Anatomical implications of the subvastus approach on major vascular injury during a distal femoral osteotomy: a computed tomographic venography study.

PURPOSE: This study evaluated major vascular injury risk in distal femoral osteotomy (DFO) via the subvastus approach and examined the relationship between the posterior border of the vastus medialis (VM) and the superficial femoral artery (SFA) and vein (SFV). METHODS: Eighty limbs from 80 patients were evaluated using lower extremity computed tomographic venography. The positional relationship between the VM, SFA, and SFV was evaluated by measuring the angle between each structure and the horizontal reference point (VMA: VM angle, FAA (femoral arterial angle), FVA (femoral venous angle) and

Angiogenesis Dysregulation in the Pathogenesis of Systemic Sclerosis.

Systemic sclerosis is a chronic autoimmune connective tissue disease characterized by vascular injury and fibrosis and by an impaired angiogenesis which cannot ensure an efficient vascular recovery. Vascular injury is responsible for hypoxia and tissual ischemia which are the primary triggers for angiogenesis and are not able to induce a compensatory angiogenesis. This review article is focused on current knowledge on the mechanisms responsible for angiogenesis dysregulation in systemic sclerosis.

Brachial plexus injury due to compression: an alternate mechanism of injury: case report and review of the literature.

We present a case of a brachial plexus injury due to compression of the nerves from a traumatic hematoma, with no associated bone or vascular injury. The paralysis in this case was not evident for 48 hours after the initial injury, implying that the brachial plexus was not damaged directly. Electromyograms documenting brachial plexopathy were obtained. The mechanism of injury in this case is different from the usual mechanisms of injury in brachial plexus trauma. The majority of brachial plexus injuries are associated with multisystem trauma. The mechanism of injury to the brachial plexus is e

Vascular complications of exposure for anterior lumbar interbody fusion.

OBJECTIVE: The purpose of this study is to document the incidence of vascular complications during anterior lumbar interbody fusion (ALIF) in 212 consecutive patients treated at the Scripps Clinic and determine what factors adversely affected outcome. METHODS: We reviewed the prospectively maintained database of all ALIF procedures performed at Scripps Clinic between August 2004 and June 2009. All procedures were performed by a spine surgeon in conjunction with a vascular surgeon who performed the exposure portion of the operation, and protected the vessels from injury during the instrumentat

[Neuromuscular complications of fractures of the extremities, part 2 : Nerve injuries and iatrogenic damage].

Lesions of peripheral nerves substantially influence the long-term prognosis and functional outcome. Approximately 70% of peripheral nerval lesions are associated with vascular injuries and occur more frequently with certain fractures and osteosynthesis types. The prognosis and treatment depend on the severity of the injury and the presence of axonal lesions, in particular, determines the further procedure. Treatment as early as possible is associated with a clear improvement in the prognosis. Defects longer than 8 mm in size necessitate a nerve transplantation. Iatrogenic vascular lesions occ

[Injuries to the main vessels and their reconstruction in pancreatoduodenal resection].

The article discusses the results of surgical correction of injuries inflicted to the vessels of the portal system in three patients during pancreatoduodenal resection. Growth of the pancreatic tumor into the main vessels may be revealed only during the operation, which requires vascular plastics. Compression of the portal vein causes marked changes of the central and regional hemodynamics.

Topographic localization of the sacroiliac joint and superior gluteal artery branches on the posterolateral ilium.

Latrogenic vascular injuries at the posterior ilium during sacroiliac screw placements are not uncommon. Though intra-operative imaging reduces the risk of such injuries, anatomical localization of the sacral segments using discrete topographical landmarks is not currently available. This descriptive study proposes the use of an anatomical grid system to localize the sacroiliac articulation on the posterolateral ilium. It also investigates the positional variability of the branches of the superior gluteal artery (SGA) within areas defined by the grid. 48 dried adult hip bones were examined to

Managing peripheral vascular injuries in gunshot trauma: A surgical perspective.

Firearm-related vascular trauma is a significant public health issue in Cape Town, South Africa, where gunshot wounds (GSWs) are a leading cause of penetrating extremity injuries. This commentary reviews the experience of Groote Schuur Hospital's Trauma Centre in managing extremity arterial injuries due to GSWs. Groote Schuur Hospital is a tertiary academic institution situated in Cape Town, South Africa. As one of the country's foremost teaching hospitals, we serve a population of approximately 4 million people from the greater Cape Town metropolitan area and surrounding regions. On average,

Neutrophils as drivers of vascular injury in sickle cell disease.

While neutrophils are the main effectors of protective innate immune responses, they are also key players in inflammatory pathologies. Sickle cell disease (SCD) is a genetic blood disorder in which red blood cells (RBCs) are constantly destroyed in the circulation which generates a highly inflammatory environment that culminates in vascular occlusions. Vaso-occlusion is the hallmark of SCD and a predictor of disease severity. Neutrophils initiate and propagate SCD-related vaso-occlusion through adhesive interactions with the activated and dysfunctional endothelium, sickle RBCs, and platelets,

Blunt Vascular Injuries of the Face and Scalp: Systematic Review and Introduction of a Grading Scheme and Treatment Algorithm.

INTRODUCTION: Branches of the internal and external carotid arteries supplying the scalp and face are vulnerable to injury from blunt trauma and penetrating injuries, especially over bony structures. Blunt trauma can lead to vessel dissection, pseudoaneurysm, or transection with extravasation. These injuries often present days to months post-trauma unless accompanied by avulsion with hemorrhage. Compounding the challenge, additional injuries are rare. Unlike blunt cerebrovascular injuries, there is no standardized grading system for diagnosing blunt cephalovascular injuries. METHODS: A system

A pDNA/rapamycin nanocomposite coating on interventional balloons for inhibiting neointimal hyperplasia.

Drug-coated balloon (DCB) is a therapeutic method that can effectively deliver antiproliferative drugs such as paclitaxel and rapamycin (RAPA) with no permanent implants left behind. However, delayed reendothelialization due to the toxicity of the delivered drugs leads to poor therapeutic effects. Here, we propose a new design of DCB coating, which incorporates both vascular endothelial growth factor (VEGF)-encoding plasmid DNA (pDNA) that can promote endothelial repair and RAPA into protamine sulfate (PrS). We demonstrate that the PrS/pDNA/RAPA coating had stability and good anticoagulation p

Diagnostic performance of computed tomography angiography in peripheral arterial injury due to trauma: a systematic review and meta-analysis.

OBJECTIVES: To evaluate the diagnostic accuracy of computed tomography angiography (CTA) in detecting arterial lesions in patients with suspected arterial injury of the upper or lower extremity due to trauma. METHODS: A systematic review and meta-analysis was carried out. Medline and Embase were searched on August 13, 2012, for studies comparing CTA with surgery, digital subtraction angiography (DSA), or follow-up, which allowed extraction of data into two-by-two tables. The methodological quality of included studies was assessed using the QUADAS tool. Summary estimates of sensitivity and spe

Cerebrovascular injuries in traumatic brain injury.

Traumatic brain injury is a complex and highly heterogeneous disease due to the host of concomitant injuries that may accompany the initial insult. Due to the dynamic interplay between the injuries that may arise, the management of these injuries is challenging. In a small subset of patients with traumatic brain injury, cerebral vascular injury may occur, which presents its own diagnostic and therapeutic challenges. These vascular injuries often present in a delayed fashion, thereby going unnoticed by clinicians. Early recognition and treatment of these injuries is crucial, given their high mo

Incidence of brachial plexus injury with concurrent subclavian or axillary vascular injury and its influence on reconstruction.

INTRODUCTION: Brachial plexus reconstruction with free functional muscle transfers (FFMT) has become a reliable tool in the armamentarium of brachial plexus surgeon (Potter and Ferris, The Journal of hand surgery, European volume, 2017, 42,693-9). The successful execution of performing FFMT relies on favourable motor nerves for coaptation and appropriate vessels for microvascular anastomoses. Due to shared traumatic aetiology, subclavian and axillary vascular injury (SAVI) can coexist with the brachial plexus palsy and may pose a surgical dilemma with such FFMT execution. METHOD: We performed
