[29 cases of malaria introduced into New Caledonia from 1992 to 1995].

Although located in a tropical zone of the South Pacific, the island of New Caledonia is malaria-free. This retrospective study of imported malaria was conducted jointly by the Pasteur Institute of New Caledonia and the Gaston Bourret Territorial Hospital between January 1, 1992 and December 31, 1995. A total of 29 patients were hospitalized for malaria. Most contracted the disease in Vanuatu. Plasmodium vivax was involved more often than Plasmodium falciparum (22 vs. 11). No case was severe, complicated, or fatal. In most cases treatment consisted of quinine followed by mefloquine. The median

Challenges and key research questions for yaws eradication.

Yaws is endemic in west Africa, southeast Asia, and the Pacific region. To eradicate yaws by 2020, WHO has launched a campaign of mass treatment with azithromycin. Progress has been made towards achievement of this ambitious goal, including the validation of point-of-care and molecular diagnostic tests and piloting of the strategy in several countries, including Ghana, Vanuatu, and Papua New Guinea. Gaps in knowledge need to be addressed to allow refinement of the eradication strategy. Studies exploring determinants of the spatial distribution of yaws are needed to help with the completion of

An assessment of the telehealth needs and health-care priorities of Tanna Island: a remote, under-served and vulnerable population.

We surveyed eight Canadian physicians who had each provided medical care for six months on the remote and under-served island of Tanna in Vanuatu. The most frequently encountered medical problems on Tanna were infectious diseases (tuberculosis, hepatitis, abscesses, malaria, pneumonia, typhoid fever, meningitis and skin infections). When physicians were asked about the top three health-care priorities, they ranked tuberculosis control, clean water and improved health-care delivery/communication between hospital and outposts as most important. The key issues were: (1) basic public health needs

Urban flooding and risk of leptospirosis; Pakistan on the verge of a new disaster: A call for action.

Leptospirosis is an overlooked zoonotic and waterborne disease that is emerging as a global public threat due to morbidity and mortality observed in both animals and humans. The outbreaks are typically related to floods and hurricanes following monsoon rains, during which Leptospiras are washed off in contaminated soil and often settle in water bodies. Wildlife trapping for scientific purposes, industrial animal employment, water-intensive crop farming, sewage work, and open-water swimming are one of the major risk factors contributing to the rapid spread of disease. Occasionally, outbreaks ar

A review of traditional remedies of ciguatera fish poisoning in the Pacific.

Ciguatera fish poisoning (CFP) is an illness caused by eating tropical coral fish contaminated with ciguatoxins (CTXs). The clinical management of patients with CFP is generally supportive and symptomatic in nature as no antidote exists. Of the many drugs prescribed, several have been claimed to be efficient in small, uncontrolled studies, but the outcomes of treatments with these medicines are often contradictory. In New Caledonia, traditional remedies are commonly employed in the treatment of CFP and of the 90 plant species catalogued as useful in CFP, the most popular herbal remedy by far i

International shipments of Wolbachia-infected mosquito eggs: towards the scaling-up of World Mosquito Program operations.

The Wolbachia insect control method, employed by the World Mosquito Program (WMP), relies on introgressing Wolbachia through target Aedes aegypti populations to reduce the incidence of dengue. Since 2010, the WMP has been producing Wolbachia-infected mosquitoes at numerous sites across the globe for release in 11 countries. As the technology has matured, greater focus has been placed on mosquito production at larger central facilities for transport to remote release sites, both domestically and internationally. Of particular note is the production of Wolbachia-infected mosquitoes at the WMP's

High incidence of malaria in alpha-thalassaemic children.

The alpha+-thalassaemias are the commonest known human genetic disorders, affecting up to 80 per cent of some populations. Although there is good evidence from both epidemiological and clinical studies that these gene frequencies reflect selection by, and protection from, malaria, the mechanism is unknown. We have studied the epidemiology of malaria in childhood on the southwestern Pacific island of Espiritu Santo in Vanuatu and here we report that, paradoxically, both the incidence of uncomplicated malaria and the prevalence of splenomegaly, an index of malaria infection, are significantly hi

Serodiagnosis of human cysticercosis using a chromatofocused antigenic preparation of Taenia solium cysticerci in an enzyme-linked immunosorbent assay (ELISA).

Crude preparations of Taenia solium cysticerci were partially purified by a chromatofocusing procedure to obtain antigenic fractions that were shown to be active in the enzyme-linked immunosorbent assay (ELISA) with known positive cysticercosis sera. Evaluation of this antigen in the ELISA serodiagnosis of human cysticercus infection showed an 80% detection of clinically typical cysticercosis patients from West New Guinea, and gave no false positive results in healthy control subjects from either Papua New Guinea or the USA. Cross-reactivity to sera from a panel of subjects with a variety of o

A missed opportunity: birth registration coverage is lagging behind Bacillus Calmette-Guérin (BCG) immunization coverage and maternal health services utilization in low- and lower middle-income countries.

BACKGROUND: Civil registration and vital statistics (CRVS) systems lay the foundation for good governance by increasing the effectiveness and delivery of public services, providing vital statistics for the planning and monitoring of national development, and protecting fundamental human rights. Birth registration provides legal rights and facilitates access to essential public services such as health care and education. However, more than 110 low- and middle-income countries (LMICs) have deficient CRVS systems, and national birth registration rates continue to fall behind childhood immunizatio

Climate-driven models of leptospirosis dynamics in tropical islands from three oceanic basins.

BACKGROUND: Leptospirosis is a neglected zoonosis which remains poorly known despite its epidemic potential, especially in tropical islands where outdoor lifestyle, vulnerability to invasive reservoir species and hot and rainy climate constitute higher risks for infections. Burden remains poorly documented while outbreaks can easily overflow health systems of these isolated and poorly populated areas. Identification of generic patterns driving leptospirosis dynamics across tropical islands would help understand its epidemiology for better preparedness of communities. In this study, we aim to m

Prevalence of human T-cell lymphotropic virus type I infection in Singapore: a preliminary report.

Human T-cell lymphotropic virus type I (HTLV-I) infection is endemic in southwestern Japan, the Caribbean basin, Colombia, Africa and in several isolated populations in Papua New Guinea, the Solomon Islands and Vanuatu. To determine the seroprevalence of HTLV-I infection in Singapore, we tested sera from 115 hospitalized patients with acute nephritis, 50 patients with suspected leptospirosis, 34 patients with non-A, non-B hepatitis, and from 28 healthy volunteers for IgG antibodies against HTLV-I using an enzyme-linked immunosorbent assay. Antibodies were detected in sera from 6 of the 199 pat

Illicit drug use and responses in six Pacific Island countries.

We report here on the illicit drug situation in six Pacific nations: Fiji, Papua New Guinea, Samoa, Solomon Islands, Tonga and Vanuatu. The report is based on the 'Situational analysis of illicit drug issues and responses in Asia and the Pacific', commissioned by the Australian National Council on Drugs Asia Pacific Drug Issues Committee. The situational analysis was a comprehensive desk-based review; data sources included published and unpublished literature and information from key informants. A range of psychoactive substances have been used traditionally across the Pacific region. Cannabis

Controlling Malaria in Western Pacific with Mosquito Nets Treated with Pyrethroids in Village Communities, 1979-1999.

Insecticide-treated mosquito nets were first put to practical use in the Western Pacific Region. Less than a decade after conducting workshops and other promotional activities, millions of people were protected by 1989. This occurred before the availability of commercially produced pretreated nets and before global funding for mass net distribution. This paper describes the sequence of steps leading to regional control success. The beginning stages in 1979 recognized that treating torn mosquito nets was a viable control option. Basic net treatment procedures were established by 1983 and worksh

Neglected tropical diseases of Oceania: review of their prevalence, distribution, and opportunities for control.

Among Oceania's population of 35 million people, the greatest number living in poverty currently live in Papua New Guinea (PNG), Fiji, Vanuatu, and the Solomon Islands. These impoverished populations are at high risk for selected NTDs, including Necator americanus hookworm infection, strongyloidiasis, lymphatic filariasis (LF), balantidiasis, yaws, trachoma, leprosy, and scabies, in addition to outbreaks of dengue and other arboviral infections including Japanese encephalitis virus infection. PNG stands out for having the largest number of cases and highest prevalence for most of these NTDs. H

[Human and animal parasitic diseases in the New Hebrides].

New-Hebrides Condominium, an archipelago in the South Pacific, is a country with a special socio-political environment, due to the duality of its French-British regime. This state of affairs is felt in all areas including Public Health, where we find French, British and Condominial personnel. The pathology of parasitic diseases is essentially tropical with a strong predominance of paludism, at times fatal, and intestinal nematodes; however we rarely find amibiasis or human hydatid disease. Strongyloidiasis as well as specific ascaris of each species are very frequent in animals. In general cat

Multicentre survey of incidence and public health impact of leptospirosis in the Western Pacific.

The objectives of the study were to describe the mode of circulation (endemic or epidemic) of human leptospirosis in various Pacific island states and territories by identifying predominant Leptospira serogroups and the most probable routes of human exposure, and to recommend a feasible laboratory strategy for leptospirosis in the Pacific. From September 2003 to December 2005, 263 leptospirosis suspect patients were recruited by public practitioners on 11 Pacific islands, using the WHO case definition. Diagnosis was confirmed using a three-level serology algorithm and a regional laboratory net

Genomic Epidemiology of Corynebacterium diphtheriae in New Caledonia.

An increasing number of isolations of Corynebacterium diphtheriae has been observed in recent years in the archipelago of New Caledonia. We aimed to analyze the clinical and microbiological features of samples with C. diphtheriae. All C. diphtheriae isolates identified in New Caledonia from May 2015 to May 2019 were included. For each case, a retrospective consultation of the patient files was conducted. Antimicrobial susceptibility phenotypes, tox gene and diphtheria toxin expression, biovar, and the genomic sequence were determined. Core genome multilocus sequence typing (cgMLST), 7-gene MLS

Alpha thalassaemia is associated with increased soluble transferrin receptor levels.

Although alpha+ thalassaemia is the commonest haemoglobinopathy in the world, it is not known if it is associated with significant ineffective erythropoiesis, a fact of importance in interpreting its complex interaction with malaria. To study this problem, we have measured the concentrations of soluble transferrin receptor (sTIR) and ferritin in 181 children from Vanuatu with heterozygous (68) and homozygous (46) alpha+ thalassaemia, and normal controls (67). sTfR concentrations were significantly higher in both homozygotes (mean 3.1 mg/l, range 2.8-3.4) and heterozygotes (2.86 mg/l, 2.6-3.2)
