Feeding Difficulties Following Vascular Ring Repair: A Contemporary Narrative Review.

Vascular rings are congenital abnormalities of the aortic arch vascular system that compress the trachea and esophagus. A review of long-term outcomes suggests that chronic feeding difficulties can persist following surgical repair of vascular rings. Previous reports of postoperative vascular ring division outcomes indicate that chronic esophageal symptoms may persist following repair, though most available data focuses on persistent respiratory symptoms. It is therefore the aim of this article to summarize and organize recent evidence reporting the frequency, presentation, and management of f

Morphology of vascular ring arch anomalies influences prognosis and management.

OBJECTIVE: This study aimed to explore the anatomical features of aortic arch anomalies associated with vascular rings, hoping to identify those which may increase the risk of symptomatic presentation and surgical intervention. METHODS: This was a retrospective observational study at a single cardiac unit. Individuals diagnosed with an aortic arch anomaly, either isolated or non-isolated, between June 2014 and September 2018 were included. The morphology of the aortic arch was established via analysis of postnatal echocardiography, CT or MRI scans. CT and magnetic resonance studies were evalu

Surgical treatment of vascular ring including right cervical aortic arch.

Cervical aortic arch is a developmental entity consisting of persistence of the right or left third branchial arch and regression of the fourth branchial arches. In most cases, the aorta is redundant and crosses behind the esophagus to the opposite side. In the presence of an aberrant subclavian artery contralateral to the side of the aortic arch and a ligamentum arteriosum, a vascular ring is formed around the trachea and esophagus. Two young patients with right-sided cervical aortic arch, aberrant left subclavian artery, and ligamentum arteriosum presented with dysphagia and exertional dyspn

Aortic diverticulum without vascular ring: a rare cause of dysphagia.

In an adult with severe dysphagia, diagnosis of aneurysmal aberrant subclavian artery was suspected on computed tomogram; no vascular ring was visible on arteriogram. A diverticulum of the aortic isthmus was surgically resected. Histologic analysis ruled out acquired disease. In conclusion, dysphagia related to esophageal compression may be caused by an isolated congenital aortic diverticulum.

[An operative case of vascular ring (Edwards III B type)].

A 28-year-old female with dysphagia due to the vascular ring (Edwards III B type) was operated successfully. Preoperative chest x-ray film showed a defect of the left first arch and deviation of the lower trachea to the left side. Esophagogram disclosed the upper thoracic esophagus stenotic and deviated left anteriorly. Arch aortogram showed the right aortic arch and aberrant left subclavian artery with the aortic diverticulum. Operation was performed through a left thoracotomy. Esophagus was squeezed by the aortic arch, aortic diverticulum and left ligamentum arteriosum. The left ligamentum a

Multidisciplinary approach to vascular rings and vascular-related aerodigestive compression: a clinical practice review.

Vascular rings, including double aortic arch and right aortic arch with aberrant left subclavian and left ligamentum, are part of a larger group of vascular-related aerodigestive compression syndromes that also includes innominate artery compression syndrome, dysphagia lusoria, aortic arch anomalies, and aneurysms of either the aorta or pulmonary artery. Additionally, post-surgical airway compression is a distinct entity in itself. The approach to the diagnosis and management of these varied phenomena has been streamlined by the multidisciplinary team at Boston Children's Hospital. Echocardiog

Outcomes of reoperation following prior vascular ring and airway compression surgery.

OBJECTIVE: After vascular ring repair, the incidence of residual symptoms ranges from 15% to 80%. Symptoms often relate to persistent vascular compression or tracheobronchomalacia. Reoperative surgery, although effective for symptom relief, remains debated due to perceived risks and variable outcomes. This study aims to characterize patients undergoing revisional vascular ring surgery at our institution. METHODS: We identified patients who underwent reoperations for persistent vascular ring-related symptoms from 1991 to 2024. Patient history, surgical approaches, and clinical outcomes were re

[Common bile duct stricture caused by vascular ring: a case report].

INTRODUCTION: The most common congenital malformations of the bile duct are biliary atresia and choledochal cyst. In addition, the most common liver anatomical variation is the right hepatic artery aberration. The goal of this study is to characterize a patient with this disease and propose the hepatoduodenal anastomosis as surgical treatment. CASE REPORT: One-month-old patient with suspected congenital biliary atresia due to progressive jaundice and acholia since birth. Liver tests consistent with a cholestatic pattern and brain MRI scan consistent with biliary atresia. Periods of decreased

[Severe respiratory failure secondary to vascular ring. Report of 2 cases].

Vascular compression of the airways is an infrequent situation that can be masked by inespecific respiratory symptoms. We present two cases with severe respiratory distress, that needed assisted mechanical ventilation in patients with overdistended lungs which improved when the endotracheal cannula was shifted near the carina. The chest radiograph showed hyperinflation of the lungs; an identation was demonstrated in the esophagogram and the endoscopy demostrated extrinsic esophagical compression. The angiogram documented double aortic arch in the first case and aberrant subclavia artery in the

Oculo-auriculo-vertebral spectrum with vascular ring and other unusual anomalies.

An infant with the oculo-auriculo-vertebral (OAV) spectrum is described with the previously unreported association of rectal agenesis with recto-urethral fistula. The patient died at the age of 14 weeks from respiratory obstruction. Autopsy revealed the cause of death to be tracheal compression from a pulmonary artery sling. Previous reports of vascular rings within the OAV spectrum are reviewed, and it is concluded that these malformations are an uncommon, but clinically important, complication of the disorder.

When does an aberrant right subclavian artery create a vascular ring?

OBJECTIVE: To describe the rare combination of findings where a left aortic arch and aberrant right subclavian artery are associated with a vascular ring, which is in the setting of a right-sided arterial duct. METHODS: Analysis of all prenatally diagnosed cases of left aortic arch and right-sided arterial duct (between 2016 and 2025) at three fetal cardiac centres (2 UK and 1 Austrian) with documentation of aortic arch branching pattern. RESULTS: In 5/6 cases, aberrant right subclavian artery was identified. Genetic abnormality was identified in 50% of cases (22q11 microdeletion). Three fet
