Penetration of an inferior vena cava filter into the aorta.

Transvenous placement of inferior vena cava (IVC) filters is commonly performed in selected patients with deep venous thrombosis and pulmonary embolism. However, filter placement is sometimes associated with serious complications. A common complication is asymptomatic perforation of the IVC and penetration of adjacent organs by the filter. Here, we report a case of an 83-year-old man whose prophylactic IVC filter penetrated the aorta. The patient was closely followed without surgical intervention for more than a year, and no additional complications were observed.

Complications of temporary vena cava filter placement.

BACKGROUND: Temporary vena cava filters have been used for protection from potentially fatal pulmonary embolism. However, recent reports suggested that they may be associated with serious adverse complications including filter-related thrombosis. The purpose of this study was to examine the clinical complications of temporary vena cava filter placement. METHODS: We enrolled 40 consecutive patients from January 2006 to December 2010 who underwent percutaneous temporary vena cava filter insertion in Saitama Medical Center, Jichi Medical University. RESULTS: Major filter complications related t

Misplacement of a vena cava filter into the spinal canal.

We report the case of a 70-year-old male with a complication of misplacement of a vena cava filter into the spinal canal. This likely happened as a result of penetration of the wire and filter sheath through the iliac vein or vena cava into the retroperitoneum, vertebral foramina, and spinal canal at the level of L2 and L3. Due to the patient's condition, the filter was not removed and no neurologic symptoms have occurred. This represents the first reported case of a filter deployment into the spinal canal. Although placement of vena cava filters is a relatively safe procedure, complications a

Current vena cava filter devices and results.

The critical focus in the acute management of deep vein thrombosis (DVT) continues to be the effective prevention of associated life-threatening pulmonary embolism (PE) and the achievement of survival for those in whom such emboli occur. As such, primary treatment for DVT or PE demands adequate anticoagulation with intravenous heparin followed by maintenance oral warfarin for 3 to 6 months. This centered approach successfully prevents 90% to 95% of cases of associated pulmonary embolism. However, under circumstances in which anticoagulation cannot be used, or in which it fails, there is clear

Potential of overuse of the inferior vena cava filter.

To examine indications for, and morbidity and mortality rate of, inferior vena cava filter insertion at a community hospital, the records of 69 patients who received inferior vena cava filters were reviewed. Patients were assigned to three groups--group 1, 45 patients with pulmonary embolism or deep venous thrombosis and a contraindication to anticoagulation; group 2, 14 patients with a diagnosis as in group 1, who received filters without consideration to anticoagulation, and group 3, ten patients with clinically suspected deep venous thrombosis and no objective assessment of the process. Placement of a Greenfield filter in the superior vena cava.

A 75-year-old man with a Greenfield filter in the inferior vena cava for previous pulmonary emboli had recurrent pulmonary emboli. He was unable to take anticoagulants due to gastric ulcer disease. Contrast studies revealed a large thrombus in the left brachiocephalic vein. A Greenfield filter was placed in the superior vena cava through the right internal jugular vein using the femoral insertion apparatus.

Fracture of a Temporary Inferior Vena Cava Filter.

Anticoagulation is recommended for the treatment of pulmonary embolism (PE) and deep vein thrombosis (DVT). In some cases, an inferior vena cava (IVC) filter is used to prevent PE. We report the case of a 70-year-old woman who developed non-massive PE and proximal DVT, which were treated using an IVC filter; two filters were placed owing to the fracture of the filters. Few previous reports have discussed IVC fractures and the difficulty in detecting such fractures on computed tomography before retrieval. Based on our experience, we suggest that a temporary IVC filter for DVT treatment should b

Expectoration of an inferior vena cava filter strut.

Inferior vena cava (IVC) filter use is widespread in patients with venous thromboembolism (VTE) and temporary contraindication to anticoagulation, though timely removal is often not performed. We report the case of an expectoration of an IVC filter strut. Review of the patient's prior imaging confirmed an infrarenal Bard G2 filter with an absent strut, which was visualised in the left lung base. The strut was presumed to have embolised to a pulmonary artery branch and eroded into an adjacent bronchus. Subsequent fluoroscopically guided filter retrieval was successful. The incidence of IVC filt

Placement of a vena cava filter with an antecubital approach.

RATIONALE AND OBJECTIVES: The authors performed this study to evaluate the use of an antecubital venous approach for inferior vena cava (IVC) filter placement. MATERIALS AND METHODS: An IVC filter was placed in 26 patients (15 men, 11 women) in whom the antecubital vein was the preferred access site. An antecubital vein was accessed with ultrasound guidance and used for IVC filter placement. This same access site was used to place a peripherally inserted central catheter (PICC) in 17 of the 26 patients. Access was obtained via the basilic vein in 15 patients (58%), brachial vein in eight (31%

[Vena cava filter for prevention of pulmonary embolism].

Vena caval filters in the prevention of pulmonary embolism All currently available caval filters can be implanted percutaneously in local anaesthesia. In the USA the FDA has approved the stainless steel Greenfield filter, the Titanium Greenfield filter, the Bird's Nest filter, the LGM or VenaTech filter and the Simon Nitinol filter. Some other caval filters are commercially available in Europe, but there exist only few clinical trials about them. The Greenfield filter is implanted since the early seventies and the greatest amount of data has been published about it. Standard indications for fi

[Vena cava umbrella filter: complications and treatment].

BACKGROUND AND OBJECTIVE: Increasing numbers of vena canal filters are being implanted to prevent pulmonary embolism, which are mainly the consequence of deep vein and pelvic vein thrombosis. Can a filter be removed again in case of complications arising from it? What is the risk of such operative explantation? What is the subsequent risk of pulmonary embolism? PATIENTS AND METHODS: In nine patients (5 males, 4 females; mean age 45 (30-39 years) who had vena caval filters implanted because of thromboembolism despite anticoagulation, complications due to the filter required its operative remov

Experience with the Amplatz retrievable vena cava filter.

The Amplatz retrievable inferior vena cava filter was designed to be used as either a permanent indwelling filter or a short-term, percutaneously removable filter. The authors placed 52 filters in 52 patients. No deaths occurred as a result of filter placement or usage. Follow-up in 42 (81%) patients included inferior vena cavography (n = 31), computed tomography (n = 4), duplex ultrasound (n = 4), and autopsy (n = 3). Inferior vena cava thrombosis was found in seven (17.5%) of the 40 previously nonobstructed venae cavae studied. Two patients with caval thrombosis required a second filter to p

Recent clinical experience with the vena cava filter.

In the past at our institution, insertion of the vena cava filter was recommended almost exclusively for recurrent pulmonary emboli in anticoagulated patients or in those with emboli and contraindications to anticoagulation. More recently, prophylaxis has become a frequent indication for patients with extensive deep vein thrombosis and contraindications to anticoagulation. Herein, we review a recent series of caval filter insertions to determine if increased utilization of this technique is justified in terms of morbidity and effectiveness. Twenty-one filters were inserted in 20 patients durin

Temporary inferior vena cava filters factors associated with non-removal.

OBJECTIVES: Inferior vena cava filter (IVCF) placement is indicated when there is a deep vein thrombosis and/or a pulmonary embolism and a contraindication of anticoagulation. Due to the increased risk of recurrent deep venous thrombosis when left in place, IVCF removal is indicated once anticoagulant treatment can be reintroduced. However, many temporary IVCF are not removed. We aimed to analyze the removal rate and predictors of filter non-removal in a university hospital setting. METHODS: We collected all the data of consecutive patients who had a retrievable IVCF inserted at the Saint-Eti

Inferior Vena Cava Filters and Prevention of Recurrent Pulmonary Embolism.

Although inferior vena cava (IVC) filters have a clear role in preventing recurrent pulmonary embolism (PE) in patients with venous thromboembolism who cannot be anticoagulated, the role of filters in patients who are candidates for anticoagulation is controversial. With limited and conflicting data, practitioners often have to make an educated patient-specific decision when encountering this scenario. This article reviews the available data on the efficacy and risks associated with adjunctive IVC filter use to prevent recurrent PE.
