Study protocol for evaluating the effectiveness of depression management on gylcaemic control in non-communicable diseases clinics in Malawi.

INTRODUCTION: Depression is associated with negative patient outcomes for chronic diseases and likely affects consistent physical non-communicable diseases (NCDs) care management in relation to clinic attendance and medication adherence. We found no published studies on the integration of depression management in physical NCD clinics in Malawi and assessing its effects on patient and service outcomes. Therefore, the aim of this study is to evaluate the effectiveness of integrating depression screening and management in physical NCD routine care on patient and service outcomes in Malawi. We wil

Detection and elimination profile of cathinone in equine after norephedrine (Propalin®) administration using a validated liquid chromatography-tandem mass spectrometry method.

Cathinone is the principal psychostimulant present in the leaves of khat shrub, which are widely used in East Africa and the Arab peninsula as an amphetamine-like stimulant. Cathinone readily undergoes metabolism in vivo to form less potent cathine and norephedrine as the metabolites. However, the presence of cathine and norephedrine in biological fluids cannot be used as an indicator of cathinone administration. The metabolism of pseudoephedrine and ephedrine, commonly used in cold and allergy medications, also produces cathine and norephedrine, respectively, as the metabolites. Besides, cath

Apparent diffusion coefficient from magnetic resonance imaging as a biomarker in oncology drug development.

Magnetic resonance imaging (MRI) can be made sensitive to diffusion of water molecules in biological tissues: this phenomenon can be quantitated to provide a biomarker, the apparent diffusion coefficient (ADC). Over the past decade, evidence has accumulated from numerous clinical and animal studies that ADC is abnormal in tumours; that elevated ADC reflects an elevated non-cellular fraction; and that acute increases in ADC following therapy can indicate that tumour cells have been killed. However there remain substantial challenges in ensuring robust and valid ADC measurements, particularly in

Immune reactivity to type VII collagen: implications for gene therapy of recessive dystrophic epidermolysis bullosa.

Recessive dystrophic epidermolysis bullosa (RDEB) is a severe genodermatosis caused by loss-of-function mutations in COL7A1 encoding type VII collagen, the component of anchoring fibrils. As exogenous type VII collagen may elicit a deleterious immune response in RDEB patients during upcoming clinical trials of gene therapies or protein replacement therapies, we developed enzyme-linked immunosorbent assay (ELISA) and enzyme-linked immunosorbent spot (ELISPOT) assays to analyze B- and T-cell responses, to the full-length type VII collagen. The ELISA was highly sensitive and specific when tested

Digital screening tool for the assessment of cognitive impairment in unsupervised setting-digiDEM-SCREEN: study protocol for a validation study.

INTRODUCTION: Dementia is one of the most relevant widespread diseases, with a prevalence of currently 55 million people with dementia worldwide. However, about 60-75% of people with dementia have not yet received a formal diagnosis. Asymptomatic screening of cognitive impairments using neuropsychiatric tests has been proven to efficiently enhance diagnosis rates. Digital screening tools, in particular, provide the advantage of being accessible without spatial or time restrictions. The study aims to validate a digital cognitive screening test (digiDEM-SCREEN) as an app in the German language.

Study protocol to redefine muscle attenuation cut-offs for better prediction of mortality in patients with cirrhosis: a comprehensive post hoc validation study - a study protocol.

INTRODUCTION: Myosteatosis, characterised by altered muscle composition detectable by muscle radiodensity attenuation on CT scans, has been associated with increased mortality in patients with cirrhosis. However, standard attenuation cut-offs, derived primarily from oncology populations, may not be appropriate for patients with cirrhosis. This study protocol aims to address this diagnostic gap by validating the Ebadi cut-offs, which are based on a retrospective cohort and have not been extensively validated in a cirrhotic population. The aim of the study is to refine these cut-offs for more ac

The validity and utility of the Cutaneous Disease Area and Severity Index (CDASI) as a clinical outcome instrument in dermatomyositis: A comprehensive review.

INTRODUCTION: Validated outcome measures are paramount in the assessment of disease progression and evaluation of new therapeutics in clinical trials. Dermatomyositis (DM) is an autoimmune disease that is frequently refractory to current therapies and warrants the development of new treatments. The Cutaneous Disease Area and Severity Index (CDASI) was developed in 2008 in response to a need for a reliable, validated measure of skin disease activity and damage for use in clinical trials and longitudinal monitoring of disease progression. METHODS: The literature was searched for all studies val

Producing a preference-based quality of life measure for people with Duchenne muscular dystrophy: a mixed-methods study protocol.

INTRODUCTION: Preference-based measures (PBMs) of health-related quality of life (HRQoL) are used to generate quality-adjusted life years, which are necessary for cost-effectiveness evaluations of health interventions via cost-utility analysis. These measures of health can be generic (ie, pandiagnostic) or condition specific. No condition-specific PBM of HRQoL in Duchenne muscular dystrophy (DMD) exists, yet there are concerns that standard generic measures lack the specificity to assess aspects of HRQoL that are especially important to people with DMD. This study has been designed to produce

Implementation of a dose-response curve for γ-radiation in the Portuguese population by use of the chromosomal aberration assay.

An in vitro dose-response curve following exposure to γ-radiation was determined at the IST/ITN, by use of the chromosomal aberration assay. This is the first study of this kind carried out among the Portuguese population. Un-irradiated and γ-irradiated peripheral blood lymphocytes from 16 healthy donors were cultured. A total of 22,395 metaphases were analyzed for frequency and distribution of dicentrics and centric rings, as a function of the radiation dose. The dose-response data for dicentrics and dicentrics plus centric rings were fitted by use of a linear-quadratic model: Y(dic)=(0.0011±

[The Alzheimer's disease: how to estimate the impact of the pathology and the treatment?].

The Alzheimer's disease (AD) is a neuro-degenerative pathology which establishes at present the most frequent cause of dementia. The therapeutic which we have actually are symptomatic treatments which do not modify the evolution of the disease. A clinical and cognitive evaluation is sufficient mostly to estimate the efficiency of these symptomatic treatments. The development of treatments to modify the natural evolution of AD (disease modifying) requires the implementation of innovative therapeutic tries with a significant number of subjects, a long period of observation and where the only cli

Clinical validation study to measure the performance of the Nerve Root Sedimentation Sign for the diagnosis of lumbar spinal stenosis.

Lumbar spinal stenosis is a common degenerative disorder of the spine in elderly patients that can be effectively treated with decompression surgery in some patients. Radiological findings in the diagnostic work-up of the patients do not always correlate well with clinical symptoms, and guidance about when to proceed to surgery is inconsistent. The recently described Nerve Root Sedimentation Sign in magnetic resonance scans has been shown to discriminate well between selected patients with and without lumbar spinal stenosis, but the performance of this new test, when used in a broad patient po

Behavioral Characteristics of Medical Students on Social Media Platforms: Topic Modeling and Social Network Analysis.

BACKGROUND: Medical students experience sustained academic, clinical, and psychosocial pressures. Online forums provide an informal space where students seek information, express concerns, and exchange peer support. However, few studies have analyzed these interactions longitudinally based on real-world data from online social platforms. OBJECTIVE: This study aimed to integrate topic modeling, temporal topic analysis, and social network analysis (SNA) to investigate evolving concerns, network positions, and influence structures within the bulletin board system of a medical university over a 7

Test-retest reliability and validity of movement-evoked pain in patients with knee osteoarthritis: a methodological study protocol.

INTRODUCTION: Pain experienced during functional activities, referred to as movement-evoked pain (MEP), is a common and disabling symptom in individuals with knee osteoarthritis (KOA). Unlike pain at rest, MEP may better reflect the real-life burden of KOA and is increasingly recognised as a core outcome in musculoskeletal pain trials. However, its clinical utility remains limited by a lack of evidence on its measurement properties. This study aims to evaluate the test-retest reliability and validity of MEP assessments during functional tasks in individuals with KOA. METHODS AND ANALYSIS: Thi

Explainable Artificial Intelligence for Early Diabetic Kidney Disease Risk Profiling in Type 2 Diabetes: Translational Readiness and Clinical Boundaries.

PURPOSE: Early kidney-risk profiling in type 2 diabetes is difficult because albuminuria, estimated glomerular filtration rate, comorbidity, and care access capture different aspects of diabetic kidney disease. This review examines where explainable artificial intelligence may support clinically defensible kidney-risk profiling. METHODS: This narrative review used structured searches of PubMed, Semantic Scholar, and OpenAlex using kidney, diabetes, and artificial intelligence/explainability terms, supplemented by citation chasing and full-text assessment of key records.
