An accurate diagnosis of pulmonary embolism is essential to prevent excessive mortality and morbidity from lack of therapy or inappropriate anticoagulation. The clinical diagnosis is highly nonspecific because none of the symptoms or signs of pulmonary embolism is unique and all may be caused by other cardiorespiratory disorders. The diagnosis of pulmonary embolism is unlikely, however, if patients do not have dyspnea, tachypnea, evidence of deep vein thrombosis, or a recognized predisposition to thromboembolic disease. Objective testing is mandatory to either confirm or exclude a diagnosis of

BACKGROUND: Phase-resolved functional lung (PREFUL) magnetic resonance imaging (MRI) is a reliable, noninvasive method for assessing pulmonary ventilation and perfusion. However, the differences in subgroups of patients with chronic pulmonary embolism (CPE) have not been thoroughly investigated, and there may be significant clinical value in examining noninvasive methods for evaluating hemodynamics and right heart function in patients with CPE. This study aimed to evaluate pulmonary perfusion and ventilation in patients with suspected chronic thromboembolic pulmonary hypertension (CTEPH) via P

Chronic thromboembolic pulmonary hypertension (CTEPH) is a complication of pulmonary embolism and a major cause of chronic PH leading to right heart failure and death. Lung ventilation/perfusion scintigraphy is the screening test of choice; a normal scan rules out CTEPH. In the case of an abnormal perfusion scan, a high-quality pulmonary angiogram is necessary to confirm and define the pulmonary vascular involvement and prior to making a treatment decision. PH is confirmed with right heart catheterisation, which is also necessary for treatment determination. In addition to chronic anticoagulat

A 19 year old man presented with a six months history of dyspnoea on exertion. Chest radiographs, isotopic lung scans, and computed tomographic scans of the chest showed unilateral translucency of the left lung, absence of ventilation and perfusion in the same lung, and a 3.5 cm aneurysm of the descending thoracic aorta which compressed the left mainstem bronchus.

A trial of separation from mechanical ventilation may induce an abnormal respiratory pattern and a maldistribution of ventilation-to-perfusion ratios (VA/Q), especially in patients with chronic obstructive pulmonary disease. This study was designed to assess the effects of three different modes of ventilation on the distribution of global and also regional VA/Q in eight patients with chronic obstructive pulmonary disease recovering from acute respiratory failure who remained dependent on mechanical ventilation after more than 5 days of attempted separation from the ventilator. VA/Q distributio

Respiratory distress developed in a 5-month-old female infant with roentgenographic evidence of hyperinflation of the right lung. Bronchoscopy revealed a vascular tumor extending from the medial wall of the right main bronchus leading to a check-valve mechanism. Radionuclide lung scans showed ventilation and perfusion of the right side reduced to 36 and 11% of the total lung ventilation and perfusion, respectively. At the age of 7 months a sleeve of the right main bronchus was resected and a reanastomosis performed. The operation specimen contained a capillary hemangioma measuring 1.2 X 0.6 cm

Pulmonary embolism remains a challenging problem in diagnosis and management for the emergency physician. Although its clinical presentation is protean and often ambiguous, risk stratification can be accomplished based on the predictive power of a limited number of physical and historical characteristics. Ventilation-perfusion lung scanning occupies a central position in the work-up of suspected PE; however, evidence exists that it may be misused by many physicians. A low probability V-Q scan does not exclude the diagnosis of PE. Patients with other than normal- or high-probability patterns of

BACKGROUND: Atypical chest pain (ACP) is a diagnostic challenge that often requires multiple laboratory and radiological investigations. While radiological procedures may not be the first line of investigation in most cases, they are invaluable when appropriately applied. Physicians need to have a good understanding of the capabilities and limitations of these constantly evolving imaging modalities.
OBJECTIVE: This article provides an overview of radiological modalities as they relate to the investigation of ACP. The focus is on radiological procedures that general practitioners may come acro

Case 1: 55-year-old male was admitted to our hospital for further examination of increased transparency of X-ray in the left lower lung. He had history of pneumonia in his childhood. Left bronchography revealed mild cylindrical bronchi-ectasia in the proximal bronchi but poor filling by contrast in the peripheral bronchi. Case 2: 61-year-old male was referred to our hospital with palpitation and dyspnea. Chest X-ray film revealed hyperlucency of the left lower lung. The ventilation scan showed a marked decrease in ventilation to the left lung and air trapping was present in the left lung. The

INTRODUCTION: Pulmonary interstitial emphysema is a severe rare complication associated with mechanical ventilation in pre-term patients. It induces alterations both in ventilation and pulmonary perfusion, and it may cause progressive overdistension of the side involved and atelectasis of the contralateral lung. Management is uneasy and requires changing ventilation strategies, with invasive procedures such as blockage and selective pulmonary ventilation being potentially necessary.
CLINICAL CASE: Premature female patient born at gestation week 26+0, with an initially good clinical progressio

Basic diagnostic procedures in pulmonary disease include chest-ray (pa/lat.), screening fluoroscopy, tomography and bronchoscopy. Due to its high specificity and sensitivity computerized tomography is the most effective non-invasive examination for identifying metastatic lymph nodes in the mediastinum. Further diagnostic steps include a perfusion/ventilation scan, pulmonary angiography (with centrally located tumors), upper caval venography (in upper venous congestion), thoracoscopy (with pleural effusion to exclude pleural carcinomatosis), and cardiopulmonary function tests.

Pulmonary hypertension (PH) is a heterogeneous syndrome in which elevated pulmonary arterial pressure arises from diverse parenchymal, thromboembolic, and vasculopathic processes. Right heart catheterization remains the hemodynamic reference standard; however, clinicians increasingly turn to noninvasive imaging to triage candidates for invasive confirmation, to determine the disease phenotype, and to monitor the response to interventions such as pulmonary endarterectomy or balloon pulmonary angioplasty. In routine practice, chest computed tomography (CT), together with echocardiography and ven

BACKGROUND: Amniotic fluid embolism is seldom recognized in nonperipartum patients. The pathophysiology is uncertain and diagnosis imprecise, making management after stabilization difficult.
CASE: A 37-year-old woman at 28 weeks' gestation presented with signs and symptoms consistent with amniotic fluid embolism including disseminated intravascular coagulopathy. A ventilation-perfusion scan demonstrated unmatched perfusion defects, but other radiographic studies were negative; the patient was treated with heparin. Four days after presentation she had spontaneous rupture of membranes followed

INTRODUCTION: Ex-vivo lung perfusion (EVLP) is an emerging technique promising an expansion of the donor pool and improvements in the outcome after lung transplantation. Reliable biomarkers for local assessment of organ function in the EVLP system are intensely sought after. This study aims to evaluate the feasibility of multiparametric functional magnetic resonance imaging (fMRI) in an EVLP system in a porcine aspiration model.
MATERIAL AND METHODS: Seven female pigs were anesthetized and gastric juice was instilled in the right lower lobe bronchus to simulate aspiration. Left lungs served a

BACKGROUND: Deep venous thrombosis (DVT) and pulmonary embolism (PE) after spinal or lower extremity surgery are well recognized as common complications. Since 1995 we have investigated the incidence of PE after orthopedic surgery using ventilation-perfusion (V/Q) lung scans, and the prevalence of PE was about 10%. With a view to detecting early-stage PE by simple examinations, we evaluated the use of both the blood gas analysis and the D-dimer measurement after spinal or lower extremity surgery.
METHODS: Altogether, 85 patients who underwent spinal or lower extremity surgery were eligible fo

Swyer-James syndrome (SJS) is a rare lung condition characterized by abnormal lung growth secondary to childhood post-infectious bronchiolitis obliterans. Usually, one lung is affected more than the other leading to asymmetrical lungs with one lung being significantly smaller. The disease can lead to pulmonary obstructive airflow physiology, bronchiectasis, and fibrosis. Dyspnea usually presents early on in infancy and symptoms can mimic asthma, however, they can go unnoticed until adulthood. We present a case of SJS in a patient with adult polycystic kidney disease (ADPKD) and color vision de

BACKGROUND: Residual pulmonary vascular obstruction (RPVO) persists in a substantial proportion of patients after acute pulmonary embolism (PE) despite adequate anticoagulation and is associated with long-term functional impairment. Angiogenesis plays a central role in thrombus resolution and vascular repair. In a recent prospective study, lower circulating CD34⁺ cell levels at follow-up were associated with the presence of RPVO, suggesting impaired regenerative and angiogenic capacity in patients with persistent perfusion defects. Whether classical circulating angiogenic biomarkers reflect th

Pulmonary scintigraphy, including Xe-133 ventilation combined with Tc-99m MAA perfusion scans, was performed in 23 children (3 girls, 20 boys, ages: 2-9 years) with pectus excavatum, and the indices from lateral chest radiography were used for comparison. The findings of pulmonary scintigraphy were analyzed as 1) ventilation fractions (VF), and 2) perfusion fractions (PF) between bilateral lungs; and 3) ventilation-perfusion ratios (V/Q ratio) of both lungs. The results showed that 6 of 23 cases had impaired VF, 11 of 23 cases had impaired PF, and 7 of 23 cases had an impaired V/Q ratio in the

Single lung transplantation has become a therapeutic option for end-stage interstitial lung disease and obstructive lung disease. Our group recently extended this treatment to three patients with primary pulmonary hypertension. All patients had marked decreases in pulmonary artery pressures and pulmonary vascular resistance and increases in cardiac output following single lung transplantation. Spirometry, lung volumes, and diffusion capacity were not different in comparison to preoperative studies. Quantitative ventilation-perfusion scans revealed the majority of perfusion distributed to the t
