The Lateral Antebrachial Neurocutaneous Flap: Case Series.

BACKGROUND: The neurocutaneous flap was developed based on the understanding that every superficial cutaneous nerve includes vessels, also known as vasa nervorum or paraneural vessels, which run around and inside the nerves and deliver blood to the skin above. A cutaneous perforator connected to paraneural vessels that vascularize the skin and nerves is referred to as a neurocutaneous perforator. The lateral antebrachial cutaneous nerve (LACN), the most dependable blood supply from the primary underlying veins of the neurocutaneous flap, is the subject of this study. METHODS: We reviewed the

Does FK506 reduce the size of the watershed area after vascular injury of the sciatic nerve?

AIM: FK506 (also known Tacrolimus, Prograf) is an immunosuppressant drug which is used to prevent rejection after organ transplantation. Although there are several studies on neuroprotective effect of FK506 on brain ischemia, few reports on effects of FK506 after peripheral nerve ischemia have been reported. In the present study, we examined the size of watershed area after stripping of the epineurial vessels and studied the effect of FK506 on reduction of the size of watershed area. MATERIALS AND METHODS: Forty-eight adult female rats were used and randomly divided into four groups as contro

Immunotactoid-like endoneurial deposits in a patient with monoclonal gammopathy of undetermined significance and neuropathy.

An 85-year-old man with a 2-year history of progressive lower limb weakness and paresthesia was found to have an IgG kappa monoclonal gammopathy of undetermined significance (mgus). Clinical and electrophysiological studies revealed a severe distal bilateral symmetrical polyneuropathy. A sural nerve biopsy showed extensive nerve fibre loss with the deposition of large amounts of amorphous material throughout the endoneurium. Electron microscopy showed the deposits to be composed of microtubular structures which were located diffusely throughout the endoneurium. The deposits were also located w

Facial palsy after embolization with Squid® 12.

Endovascular procedures with liquid embolic agents such as ethylene-vinyl alcohol (EVOH) copolymers are indicated before surgical treatment of cervical paraganglioma. Consequently, these agents are now available as low viscosity formulations, one of which is Squid 12, which are demonstrating superior vascular penetration. Cases of facial paralysis secondary to embolization of cervical vascular lesions with classic embolic agents have been reported in the English literature, however, this complication has not been described with new generation options such as Squid 12.We describe the case of a

Topographical anatomy of the transverse facial artery.

The transverse facial artery (TFA) is found in the lateral face and supplies the parotid gland and duct, facial nerve, facial muscles, and skin. To better understand the cutaneous vascularization of the lateral face and to better characterize the topography and other anatomical features of the TFA, microsurgical dissection was performed in 44 cadavers. The number of TFAs present ranged from one to three, and a single TFA was most common (70.5%). The TFA originated from the superficial temporal artery at or above the level of crossing by the temporofacial trunk of the facial nerve in the paroti

Pathophysiological mechanisms of intraoperative and postoperative hearing deficits in cerebellopontine angle surgery: an experimental study.

Pathophysiological mechanisms responsible for intraoperative and postoperative hearing deficits associated with cerebellopontine (CP) angle operations were explored experimentally in dogs. The CP angle operative manipulations performed were the same as those experienced by human patients, and auditory evoked potentials were monitored intraoperatively. As a result of the operative manipulations, petechial or confluent hemorrhages occurred at the compressed portions of the cochlear nerve, and intravascular clots were often observed. Disintegration of the nerve fibers was verified by ultrastructu

Lower limb cold exposure induces pain and prolonged small fiber dysfunction in Fabry patients.

In Fabry disease, an X-linked alpha-galactosidase A deficiency, painful crises and limb paresthesias are possibly linked to thermal exposure. Small nerve fiber function has not yet been tested after cold challenge. In two Fabry patients (15 and 17 years old), their heterozygote mother, their healthy sister, and eight controls, we determined warm and cold perception thresholds at the dorsal foot and the lower medial calf (method of limits, Somedic-Thermotest), before and 1, 5, 10 and 15 min after 30 s immersion of one leg into 5 degrees C water. Discomfort was rated from 0 to 10. At baseline, t

Case Series of Intraoperative Indocyanine Green Angiography Findings in Minimally Invasive Surgical Ligation of Cerebrospinal Fluid-Venous Fistulas.

BACKGROUND AND OBJECTIVES: Spontaneous intracranial hypotension is a debilitating condition resulting from cerebrospinal fluid (CSF) loss. Spontaneous CSF leaks are classified as ventral dural defects (Type 1), meningeal diverticula (Type 2), and CSF-venous fistulas (CVFs, Type 3). CVFs are recently described, diagnostically challenging, and poorly understood intraoperatively. We present a technical note involving 8 patients undergoing intraoperative indocyanine green (ICG) angiography during CVF ligation. METHODS: We conducted a retrospective review of consecutive patients undergoing minimal

Technical Description and Microsurgical Outcomes in Phalloplasty Using the Deep Inferior Epigastric Artery and Locoregional Veins.

BACKGROUND: Phalloplasty often requires free tissue transfer. There is ample literature describing flap-related outcomes, but the microsurgical technique used, including choice of recipient vessels, has been an overlooked yet important topic. In this study, the authors review the outcomes of their experience with the deep inferior epigastric artery and locoregional veins and outline technical modifications that occurred during the study period. METHODS: A retrospective chart analysis of patients who underwent microsurgical phalloplasty between September of 2016 and July of 2019 was performed.

Improving Microcirculation With Nerve Decompression: The Missing Link in Treatment of Diabetic Neuropathy and Diabetic Foot Ulcer.

Sympathetic dysfunction in skin is well known in diabetic peripheral neuropathy. This produces dry, cracked, peeling skin susceptible to infection and also epidermal microcirculation insufficiency. Impaired autonomic neurovascular control opens dermal arterio-venous anastomoses and shunts microcirculation away from the epidermis and impairs skin oxygenation and nutrition. Few recognise that diabetic neuropathy includes swelling-induced entrapment neuropathy. Multiple peripheral nerves, swollen by the secondary polyol metabolic pathway, suffer local compressions at fibro-osseous tunnels. This i

Preoperative Embolization of Brain Tumors Using a Provocative Test: Evaluating the Safety and Efficacy of Embolization of Test-Positive Vessels.

PURPOSE: The embolization of vessels potentially involved in the vasa nervorum during brain tumor embolization is often a non-aggressive procedure. In this study, we aimed to investigate the safety and efficacy of embolization of vessels with positive provocative test results. METHODS: Embosphere (Merit Medical Systems, South Jordan, Utah, USA) was the embolization material of choice. A provocative test was performed using 30 mg of 1% lidocaine, with a positive result if neurological symptoms appeared. A size of 500-700 µm was used for positive results and 300-500 µm for negative results. Emb

The dynamics of beta 1 integrin expression during peripheral nerve regeneration.

The aim of the present study was to examine the expression of beta 1 integrin subunit after peripheral nerve transection. After sciatic nerve transection two experimental procedures were used; changes in the freely regenerating rat sciatic nerve were compared to a situation in which spontaneous regeneration was prevented by suturing both ends of the nerve to the muscle next to the point of transection. Specimens for morphological analysis were collected 6 h, 1, 3, 5, 7 days and 2, 4, 6 and 8 weeks after the axotomy. Sections from the proximal (two zones) and distal (three zones) stumps next to
