In this review, we discuss the evidence supporting the use of

Unilateral vagus nerve lesion revealing clival meningocele.

INTRODUCTION: Cranial nerve lesions can be secondary to a space-occupying lesion of the skull base compressing adjacent nerves. CASE REPORT: We report the case of an 84-year-old man, who presented with rapid and concomitant onset of dysphagia and ipsilateral recurrent laryngeal nerve paralysis, suggesting an isolated lesion of the vagus nerve. MRI revealed a diagnosis of previously unknown clival meningocele. DISCUSSION: Unilateral vagus nerve paralysis constitutes an exceptional mode of presentation of meningocele. Only a few isolated cases of clival meningocele have been reported to date,

Revision surgeries following vagus nerve stimulator implantation.

The vagus nerve stimulator (VNS) has been shown to provide a safe, albeit costly, treatment for intractable epilepsy. We aimed to analyze the incidence, timing, and clinical/demographic associations of revision surgery post-VNS implantation in epilepsy patients. The Thomson Reuters MarketScan database, containing data from 23-50million individuals, was used. Epilepsy patients receiving VNS implantations from 2003 to 2009 were identified by Current Procedural Terminology and International Classification Of Diseases Ninth Revision codes. Incidence and timing of subsequent implant-related surgeri

Enhancing Rehabilitative Therapies with Vagus Nerve Stimulation.

Pathological neural activity could be treated by directing specific plasticity to renormalize circuits and restore function. Rehabilitative therapies aim to promote adaptive circuit changes after neurological disease or injury, but insufficient or maladaptive plasticity often prevents a full recovery. The development of adjunctive strategies that broadly support plasticity to facilitate the benefits of rehabilitative interventions has the potential to improve treatment of a wide range of neurological disorders. Recently, stimulation of the vagus nerve in conjunction with rehabilitation has eme

Ancient schwannoma of vagus nerve mimicking hamartoma.

Schwannomas arising from vagus nerve sheath are rare mediastinal neurogenic tumours. Schwannomas usually arise from left hemithorax. Unlike a hamartoma, radiologically, calcification is rarely seen in schwannomas. We present the rare case of an ancient schwannoma arising from vagus nerve sheath from the right hemithorax presenting with gross calcification.

Vagus nerve stimulation in Lafora body disease.

INTRODUCTION: Lafora body disease (LBD) is a rare autosomal recessive disorder characterized by progression to inexorable dementia and frequent occipital seizures, in addition to myoclonus and generalized tonic-clonic seizures (GTCSs). It belongs to the group of progressive myoclonus epilepsies (PMEs), rare inherited neurodegenerative diseases with great clinical and genetic differences, as well as poor prognosis. Since those patients have a pharmacoresistant disease, an adjunctive treatment option is vagus nerve stimulation (VNS). To date, there are four reported cases of the utility of VNS i

Vagus nerve stimulation: mood and cognitive effects.

Many antiepileptic medications modulate affective and cognitive functions. In keeping with these observations, a growing body of literature suggests that vagus nerve stimulation (VNS) may have similar effects. This review evaluates the published evidence for VNS-associated changes in mood and cognition in patients with neuropsychiatric disorders.

[Vagus nerve stimulation and refractory partial epilepsies].

Since 1988, intermittent electric stimulation of the cervical portion of the left vagus nerve is proposed as additive treatment of epilepsy. The effects of vagus nerve stimulation (VNS) on the modulation of cerebral activity, shown in animals and confirmed by the data of functional imagery in human beings, can be apprehended by the anatomic and functional characteristics of this nerve, whereas the anti-epileptic mode of action remains unknown. Following five controlled multicentric studies, including 440 patients presenting refractory epilepsy, 20,000 patients worldwide have been treated by VN

Vagus nerve stimulation therapy: a research update.

Over the past 5 years, and especially within the last year, there has been a rapid expansion of vagus nerve stimulation (VNS)-related preclinical research, as well as clinical studies in indications other than epilepsy. The research advances in understanding VNS are occurring in the midst of a blossoming of other forms of therapeutic brain stimulation, such as electroconvulsive therapy (ECT), transcranial magnetic stimulation (TMS), and deep brain stimulation (DBS). In general, improved understanding of the neurobiological effects of VNS therapy as a function of the different use parameters (f

MRI-Only Preoperative Localization and Semiautomated Three-Dimensional Modeling of the Cervical Vagus Nerve for Vagus Nerve Stimulation Implantation.

BACKGROUND: We evaluated whether three-dimensional steady-state free precession with time-spatial labeling inversion pulse (3D-SSFP with time-SLIP) MRI could localize the cervical vagus nerve before vagus nerve stimulation (VNS) implantation and developed training-data-free semiautomated vagus nerve tracking software. METHODS: Thirty patients with drug-resistant epilepsy underwent preoperative cervical 3D-SSFP with time-SLIP MRI followed by VNS implantation. The left vagus nerve was visually identified at the planned surgical level, and its relationship to the common carotid artery (CCA) and

Sympathetic nerve fibers in human cervical and thoracic vagus nerves.

BACKGROUND: Vagus nerve stimulation (VNS) therapy has been used for chronic heart failure and is believed to improve imbalance of autonomic control by increasing parasympathetic activity. Although it is known that there is neural communication between the VN and the cervical sympathetic trunk, there are few data regarding the quantity and/or distribution of the sympathetic components within the vagus nerve (VN). OBJECTIVE: To examine the sympathetic components within the human VN and correlate them with the presence of cardiac and neurologic diseases. METHODS: We performed immunohistochemist

Effect of aging on vagus somatosensory evoked potentials and ultrasonographic parameters of the vagus nerve.

Vagus somatosensory evoked potentials (VSEP) and ultrasonography can be used to detect functional and structural changes of the vagus nerve (VN) that are hypothesized to be associated with neurodegenerative diseases. However, it has not yet been established whether age-related changes in the VN occur in the healthy population. In this pilot study we included healthy volunteers in the 26-30 and 51-55 age range who comprised the younger (n = 20) and older (n = 20) groups, respectively. VSEP were recorded separately for stimulation of the auricular branch of the left and right VN. The VN CSA was

Extraction of Evoked Compound Nerve Action Potentials from Vagus Nerve Recordings.

Cervical vagus nerve stimulation (VNS) is a neuromodulation therapy for the treatment of several chronic disorders. The effects of VNS are mediated by activation of nerve fibers of different types. In order to maximize the desired and minimize the undesired effects of VNS, assessing activation of vagal fiber types by VNS is essential. Evoked compound nerve action potentials (CNAPs) are commonly used as a method to estimate vagal fiber activation in the context of neurostimulation. However, vagal CNAPs are frequently contaminated by signals from non-neural sources, like electrocardiography (ECG

Selective vagus-recurrent laryngeal nerve anastomosis guided by intraoperative neuromonitoring: evidence of lateral motor fiber clustering in the vagus nerve.

OBJECTIVE: Direct anastomosis (DA) is the standard approach after recurrent laryngeal nerve (RLN) transection but is often not feasible due to excessive tension. This experimental study evaluated a novel intraoperative neuromonitoring (IONM)-guided selective vagus-recurrent laryngeal nerve anastomosis (SVRA) technique and compared its immediate electrophysiologic performance with DA in a porcine thyroid surgery model. METHODS: 18 transected nerves from 9 pigs were randomized to DA or SVRA (9 nerves per group). In the SVRA group, low-current IONM was used to map vagus nerve (VN) motor fibers i
