Comparison of manual compression and vascular hemostasis devices after coronary angiography or percutaneous coronary intervention through femoral artery access: A meta-analysis of randomized controlled trials.

OBJECTIVES: To compare the efficacy and safety of manual compression (MC) with vascular hemostasis devices (VHD) in patients undergoing coronary angiography (CA) or percutaneous coronary intervention (PCI) through femoral artery access. INTRODUCTION: The use of femoral artery access for coronary procedures may result in access-related complications, prolonged immobility and discomfort for the patients. MC results in longer time-to-hemostasis (TTH) and time-to-ambulation (TTA) compared to VHDs but its role in access-related complications remains unclear in patients undergoing coronary procedur

Vascular closure with a suture retention device versus suture-mediated closure system after large bore venous access for electrophysiologic procedures.

BACKGROUND: The LockeT is a suture retention device that may be a postoperative closure option for electrophysiologic procedures requiring large-bore vascular access (LBVA) but warrants further evaluation. OBJECTIVES: We compare the use of LockeT to a suture-mediated vascular closure device for LBVA after left atrial appendage occlusion (LAAO). METHODS: We performed a retrospective study of LBVA after LAAO from March 2023 to August 2024 using LockeT or Perclose ProGlide. Endpoints included access-site-related complications, time to hemostasis (TTH), time to ambulation (TTA), and same-day dis

[Outcomes of suture-mediated vascular closure device in the closure of left brachial artery access site after thoracic endovascular aortic repair].

OBJECTIVE: To evaluate the feasibility and safety of the suture-mediated vascular closure device in the closure of the left brachial artery access site after thoracic endovascular aortic repair (TEVAR). METHODS: Data from 91 patients receiving TEVAR with left brachial artery puncture from January 2021 to May 2023 were retrospectively collected and analyzed. In 27 cases whose brachial artery was over 5 mm, a suture-mediate vascular closure device (Perclose ProGlide, Abbott) was used to deal with the left brachial artery access, and in the other 64 cases with brachial artery < 5 mm, manual comp

Persistent Left Superior Vena Cava Associated with Right Aberrant Subclavian Artery Detected during Totally Implantable Vascular Access Device Insertion.

Introduction  Persistent left superior vena cava (PLSVC) is a rare vascular malformation, with several cases reported in the English literature. The diagnosis is made incidentally, during cardiovascular imaging or when a catheter is placed in the left jugular or subclavian vein. They are without associated hemodynamic alterations, except if they have left atrial drainage or an associated dilation of the coronary sinus. If necessary, long-term PSLVC catheterization with right atrial drainage is safe. Case Presentation  We report the case of 40-year-old man, admitted for placement of totally imp

A Novel Suture-Based Vascular Closure Device to Achieve Hemostasis after Venous or Arterial Access While Leaving Nothing behind: A Review of the Technological Assessment and Early Clinical Outcomes.

Vascular hemostasis after venous and arterial access in cardiovascular procedures remains a challenge. As sheath size gets larger for structural heart and vascular procedures, no dedicated closure devices exist that can overcome all the challenges of achieving vascular hemostasis, in particular on the venous side. Efficiently and reliably ensuring hemostasis at the access point is crucial for enhancing the safety of a procedure. Historically, hemostasis relied on manually compressing venous access sites. However, the shift towards larger sheaths and the more frequent use of continuous anticoag

Vascular access problems revisited: the Multinational Association of Supportive Care in Cancer (MASCC) experience.

Despite the increased availability of vascular access devices, there is limited information regarding their pattern of use in the clinical setting or the criteria used for their utilization. To obtain this information, we sent questionnaires to MASCC members. Forty-five percent of the respondents stated that the decision to use vascular access devices depended on the drugs used at the beginning of therapy. Another 30% replied that the decision to use long-term vascular access devices depended on the status of the patient's veins at the beginning of therapy. Only 12% of those surveyed waited un

Scoping Review of Early Intravenous Infiltration and Extravasation Detection Devices.

Failure to promptly detect intravenous (IV) infiltration can often lead to damaging effects, such as necrosis and compartment syndrome, which increase the length of hospital stay and cost of care. Currently, nurses periodically monitor the vascular access device (VAD) site and extremity for symptoms of swelling, blanching, and change in temperature. However, nurses are often unable to monitor the VAD site frequently enough to detect subtle symptoms that may present immediately following an infiltration or extravasation. Nurses need a highly sensitive way to rapidly detect IV infiltration to mi

Femoral Artery Closure Devices vs Manual Compression During Cardiac Catheterization and Percutaneous Coronary Intervention.

BACKGROUND: Femoral arterial access remains widely used despite recent increase in radial access for cardiac catheterization and percutaneous coronary intervention (PCI). Various femoral artery closure devices have been developed and are commonly used to shorten vascular closure times, with variable rates of vascular complications observed in clinical trials. We sought to examine the rates of contemporary outcomes during diagnostic catheterization and PCI with the most common femoral artery closure devices. METHODS: We identified patients who had undergone either diagnostic catheterization al

The use of experimental preclinical models in evaluating endovascular interventions for vascular access stenosis: A review.

Stenosis of vascular accesses, particularly in arteriovenous fistulas and grafts, remains a significant challenge in hemodialysis patients, often leading to access dysfunction and failure. Endovascular interventions, including stent grafts and drug-coated balloons (DCBs), have emerged as key treatment modalities to mitigate restenosis and prolong access patency beyond simple balloon angioplasty. Preclinical evaluation of these devices relies on large animal models to assess safety, effectiveness, and biological responses before clinical application. This review provides a comprehensive analysi

Infusion sets and vascular access as a strategy to address future challenges in intravenous therapy.

This special article addresses a highly relevant issue in healthcare, where the demand for care and attention has significantly increased, coinciding with a progressively aging population and the prevalence of multiple chronic conditions. These factors pose considerable challenges for contemporary healthcare systems. Vascular access devices, including peripheral intravenous catheters, are widely utilized for intravenous therapy in hospitals; however, they are associated with risks and complications. Nearly 50% of these catheters are removed in an unplanned manner, which exacerbates the problem

Femoral and Brachial Artery Cut-Down for Vascular Access in Interventional Procedures.

Femoral and brachial artery cut-downs are classic surgical methods for establishing vascular access in interventional procedures. Although largely replaced by less invasive percutaneous techniques and vascular closure devices, these open approaches remain essential when anatomy is challenging or large devices are needed. This paper outlines the indications, preoperative assessment, and step-by-step surgical techniques for safely exposing the femoral and brachial arteries. Direct visualization during cut-down facilitates reliable hemostasis and mitigates access-related complications, such as ps
