Chronic venous insufficiency and venous leg ulcers: Aetiology, on the pathophysiology-based treatment.

The chronic venous disease covers a wide spectrum of venous disorders that are characterized by severely impaired blood return that primarily affects veins in the lower extremities. Morphological and functional abnormalities of the venous system led to chronic venous insufficiency (CVI), and present as leg heaviness/achiness, edema, telangiectasia, and varices. The term 'chronic venous insufficiency' (CVI) refers to a disease of greater severity. Venous dysfunction is associated with venous hypertension and is associated with venous reflux due to poorly functioning or incompetent venous valves

The utilisation of superficial venous surgery for chronic venous insufficiency by the U.K. Asian population.

BACKGROUND: The epidemiology of superficial venous disease is relatively well defined in the U.K. Caucasian population. By contrast, there are currently no data available for Asians, who comprise 3.6% of the U.K., and 14.1% of this institution's catchment population. The aim of this study was to compare surgery for superficial venous disease in Caucasians and Asians in this institution, in the context of our local population. METHODS: A prospectively gathered database of all 2011 superficial venous operations performed between January 1997 and April 2002 was retrospectively analysed with rega

Does severe venous insufficiency have a different etiology in the morbidly obese? Is it venous?

OBJECTIVE: Chronic venous insufficiency (CVI) is the most common cause of leg ulcers. Patients with morbid obesity are remarkable for particularly recalcitrant ulcers. Because obesity is not specifically incorporated in CEAP or other venous scoring systems, we sought to characterize this group of patients more completely. METHODS: Patients with severe CVI (CEAP clinical class, 4, 5, and 6), and class III obesity (body mass index [BMI], >40) were reviewed. Findings from clinical and duplex ultrasound scan (DU) examinations were compared with the CEAP classification, its adjunctive venous clini

Quality of life in patients with venous stasis ulcers and others with advanced venous insufficiency.

The quality of life (QoL) in patients with advanced venous insufficiency (including venous stasis ulcers, skin discoloration, stasis eczema, and lipodermatosclerosis) assessed using the Clinical Etiological Anatomical Pathophysiological (CEAP) and Venous Clinical Severity Score (VCSS) classifications is presented. Also, disease features such as: intensity of pain, edema and inflammatory response that exerted the most profound effect on different domains of QoL are reported. The global QoL in patients with lower leg venous ulcerations was relatively similar to that observed in other patients wi

A systematic review of pneumatic compression for treatment of chronic venous insufficiency and venous ulcers.

INTRODUCTION: As part of a reconsideration of coverage policy, the Centers for Medicare and Medicaid Services requested a systematic review of the evidence on the use of pneumatic compression devices in the home environment for treatment of chronic venous insufficiency (CVI) and venous ulcers. METHODS: Articles were found with a systematic literature search of MEDLINE, EMBASE, and AMED (Allied and Complementary Medicine) databases, hand searches of reference lists, and suggestions of experts. RESULTS: Eight trials that met the inclusion criteria, including several randomized control trials,

Predictive Values in the Progression of Chronic Venous Insufficiency Associated with Superficial Venous Incompetence.

The purpose of this study was to define predictive values in the progression of chronic venous insufficiency in patients with isolated superficial venous incompetence using duplex ultrasound and air plethysmography (APG). Five hundred eight legs in 371 patients with isolated superficial venous incompetence were included in this study. A color duplex scanner with a 5- to 10-MHz linear transducer was used to assess the distribution and the extent of venous reflux as well as the duration of reflux and the retrograde peak velocity at the saphenofemoral junction, greater saphenous vein in the thigh

Patterns of venous reflux in limbs with skin changes associated with chronic venous insufficiency.

The distribution of venous reflux in patients with skin changes associated with chronic venous insufficiency presenting to a specialist clinic was assessed. A total of 300 limbs in 153 patients were examined by Doppler ultrasonography with colour-flow imaging for the presence of venous reflux in superficial veins, deep veins and medial perforating veins, both above and below the knee. Ninety-eight limbs had skin changes, which included hyperpigmentation, lipodermatosclerosis, atrophie blanche and ulceration. Of this group, 2 per cent had no evidence of venous reflux on duplex scanning, 39 per

Rapid post exercise venous refilling time is an independent contributor to chronic venous insufficiency.

BACKGROUND: Chronic venous insufficiency (CVI) results in complications such as pain, swelling, edema, skin changes, and ulcerations of the lower extremities. Valvular incompetence and venous obstruction are well-recognized contributors to CVI. Post-exercise venous refilling time (P-EVRT), the time to refill veins after calf muscle contractions during venous plethysmography study, is an understudied contributor to CVI. METHODS: In this cross-sectional study of 4755 patients who were evaluated with venous air plethysmography, 9510 lower limbs were categorized based on P-EVRT into two groups: r

Chronic venous insufficiency and cardiovascular risk: Marker or bystander?

Chronic Venous Insufficiency (CVI) is a complex condition resulting from venous valve dysfunction or a loss of structural integrity in the vein walls, affecting 60% of the general population. Although traditionally regarded as a localized peripheral pathology, emerging evidence suggests that CVI may serve as an early indicator of systemic vascular disease. Our comprehensive review aims to analyzes the pathophysiological links between CVI and global cardiovascular risk, evaluating the clinical impact of venous disease on cardiovascular morbidity and mortality. The current literature highlights

Does total hip arthroplasty predispose to chronic venous insufficiency?

One hundred thirty-four limbs (40% retrieval) were reviewed 14 to 21 years after total hip arthroplasty. Each had been screened for deep vein thrombosis following surgery by the fibrinogen uptake test, with proximal thrombi confirmed venographically. The limbs were assessed for chronic venous insufficiency with a standard clinical grade and photoplethysmography. Clinical chronic venous insufficiency was found in 4 of 36 (12%) limbs without and 11 of 98 (11%) with previous thrombosis (chi-square = .09, P = .77). Clinical chronic venous insufficiency was detected in 9% of limbs (6/67) with calf

[Diagnostic pitfalls of cutaneous complications of chronic venous insufficiency].

Cutaneous lesions of the lower limbs are not always of venous origin. Only careful examination of the skin and of the leg can detect other causes. Such examination is important not only for understanding the pathophysiological origin but especially for treatment decision. Certain possible causes should be routinely sought such as arterial abnormalities and cutaneous vasculitis. Some ulcerations from the onset have an atypical appearance, such as necrotic angiodermatitis and pyoderma gangrenosum. Rarer causes (tumoural, haematologic, infectious or congenital) should be considered in case of res

Prophylaxis for recurrent cellulitis complicating venous and lymphatic insufficiency.

Serious cellulitis, a common acute illness, frequently is associated with underlying venous and lymphatic compromise. The process is easily treated but, when recurrent, is associated with significant morbidity. Local measures, such as support hose and elimination of tinea pedis, have a role in its therapy but do not uniformly prevent recurrence. Prophylactic antibiotic therapy modeled after rheumatic fever prophylaxis can be effective in the prevention of this process.

[Chronic venous insufficiency and microcirculation. Physiopathologic and therapeutic reflections].

In patients with chronic venous insufficiency, tissular damage occurs as a consequence of microcirculatory disturbances. Venular distension, venulo-arteriolar reflex and probably valves in collecting venules are the only microcirculatory protective mechanisms against venous pressure overload. Edema is primarily a consequence of increased capillary hydrostatic pressure. However, the increase of endothelial macromolecular transport and a true lymphatic microangiopathy are important contributive factors. Venous ulceration is hypoxic although the amount of total blood flow is normal in the surroun
