Invasive and non-invasive mechanical ventilation.

Early recognition of patients who might potentially require ventilatory support is a key goal of critical care outreach programs and an important skill for all hospital medical staff. Decisions about the initiation and timing of invasive ventilation can be difficult and early discussion with critical care colleagues is essential. Appropriateness of invasive ventilatory support may also be an issue requiring advanced discussion with patients and their families. In the past 10-15 years, the role of non-invasive ventilation (NIV) has expanded, not least in an attempt to minimize the complications

Weaning infants from mechanical ventilation.

Protracted mechanical ventilation is associated with increased morbidity and mortality in preterm infants and thus the earliest possible weaning from mechanical ventilation is desirable. Weaning protocols may be helpful in achieving more rapid reduction in support. There is no clear consensus regarding the level of support at which an infant is ready for extubation. An improved ability to predict when a preterm infant has a high likelihood of successful extubation is highly desirable. In this article, available evidence is reviewed and reasonable evidence-based recommendations for expeditious

Myth: mechanical ventilation is a therapeutic relic.

Non-invasive respiratory support techniques such as continuous positive airway pressure (CPAP) have been increasingly used for management of surfactant-deficient lung disease in preterm infants. The successful use of this approach depends upon the condition of the baby at birth and requires the establishment of spontaneous breathing at birth. The reported advantages of CPAP in observational studies demonstrating a reduction in chronic lung disease have not been substantiated in recently reported well-designed randomised trials. This approach is now more established in larger and more mature pr

Pneumothorax after mechanical ventilation in newborns.

OBJECTIVE: Air leak syndromes including pneumothorax, pneumomediastinum and pulmonary interstitial emphysema are frequent in neonatal period. Mechanical ventilation with positive pressure is one of the most common causes of these syndromes. The aim of this study was to evaluate predisposing factors and incidence of pneumothorax in newborns under mechanical ventilation. METHODS: This descriptive cross sectional study was performed in 400 newborns under mechanical ventilation in intensive care unit of a teaching hospital in Iran from April 2004 to December 2008. Predisposing factors leading to

Complications of mechanical ventilation in the pediatric population.

BACKGROUND: Mechanical ventilation (MV) strategies are continuously evolving in an effort to minimize adverse events. The objective of this study was to determine the complications associated with MV in children. STUDY DESIGN: Prospective observational study. Over a period of 10 consecutive months, 150 patients (median age 0.8 years, IQR 4.4, 59% male) were enrolled in this study. RESULTS: The median duration of MV was 3.1 days (IQR 3.9). A total of 85 complications were observed in 60 (40%) patients (114 complications per 1,000 ventilation days). 16.7% of patients developed atelectasis, 13.

Interfaces and humidification for noninvasive mechanical ventilation.

During noninvasive ventilation (NIV) for acute respiratory failure, the patient's comfort may be less important than the efficacy of the treatment. However, mask fit and care are needed to prevent skin damage and air leaks that can dramatically reduce patient tolerance and the efficacy of NIV. Choice of interface is a major determinant of NIV success or failure. The number and types of NIV interface has increased and new types are in development. Oronasal mask is the most commonly used interface in acute respiratory failure, followed by nasal mask, helmet, and mouthpiece. There is no perfect N

[Patient-noninvasive mechanical ventilation interaction].

Noninvasive mechanical ventilation is one more step in the treatment of patients with acute respiratory failure. In addition to gas exchange disorders, its primary indication to initiate it is the presence of signs of respiratory muscles fatigue. To assure successful mechanical ventilation, the ventilator and patient must be synchronized, that is, the effort the patient makes to start inspiration is recognized by the ventilator and it quickly delivers gas flow, that the flow provided by the ventilator adapts to the flow need of the patient during delivery of gas phase and that the ventilator r

Home mechanical ventilation in Hong Kong.

Home mechanical ventilation (HMV) is increasingly used to treat chronic respiratory failure. This present study was aimed to examine the trend, the disease categories treated and the outcomes of HMV use in Hong Kong. In the year 2002, all adult respiratory units in Hong Kong were invited to report to a multicentre retrospective survey of HMV use. A total of 249 patients (156 males, mean age 62.7 +/- 13.8 yrs) were treated since 1980, with 197 (79%) continuing with HMV at the time of the survey. Cumulative number of HMV grew as a cubic function of time since 1980. Currently, there are 2.9 users

Causes of failure of noninvasive mechanical ventilation.

With selected patients noninvasive positive-pressure ventilation (NPPV) can obviate endotracheal intubation and thus avoid the airway trauma and infection associated with intubation. With patients who can cooperate, NPPV is the first-line treatment for mild-to-severe acute hypercapnic respiratory failure. NPPV is also used for hypercapnic ventilatory failure and to assist weaning from mechanical ventilation, by allowing earlier extubation. Some patients do not obtain adequate ventilation with NPPV and therefore require intubation. Also, some patients will initially benefit from NPPV (for one-t

Liberation from prolonged mechanical ventilation.

After weaning from PMV, patients are usually far from ready to resume normal activities. A prolonged recovery period after catastrophic illness is the rule, with multidisciplinary rehabilitation and discharge planning efforts. Following such efforts, reports of success of restorative care are institutional and population specific. That all PMV patients are not "chronically critically ill" introduces selection factors that make comparisons between institutions even more difficult. Half of the authors' patients were able to go home in past years [14], although more recently, with patients admitt

Newer techniques of mechanical ventilation: an overview.

The introduction of newer, state-of-the-art, microprocessor controlled ventilator systems provides clinicians with opportunities to apply a number of advanced ventilatory modalities which were not previously available for treating newborns. Some of these techniques will need further scientific evaluation in controlled trials, but this should not preclude their use in clinical settings, as their safety has already been proved by "standard setters" for use in neonates. There is a firm physiological rationale for their use, and individual centres have already acquired substantial experience in th

Recent advances in intubation and mechanical ventilation.

The ideal care of the critically ill infant or child should demonstrate a seamless service starting with recognition of potential critical illness and initiation of early resuscitative interventions, escalating to advanced life support and skilled transfer to an appropriate location where intensive care can be continued. This article discusses advances in the package of respiratory care commencing with the decision to provide respiratory support, which includes the decision whether or not to intubate, leading to the choice of mechanical ventilatory support mode in the critically ill child. Mechanical ventilation for children with status asthmaticus.

Status asthmaticus is the most common diagnosis leading to hospital admission in childhood. Most of these patients respond well to oxygen, steroids, beta-agonists, anticholinergics, and other medications. The few patients who develop respiratory failure from severe status asthmaticus pose many challenges to those supporting them in the intensive care unit. This Article discusses the pathophysiology of asthma and how that relates to appropriate ventilator management. Alternative therapies are briefly discussed.

Pharmacologic adjuncts during mechanical ventilation.

Despite recent advances in technology, the mortality rate for patients suffering from adult respiratory distress syndrome remains in the range of 40-50%. This high mortality rate may be in part related to complications from ventilator management, such as ventilator-induced lung injury. In these patients, adjunct therapies aiming at ameliorating ventilator-induced lung injury are being developed. This article discusses the rationale for use of pharmacologic adjunct therapies, including inhaled nitric oxide, surfactant replacement therapy, antioxidants, prostaglandins, and corticosteroids, in pa

Mechanical ventilation as a bridge to lung transplantation.

Data describing the use of ventilation as a bridge to lung transplantation are scant. However, data from the International Registry suggest that patients who are ventilated at the time of transplantation are at increased risk. The decision to offer invasive ventilatory support to a lung transplant candidate with acute respiratory failure should be individualized and based on variables that include likelihood of expeditious transplantation, and the presence of a reversible superimposed process. A trial of NPPV is justified in patients who present in acute respiratory failure, but is more likely

[Nasopharyngeal myiasis during mechanical ventilation].

We report a case of myiasis caused by Phaenicia sericata during mechanical ventilation. An 86-year-old woman with bronchiectasis was admitted to our hospital with severe respiratory failure. Treatment with mechanical ventilation and sedatives was initiated. On the 10th day of hospitalization, about 20 white larvae were found in the patient's oral or nasal cavities. The larvae were removed and identified as Phaenicia sericata. No mucosal injury was found in the patient's oral or nasal cavity by endoscopic examination. The patient died of multiple organ failure caused by sepsis that had no assoc
